1 To: From: Re: ALL Full-Time Iredell County Government Employees Theresa Patton, HR Benefits Specialist 2016 Section 125 Benefits Enrollment Date: April 12, 2016 READ THIS MEMO IN ITS ENTIRETY Once again this year, an open enrollment benefits video will be available to you through MUNIS ESS and it MUST BE viewed between April 18 th - April 29, To access the Open Enrollment Video, login to MUNIS ESS. Click on the Resources tab in the upper right hand corner. Select Benefit Information and click on the Open Enrollment Video link. You will review the videos and then sign the Video Roster that your Department Director or Supervisor has. Please Note: Computers MUST have Microsoft XP or higher to access MUNIS ESS. No computer access? A computer will be available in the Human Resources Department, you will need to bring your own ear buds. The Iredell County Library will also have computers available (take your own ear buds go to the upper level reference desk and identify yourself as a County employee), or check with your Department Head for computer access at your location. You should have already received the results from your Health Assessment Screening. Have the results in hand when you watch the videos. Like last year, Open Enrollment will be conducted through MUNIS Employee Self Service (ESS). Detailed instructions for using the Open Enrollment site on ESS are attached. If you list an address in MUNIS ESS, we will be able to contact you with reminders and updates to the open enrollment process as they occur. Beginning Monday, May 2, 2016 through Tuesday May 31, 2016 you may access MUNIS ESS to perform the following: Review current benefit elections and make changes. (Changes made during the current plan year will not show, nor will any voluntary benefits you currently hold.) If you were a new hire during FY 15/16, you must make new elections. Be sure to verify that your personal information is correct and update as needed e.g., address, phone number, etc. (this is important for ID cards to be mailed to the correct address). Add and modify family information (SSN s and DOB s are required for spouse and children s insurance coverage). Verify your beneficiaries on file are still current by calling Human Resources at or theresa.patton@co.iredell.nc.us. You MUST enroll in the following benefits through the MUNIS ESS site. Cigna Health Plan(s) PPO Plan & High Deductible Plan Health Savings Account (only available with enrollment in the High Deductible Plan) Ameritas Dental Community Eye Care Comprehensive & Eyewear Plans Gilsbar Medical & Dependent Care Flexible Spending Accounts (MUST re-enroll if you wish to participate beginning July 1, 2016) 1

2 Mark III Benefit Counselors will be available Monday, May 9th through Friday, May 13th (see the attached schedule) to answer questions and to enroll in benefits that you may be interested in. You WILL be required to see a Mark III Benefits Counselor if you want to add or make changes to the benefits listed below. These plans cannot be changed or added through the MUNIS ESS site: AUL Short-Term Disability AUL Long-Term Disability Assurity Cancer (cannot enroll in this benefit, but it remains on payroll deduction) Allstate Cancer AFLAC Accident Dearborn Term Life - Basic Dependent & Voluntary Coverage(s) Texas Life Whole Life Zurich Accident REMINDERS: No appointment is necessary to visit with a Mark III Counselor. To ensure that you have ample time to make a decision about your benefits, DO NOT WAIT UNTIL THE LAST MINUTE to visit with a Mark III Counselor or to go to MUNIS ESS. The schedule for the Mark III enrollers is attached to this letter. The open enrollment video must be reviewed on or before Friday, April 29, If you need assistance with open enrollment selections through the MUNIS ESS, please contact the Iredell County HR Department at or DO NOT WAIT UNTIL THE LAST MINUTE TO ENROLL. BENEFITS OVERVIEW: Cigna Open Access PPO & High Deductible with HSA (Health Savings Account) Health Plans (MUST enroll through the MUNIS ESS site): Our Health Plan premiums will be increasing 8% this year. The rates listed are only tentative and are subject to change until the Board of Commissioners make a final decision. There will be no plan design changes this year. Tentative Semi-Monthly Rates: Wellness Participant Rates Employee Only: $0 Employee/Spouse: $ Employee/Child: $ Employee/Children: $ Employee/Family: $ Pay Periods Non-Wellness Participant Rates Employee Only: $30.00 Employee/Spouse: $ Employee/Child: $ Employee/Children:$ Employee/Family: $ ***$30 per non-participant on the plan*** REMINDERS: Open Access PPO (co-pay) Plan: Co-pays (excludes pharmacy co-pays) apply towards the out-of-pocket maximum. Out-of-pocket maximum on the Family Plan is $10,000. Specialists visits are subject to the deductible (owe full cost until the deductible is met). There is a $150 individual/$300 family separate pharmacy deductible that must be met before the Tier 1, Tier 2, Tier 3, and Tier 4 benefits are paid. 2

