2 2016 Highlights As a group, Graco lost a total of 3,130 pounds. 1,361 employees were screened. 304 employees have improved their blood pressure by at least one category since their last screening. 97% of employees were satisfied with their screening experience. Opportunities There was a 1.3% increase in employees with a BMI of 30 or greater (obese category). 353 employees had a glucose level within the range of Prediabetes or Diabetes. Only 43% of employees had an LDL cholesterol level within the desirable range (under 100mg/dL). Graco Wellness

Complete Bravo Registration online at Complete KnowYourNumber Health risk assessment questionnaire at Complete a Health Screening. This can be done on-site at Graco during the scheduled dates (see below), by a personal healthcare provider, or at a Retail Clinic. If the screening is done by a personal healthcare provider or Retail Clinic, inform the provider the visit is for an employer wellness screening. 4 2017 Wellness Program Welcome to year nine of Graco s Wellness Program! Bravo Wellness continues to administer Graco s Wellness Program. This ensures confidentiality and protection of your health information. Bravo does not share your personal health information with Graco; Bravo only communicates to Graco how your results impact your premium discounts or company funded Health Savings Account (HSA) contributions. This program is voluntary and requires annual enrollment for both employees and spouses. 75% of all healthcare costs are spent treating conditions that are lifestyle related and preventable. The purpose of Graco s Wellness program is to improve the health of employees and spouses by offering a program that promotes healthy lifestyle choices and disease prevention which drives employee engagement and productivity. Employees and spouses that voluntarily choose to enroll in Graco s Wellness Program can earn points towards premium discounts or company funded HSA contributions Premium Discounts or Contribution to Health Savings Account (HSA) 2017 Premium Discount Levels Number of Wellness Points % Discount on Monthly Premiums $ Contributions to HSA Blue % $ Bronze % $$ Silver % $$$ Gold % $$$$ Platinum 90 40% $$$$$ HSA contribution amounts to be announced this fall In order to ensure that our employee only or employee + children participants have the same opportunity to earn points at the same rate as our employee + spouse or employee + family participants, we have allotted similar 15 points achievement categories. Specifically, if you are an employee only or employee + children participant you can earn 15 points for completing a health risk assessment and a health screening (biometric). Respectively, if you are an employee + spouse or employee + family participant, you can earn up to 15 points: 10 points for your spouse completing both a health risk assessment and a health screening (biometric), and 5 points for negative spouse tobacco use. Effective January 2017, the Equal Employment Opportunity Commission (EEOC) has created new regulations for Wellness programs. This will impact the maximum discount allowable for our Family plan if the spouse chooses not to voluntarily participate. If you re ready to enroll, review page 10 for enrollment instructions. 6 Page Wellness Program Details at a Glance: Online wellness program registration and appointment scheduling for on-site health screenings begins June 2, 2016 at Online scheduling will close June 30. Employees who wish to enroll in the wellness program and receive premium discounts or company funded HSA contributions beginning January 1, 2017 must complete all enrollment requirements by Friday, October 28, Bravo will accept health screening (biometric) results dating back to May 1, Participants will receive a results letter and health report from Bravo within four to six weeks after they complete all enrollment steps and submit health screening results to Bravo. See the Wellness Resources on page 9 to learn how to improve your health and prevent disease. Appeals and Alternative Goals: A participant who improves one or more health factors during the 2017 program year, and submits their results to Bravo by 7/31/17 may increase their premium discounts or receive additional HSA contributions for the remainder of the program year. Visit Graco s Wellness website to obtain the Bravo Type 3 Appeal Form and to learn more about type 3 appeals. Participants who think they may be unable to meet a standard for a reward under this wellness program may qualify for an opportunity to earn the same reward by different means. Contact Bravo within 30 days of receiving your results letter and they can work with you (and, if you wish, with your doctor) to establish an alternative goal with the same reward that is right for you based on your health status. Employees who don t achieve Graco s goal or alternative goal before the deadline indicated on their results letter, but achieve Graco s goal or an alternative goal by 7/31/17 will receive discounts or additional company funded HSA contributions for the balance of the plan year. For all goals, in accordance with The Affordable Care Act (ACA) Wellness Regulations, Bravo manages the appeal process and the completion of personalized reasonable alternatives if the program goals or alternative standard goals are medically inadvisable or unreasonably difficult due to a medical condition. Contact Bravo within 30 days of receiving your results letter to initiate an appeal. Get Out and Get Moving! Do you belong to a league or participate in activities similar to 5K walks/runs? To support Graco employees in their active lifestyle pursuits, the Wellness Program offers Get Out and Get Moving an initiative to reimburse up to $45 per year for employees registration fees or memberships to participate in organized group physical activities, events, and competitions such as some sports leagues, runs, walks, and bike rides. 8 Spouse Participation Instructions Graco s Wellness Program provides spouses on Graco s medical plan an opportunity to voluntarily participate and earn up to 15 points towards premium discounts or company funded HSA contributions. Enrollment consists of the following: Bravo registration (required). Go to www. bravowell.com/graco Completion of both a health risk assessment (HRA) and health screening (biometric) 10 points Tobacco free (Acknowledged on Bravo registration form) 5 points Spouse health screening (biometric) results must be submitted to Bravo Wellness by Friday, October 28, 2016 in order to apply towards premium discounts or HSA contributions beginning January 1, In order for spouses points to apply towards premium discounts, employees must complete annual enrollment. Personal health information is kept confidential and will not be shared with Graco. *Please note that the medical plan opt-out credit is not available to married couples who are both employees of Graco. 