1 Summary of Health Benefits Effective January 1, 2017 At AVT, we do everything possible to ensure our employees enjoy a comprehensive benefits package which meets a wide variety of needs. Our Employee Benefits Program is reviewed annually and may be adjusted in order to continue providing a competitive, well-rounded program. Following is a brief summary of all of our Health Benefit Programs. Please refer to the Summary Plan Descriptions for each plan for more specific coverage details, limitations, and exclusions as this document is only a brief summary. As a regular full-time employee, you are eligible to participate in the company s health plan beginning on your 1 st day of employment. Your benefits include a Medical Plan with prescription drug benefits, a Dental Plan, a Vision Plan, Group Term Life and Accidental Death & Dismemberment (AD&D) insurance, Short and Long Term Disability insurance, the option to purchase additional Voluntary Term Life & Voluntary AD&D for you and your family members and the option to purchase Supplemental insurance (Accident, Cancer, MedicalBridge, & Critical Illness). Your benefits also include a cafeteria plan to pay for your eligible choices with pre-tax dollars which include two flexible spending accounts (Health Care FSA and Dependent Care FSA). The Medical, Dental, & Vision plans are packaged together. In order to receive Dental and/or Vision benefits, you or your family members must be enrolled in the Medical Plan. However, all full-time employees (regardless if you are declining medical insurance) will receive company provided Life/AD&D insurance, Short, and Long Term Disability insurance. 4 Vision Plan (Policy # ) Carrier Vision Service Plan (VSP) Member Services (800) Routine Vision Exam 1 every Calendar Year; $10.00 Co-pay Lenses 1 set every Calendar Year; $30 Copay Frames 1 every other Calendar Year Elective Contacts $130 allowance per pair (in lieu of lenses/frames) Please refer to the list of participating vision providers through Vision Service Plan. You may also call VSP at , 6:00 a.m. 6 p.m. EST, Mon Fri. To ask questions via , view plan information, or find a preferred provider in your area, go to The network frame allowance is $130 which will cover most frames in full. If you select a frame which costs more than $130, the plan will cover 20% of the amount above the allowance and you will pay the rest for in network providers. Examples of materials not covered include, coating or laminating of the lens or lenses, blended, oversize, progressive multifocal, cosmetic, or UV protected, photochromic and tinted lenses (except pink 1 & 2). After you have paid your copay, the remaining cost for standard lenses and frames (single vision, bifocal, trifocal, or lenticular) are covered in full when utilizing a panel doctor. However, if you utilize a non-panel doctor, you will be reimbursed at the Non-Panel Doctor Reimbursement Schedule. Your contact lens exam will not be counted against your $130 allowance. You will be responsible for up to a $60 maximum copay for your contact lenses exam in addition to your $10 exam copay. If you utilize a network provider, you will also be eligible for a 15% discount for the contact lens exam. For example, if your network provider charges $55 for the contact lens exam, you will pay $56.75 (which includes 15% off of the $55 contact lens exam plus your $10 exam copay). You will also have a full $130 to be used towards your contact lenses. Mail All Out of Network Vision Claims to: VSP P.O. 5 Survivor & Disability Insurance BASIC GROUP TERM LIFE / AD&D INSURANCE (100% Paid by AVT) As a full time employee, you are covered for a Basic Life benefit equal to one times (1X) your annual base rate of pay up to a maximum benefit of $50,000 and an additional AD&D benefit equal to your Basic Life benefit. Your amount of Basic Life/AD&D coverage is subject to reductions based upon your age (age 65 benefit reduce by 35%, age 70 benefit reduce by 50%). If you are terminally ill, you may be eligible to receive up to 100% of your basic life benefit while you are still alive. All full time employees will receive Basic Life/AD&D regardless if you decline health insurance. Other additional benefits include riders for Common Carrier Accident, Repatriation, Seatbelt & Airbag Riders, and Child Education. Please refer to Summary Plan Description for amount of eligible coverage on these additional benefits. This benefit is provided at no cost to you! VOLUNTARY TERM LIFE INSURANCE AND VOLUNTARY AD&D INSURANCE (100% Paid by Employee) You have the opportunity to elect Voluntary Term Life coverage at competitive group rates conveniently paid through payroll deductions. Optional Coverage is available for you in the amounts of one, two, three, or four times your salary rounded to the next $1,000 to a maximum benefit of $500,000. Your spouse is eligible for up to 100% of employee amount in increments of $5,000, not to exceed $100,000 of Life coverage. Your children are eligible for up to 100% of employee amount in $5,000 increments, not to exceed $10,000 of Life coverage. Employees must participate in the Voluntary Term Life plan for family members (spouses and/or children) to participate for Voluntary Term Life. Guarantee Issue: The first $100,000 of additional Term Life coverage on yourself is offered without providing evidence of insurability (answer medical questions) and up to $50,000 on your spouse if you enroll within the first 31 days of your eligibility date. Please refer to the information provided for additional details on the Voluntary Term Life program. Additionally, you have the option to elect Voluntary AD&D coverage for both you and your spouse in the amounts of one, two, three, or four times your salary rounded to the next $1,000 to a maximum benefit of $500,000. The cost is $.04 per month per $1,000 benefit. Your spouse is eligible for up to 100% of the employee amount in $1,000 increments and your children are eligible for $10,000 AD&D coverage. Employees must participate in the Voluntary AD&D plan for family members (spouses and/or children) to participate for Voluntary AD&D plan. DISABILITY INSURANCE (100% Paid by AVT) Short Term Disability (STD) Insurance - Employees may be eligible for short term disability coverage if they become temporarily disabled due to an injury or sickness. After a thirty (30) day elimination period is completed, the short-term disability weekly benefit is 67% of the employee s basic weekly earnings, subject to a maximum weekly benefit of $1,400 and reduced by any other income. This benefit is provided to our employees at no additional cost. Long Term Disability (LTD) Insurance - Employees may be eligible for long-term disability benefits if they become seriously disabled and cannot work for longer than 180 days. Long term disability continues part of your income at a rate of 60% of your pay, up to a monthly maximum benefit of $9,000. This benefit is provided to our employees at no additional cost. This is only intended to provide you with a brief summary of the benefits. Please refer to your booklet for complete details.