3 REMINDERS: High Deductible Plan (HSA): CAREMARK is the Administrator of the prescription coverage. The County will continue to contribute $750 towards the HSA account for the FY Plan Year. The $750 contribution will be deposited to your HSA account on July 1, You may also contribute additional funds on a pre-tax basis to your account to pay for your medical expenses. The total amount (including the $750 that the County will contribute) for the plan year that you can put into the HSA as an individual is $3,350 or $6,750 if family coverage is selected. An additional $1,000 can be contributed if an employee is over age 55. If you choose the High Deductible Plan with HSA for the first time, you will be contacted by HR after Open Enrollment to set up your HSA account. Gilsbar Health & Dependent Care Flexible Spending Accounts (MUST enroll through the MUNIS ESS site): - No Plan/Rate Changes You MUST re-elect your Health Care and/or Dependent Care account each year if you wish to participate. Remember that the Health Care & Dependent Care Spending Account balances DO NOT carry over from one year to the next. Any unused balance (besides the new rollover) will be forfeited under the IRS regulation, "Use it or Lose it". If you terminate employment with Iredell County, you will have 90 days to submit any claims you incurred PRIOR to your termination date. It is also important for you to retain all receipts during the plan year in the event that Gilsbar or the IRS asks for verification of a purchase. The entire Health Care amount elected can be used on the first day of the new plan year. For 2016 your maximum contribution to your healthcare flexible spending account is $2,550. The minimum is $ For dependent care the max remains at $5,000. As a reminder with the Dependent Care Flexible Spending Account, you must have the exact dollar amount in your account that you wish to get reimbursed for. Ameritas Dental (MUST enroll through the MUNIS ESS site): - No Plan/Rate Changes Dental coverage you may elect for you and your family. o As a reminder, the age limit for dependents is up to age 26. Community Eye Care (MUST enroll through the MUNIS ESS site): - No Plan/Rate Changes You may choose between the Eyewear Only Plan and the Comprehensive Plan. o As a reminder, the age limit for dependents is up to age 26 You must be actively at work to elect or change coverage for the following benefits. Also, you must see a Mark III Benefits Counselor: There are no Plan/Rate changes to any listed below. AFLAC Critical Illness The Critical Illness plan can reduce the financial stress of a major illness by putting money directly into your pocket when you need it the most. With the diagnosis of a covered, illness such as heart attack, stroke or kidney failure, you can receive a check for the benefit amount that you selected when you applied for the plan. This plan also includes a wellness screening benefit which allows you to collect $100 each calendar year if you have specified tests performed. The wellness screening benefit will also apply to your spouse if covered on the plan. AUL Short-Term Disability (STD) The Short-Term Disability is designed to protect your salary if you are out of work due to a sickness or an accident. The AUL STD coverage applies to on and off the job accidents. You can receive pay for short-term disability in addition to sick leave and annual leave. 3

4 AUL Long-Term Disability (LTD) Long-Term Disability is designed to protect you from loss of income in the event that you are unable to work due to illness, injury, or accident for a long period of time (5 years will be the maximum length of time you will get paid from this Policy). There are stipulations to the amounts you may elect if you did not apply for coverage when it was first offered. The monthly income selected cannot exceed 60% of your gross salary. There is a lump sum benefit of $10,000 attached to the LTD that will pay directly to you when you are placed out of work by a physician. Allstate Cancer This supplemental plan is designed to put money in your pocket if you ever have to battle cancer. The Plan also provides coverage for 29 specified diseases and is portable at the current price if your employment ends. Another great feature of the cancer plan is the annual $100 wellness benefit that each person on the Plan can take advantage of when specified tests are performed by a physician. Assurity Cancer The County will allow you to keep this benefit on payroll deduction, so that you will continue to reap the pre-tax savings. Assurity was the previous Cancer provider before Allstate. This applies to those employees who were grandfathered in under this Company. AFLAC & Zurich Accident Plans (AD&D) The Accident Plans are guaranteed issue and assists you with a cash benefit for accidents, hospitalization, broken bones, fractures, and emergency treatment just to name a few. These plans provide on & off the job coverage and will pay regardless of any other insurance you may have. You are able to apply for Accident coverage on your spouse and children as well. Dearborn Term Life Term Life is an inexpensive means of having life insurance while employed at Iredell County. At no cost to you, the County will provide your named beneficiaries with one times your annual salary in the event of your premature death. You may also apply for Basic Dependent Term Life Coverage of $10,000 on spouse and/or children. You can also purchase additional Voluntary Term life coverage on yourself, spouse and dependents at minimal cost. Please check to make sure that you have named the appropriate beneficiaries for your term life coverage by calling Theresa Patton in Human Resources at or by at theresa.patton@co.iredell.nc.us. Texas Life Solutions 121 Whole Life Texas Life Solutions 121 allows you to address both your current life insurance needs and the ones that will continue after you retire or change jobs because it is fully paid up at age 65 (or after 20 years if you are 46-years of age or older). As importantly, you qualify for coverage simply by being actively at work, performing your usual duties and answering a few health questions the day you apply. Additionally, you can even apply for coverage on members of your immediate family (spouse, children and grandchildren). 4