9 2017 Wellness Activity Calendar New Year-New You challenge begins. January April February is American Heart Month. March is National Nutrition Month: Oats in a Cup event! Preventive health screenings are 100% covered. Get your BP and cholesterol checked! June is National Safety Month. May August July-August On-site Biometric Screening for 2018 plan year begins. 7/31/17 Deadline to submit health status update to Bravo. Graco 5K at Minnesota Company Picnic in Anoka. Fall Into Fitness challenge begins. 9/29/17 deadline to submit for Annual Wellness Recognition Awards. September December October is National Breast Cancer Awareness Month: Schedule On-site Mammogram. On-site Flu Shot Clinics in October. Healthy Holiday Eating ideas! Drunk Driving Awareness! Check out Safety Smart Trivia! Visit the Graco Village on the first and last Monday of each month for Safety Smart Trivia which provides the opportunity to test your knowledge of safety rules and smart safety practices. Answer the trivia question and check back the next day for the correct answer and percentage of correct responses. Get fit. Get Rewarded! Did you know that you can receive a $20 discount when you visit your health club eight times or more per month? You can receive a maximum of two discounts ($40) per family each month (spouse or child 18 years or older). See the Resources page of the Graco Wellness website for more information about BlueCross BlueShield Gym and Fitness Center Discounts and how to locate a participating health club or fitness center. 10 2017 Wellness Challenge Drawings and Annual Wellness Recognition Awards Open to both employees and spouses! Wellness Challenge Drawings: A drawing for four $250 cash prizes for employees and two $250 cash prizes for spouses will be held semi-annually for those who participate in the two annual wellness challenges. The New Year-New You challenge will take place in January 2017 and the Fall Into Fitness challenge will take place in September Annual Wellness Recognition Awards: Employees who achieve a significant wellness goal and are willing to share their success story can submit an application. Annually, $1,000 will be awarded to one employee winner and $250 will be awarded to four employee honorable mentions. $500 will be awarded to one spouse winner and $125 will be awarded to four spouse honorable mentions. The deadline to submit your Wellness Recognition Award application is September 29, The Graco Wellness Champions, which is made up of employee representatives, will review eligible applications. The application form can be found on the Graco Wellness website. The success stories of the award winners and possibly other applicants will be shared with the company. 11 Wellness Resources Nutrition Counseling: Graco s BCBS plans offer 100% coverage on nutrition counseling services. Viverant offers individualized nutrition counseling services to Graco employees and covered dependents in order to improve their long term health. Call , option 6, and let them know you are a BCBS member covered under Graco s medical plan. Weight Management: Up to $50 reimbursement for three-month paid membership to Weight Watchers online, or Weight Watchers meetings pass, if 90-day weight loss goal is met. See Graco s Wellness website, or call for the Weight Watchers Reimbursement form and more details. Has a BMI calculator and tools to help achieve and maintain a healthy weight for a lifetime. Heart Health: Sign up for heart healthy text messaging, access to > 200 fact sheets on heart disease, and many heart education videos to watch. Click on Conditions, check out the Blood Pressure Risk Calculator, and information on the risk, prevention, and treatment of high blood pressure. Diabetes (Glucose): Provides new recipes, cooking tips, and monthly meal planning. Has information on preventing and managing diabetes. Tobacco Cessation: BlueCross BlueShield Stop-Smoking support program for members provides nicotine replacement therapy (patch, gum, lozenge) at no cost. Call BLUE(2583). Has information and resources to help you prepare to quit and support you in the days and weeks after you quit. Chronic Health Conditions: Employees with a chronic health condition may qualify for the BlueCross BlueShield (BCBS) Chronic Condition Management program. Contact BCBS at Employee Assistance Program: 24 hour/day professional EAP counselors available to help you sort through life s challenges by calling , or visit bluecrossmn.com/eap. Has online EAP resources. Volunteering & The Graco Foundation Dollars for Doers: Each year, Graco employees volunteer many hours of their personal time to help their favorite non-profit causes. These efforts can become even more valuable with a supplementary financial gift from The Graco Foundation. In an effort to recognize and support these important and worthwhile volunteer hours, Graco will donate $1,000 to any eligible nonprofit organization where an eligible employee has volunteered 50 hours or more during one calendar year. Time that a spouse or dependent of a Graco employee volunteers may be counted toward the 50 hours. Check out the Graco Foundation website under Community on the Village. Did you know you and your spouse can earn $50/each for having your biometric screening if you are enrolled in the Critical Illness or Accidental Injury coverage? That s up to $200 total if enrolled in both plans! More information on this benefit can be found at mybenefits.graco.com. Bravo Page 9

12 Enrollment Instructions 1. Go to 2. Log in with your Username and Password, or Create an Account. If you are a returning user, select Log in and enter your Username and Password. If you have forgotten your Username and Password, help links are available to retrieve or reset your information. If you are creating an account for the first time, select Create an Account to begin the registration process. 3. Registration Icon: Click on Get Started. Please read and complete the form. You must complete this step in full before you can complete the Know Your Number (health risk assessment questionnaire) or schedule a screening appointment. Click Submit. 4. Know Your Number Icon: Click on Get Started and complete the questionnaire. Click Submit when completed. 5. Health Screening Icon: Click on Get Started. A listing of screening locations will appear. Select the location, date, and time most convenient for you and click Book Appointment. If you provided an address, a confirmation will be sent to you with the time you selected, as well as a link to change or cancel your appointment. If you re completing the health screening with your healthcare provider or at a retail clinic, you may download and print the Provider Screening Form during this step. When you are finished, be sure to click the Logout button. This will help protect your confidential information.
