2 What is Annual Enrollment? Once a year, benefit eligible associates can adjust their insurance coverage without having a qualifying life event. A Wallick benefit eligible associate is classified as a full-time associate (30 or more scheduled work hours) 2018 Annual Enrollment Dates Monday, Dec 4, 2017 ~ Friday, Dec 15, 2017
3 Am I Required to Re-enroll? Yes, this year we are holding a mandatory ACTIVE Annual Enrollment. This means every benefit eligible associate must take action; you must either enroll in or waive every Wallick benefit offered If you do nothing, you will NOT be covered by Wallick s insurance in 2018!!!

16 Wallick Paid Health Fund Contribution After you have used the Health Fund dollars paid by Wallick, you will be responsible for the remainder of the deductible: Remaining deductible for single - $2,250 Remaining deductible for family - $4,500 Unused Health Fund Dollars will rollover to the next year decreasing your out-of-pocket deductible.

24 Voluntary Benefits Additional Life & AD&D Insurance Coverage can be elected up to the guaranteed amount without having to provide proof of good health. Guaranteed coverage amounts for each benefit: Associate: $100,000 / Spouse $25,000 / Children $10,00 Proof of good health is required if you apply for coverage over the guaranteed amount.
25 Voluntary Benefits Short Term Disability (STD) Insurance Short term disability insurance pays a portion of your salary if you are unable to work due to a covered disability. You will receive 60% of your base weekly salary to a maximum of $1,000 per week. You will continue to receive short term disability payments for up to a maximum of 12 weeks.