6 Flexible Spending (Section 125 or Cafeteria) Plan Claims Administrator ADP FSA Services 800/ Flexible Spending Programs allow you to pay for some health and dependent care-related items as well as your insurance premiums without having to pay Federal & FICA taxes on the money you spend on those items. Employees may select this benefit on their date of hire or during the Company s Annual Open Enrollment Period. A new enrollment form must be completed for each Calendar Year. Generally, there are three components to a Flexible Spending Program (i.e. Section 125 or cafeteria plan). Premiums. The easiest way to participate in the flexible spending program is to pay your health insurance premiums on a tax-free basis. Health Care Spending Account. A Health Care FSA allows you to set aside pre-tax dollars to pay for medical, dental, and vision expenses incurred by you or your legal dependents that are not reimbursed under any insurance plan. The maximum you can contribute is $ per pay period which is equivalent to $2,600 per Calendar Year. Dependent Care Spending Account. A Dependent Care FSA allows you to set aside pre-tax dollars to pay for the dependent day care expenses you incur because you (and your spouse if you are married) are working or are full-time students. The maximum you can contribute is $ per pay period which is equivalent to $5,000 per Calendar Year. NOTE: If you are married and filing separately, your plan year maximum is $2,500. Box 1853 Alpharetta, GA or Fax: (toll-free) or The Section 125 plan runs on a calendar year basis beginning January 1 st and ending December 31 st and is governed by the IRS, therefore, it is subject to IRS rules and regulations. This is only a brief summary of the Section 125 benefits. Please refer to the ADP Benefit Services Website for more information on the flexible spending programs including important IRS rules and regulations. While the Section 125 plan runs on a calendar year basis (January 1 st to December 31 st ), the plan includes a 2 ½ month rollover feature. This means that eligible expenses incurred towards your FSA accounts through March 15 th may be applied to your prior calendar year s account.

7 Supplemental Insurance Supplemental insurance is designed to enhance, not replace your already existing coverage. Your group health insurance provides you with a comprehensive medical plan, but medical plans do not cover all of the expenses related to accidents and sicknesses, such as co-payments, deductibles, travel & lodging expenses, etc. At AVT we have selected standard supplemental policies; however, since these are individual policies, they can be customized to your individual needs. Benefits & rates may vary based on the enrollment location. You will be eligible for the benefits that match as closely as possible. BENEFITS ARE PORTABLE AT THE YOUR ISSUE AGE, even if you terminate your employment with your Employer Pays you directly, in addition to health insurance 30 day waiting period for Cancer, Critical Illness, & all Wellness Benefits ACCIDENT1.0 PREFERRED OFF THE JOB PROTECTION Employee, Employee/Spouse, Employee/Child(ren), or Family Premiums paid with Pre-tax dollars through employer Section 125 plan $50 Health Screening per covered person, per calendar year rider is included Pays $125 For Doctor s office, ER or Urgent care, $30 X-ray, & $50/visit (up to 3 visits) for follow-up care Covers Children in Sports Hospital Admission $1,000; plus $225/day ICU $2,000 plus $450/day for up to 365 days per accident Accidental Death Employee $25,000; Spouse $25,000; Children $5,000 Riders available for On the Job Accident, Sickness Hospital Confinement, & Accidental Disability CANCER 1000 PROTECTIO Employee, Employee/Spouse, Employee/Child(ren), or Family Premiums paid with pre-tax dollars through employer Section 125 plan $100 Cancer screening/wellness benefit per covered person, per calendar year Pays you income if cancer strikes you or your family with NO LIFETIME LIMITS Several local clients received over $50,000 during the past 12-months from this coverage Riders available for specified disease, lump sum initial diagnosis, and progressive payment CRITICAL ILLNESS Employee, Employee/Spouse, Employee/Child(ren), or Family Premiums paid with after-tax dollars $50 Health Screening per covered person, per calendar year You select lump sum benefit between $5,000 to $75,000 paid directly to you when stricken with heart attack, stroke, major organ transplant, kidney failure, coma, blindness, paralysis, & 25% for coronary bypass surgery; includes subsequent diagnosis Spouse coverage is available at 50% and children are available at 25% of face amount for name insured HOSPITAL INCOME /(MEDICAL BRIDGE3000 Employee, Employee/Spouse, Employee/Child(ren), or Family Premiums paid with pre-tax dollars through employer Section 125 plan $50 Wellness benefit per calendar year for employee only coverage; two per CYM for family coverage Lump sum payment of $1,000 per hospital confinement Lump sum payment for outpatient surgical benefit of $1,000 tier 1 and $2,000 tier 2 to a CYM of $3,000 Covers pregnancy (Policy must be in force 10 months) Please contact your benefits broker to discuss your enrollment location and riders you may want to include in your policy.

8 SUPPLEMENTAL INSURANCE PER PAY PERIOD (BI-WEEKLY) ILLUSTRATIVE PREMIUMS AVT has selected a standard benefit for the supplemental policies. Supplemental insurance policies are individual policies, therefore, factors such as your age, riders you select, and your enrollment location will impact the premium. These bi-weekly illustrations reflect participants enrolled in Texas. Actual rates will vary slightly due to rounding from monthly premium rates.