5 You can find the FY Benefits Booklet two ways: 1. One way is on the Iredell County website at click the Departments tab and go to Human Resources, and click on the ESS Resources tab to locate the benefits booklet. 2. The other way is to log in to Employee Self Service (ESS), click on the Resources tab at the right of the screen under the Iredell County Logo. Click on Benefit Information to locate the benefits booklet. Once the updated FY Benefits Booklet is available, it will replace the FY Benefits Booklets. This letter should serve as a reference for the FY updates until the Benefits Booklet is available. Should you have any questions or need any further information, please feel free to contact the Human Resources Department at or You may also contact Kathy Yount (Mark III Brokerage), at WELLNESS PROGRAM CHANGES: Biometric Screening: We are excited to provide up to a $500 wellness incentive for employees who meet 4 out of 5 in regards to the following biometric measurements: blood pressure, waist circumference, cholesterol ratio, blood glucose, and the absence of tobacco use. Please refer to the chart below. Incentive #1: Employees meeting 3 out of 4 of the criteria below are eligible for a $250 incentive. Risk Factor Criteria for Additional Reward Blood Pressure < 138/< 86 mm/hg Cholesterol Ratio < 4.5 Blood Glucose Fasting Glucose < 100 or Hemoglobin A1C < 6.5 Tobacco Tobacco Free Incentive #2: Employees meeting the criteria listed in the box below are eligible for a $250 incentive. Risk Factor Criteria for Additional Reward Waist Circumference <40 Male or <35 Female Nurse Visit Requirements: If you have two or more risk factors above you will be required to meet with the Wellness Nurse and/or complete wellness activities quarterly in order to qualify for the wellness discount of $30 per pay period. Employees who are identified as having two or more risk factors for chronic disease as assessed through this process are required to meet with the Wellness Nurse each quarter in order to receive the wellness discount. This year you have the option to participate in a wellness activity offered through the Wellness Program for two of the quarters instead of meeting with the Wellness Nurse, if you prefer. Your health plan is committed to helping you achieve your best health status. Rewards for participating in a Wellness Program are available to all employees. If you think you might be unable to meet the standard for a reward under this Wellness Program, you might qualify for an opportunity to earn the same reward by different means. Contact Sarah Williams at (704) and we will work with you to find a Wellness Program with the same reward that is right for you in light of your health status. 5

6 What if I miss a quarterly Nurse visit? If you miss a quarterly visit with the Wellness Nurse you will be charged $25 per pay period (total of $150 for the quarter) for each quarterly visit that you miss. The charge will begin being deducted at the start of the following quarter. You will only be charged for the number of quarters that you miss your appointment. For two of the quarters, you may choose to participate in a wellness activity offered through the Wellness Program instead of visiting with the Nurse. 1 st Quarter: July 2016 September nd Quarter: October 2016 December rd Quarter: January 2017 March th Quarter: April 2017 June 2017 How much will I be charged if I decide not to participate in the wellness program at all? You may choose to opt out of the wellness program entirely. To do this, you will be required to pay $30.00 per pay period for employee only health insurance for the entire fiscal year. You will not be eligible to opt in again until July 2017 (you will need to complete an HRA in February 2017). Dependents and Retirees: This year the Wellness Program will continue to be open to spouses and dependents over the age of 18! The requirements will differ from those for employees. Spouses and dependents who elect to take the County s Health Insurance will need to complete an online Health Assessment Questionnaire as well as see their Primary Care Physician for a wellness exam in order to receive the discounted premium. This applies to retirees and their dependents as well. Be sure to tell the doctor s office it is a wellness check-up so the charges are paid at 100%. Remember that if you DO NOT participate in the Health & Wellness Plan, as an employee, you will pay an additional $30 per pay period for the PPO and/or the High Deductible with HSA health coverage that you select. The higher premium will apply for any spouses or dependents who opt not to participate (the fee will be per non-participant). 6
