2 Section 125 Cafeteria Plan Financial Benefit Services is the Third Party Administrator for Friendswood ISD. The plan year for Friendswood ISD is September 1 August 31 of each year. Although coverage is voluntary, every employee is required to review their current elections, make changes as desired, by completing the Friendswood ISD online enrollment in the THEbenefitsHUB. Any pre-tax elections will remain in effect unless you have a qualified event change in family status. Qualifying event benefit changes must be made within 30 days of the event and changes must be consistent with the event. Qualifying Event Examples: Marriage, Divorce, Birth or Adoption, Death, Change in Dependent Eligibility Status, etc. Your Benefits Website:

4 Health Savings Account What is an HSA? A Health Savings Account (HSA) works with a High Deductible Health Plan, ActiveCare 1 HD, and lets you set aside a portion of your paycheck before taxes into an account to help you pay for medical expenses before you reach your deductible or that aren t covered by your plan. What you need to know : H.S.A. Enrollment will be during July-August 2018 Annual Maximums: $3450 for Individual and $6850 for Family Your HSA stays with you, even if you change jobs. An HSA s balance will rollover every year. Unlike an FSA, contributions to an HSA are NOT up-fronted to you. You do not have to submit receipts for reimbursement (keep receipts for documentation for federal tax return). Only the balance in your account is available for you to use. At 65 you can withdraw your funds to use as you like. It becomes taxable income if you withdraw.
5 PPO Dental by Cigna High Plan Option NO WAITING PERIODS Yearly maximum benefit per covered person: $1250 Deductible is $50 per person Preventative treatment is covered at 100%. With 2 cleanings per year. NO $5 COPAY Basic Restorative Care is covered at 80%. Major Restorative Care is covered at 50% Orthodontia Is covered at 50% up to a life maximum of $1000 per dependent child to 19 Low Plan Option - Yearly maximum benefit per covered person: $750 Deductible is $50 per person Preventative treatment is covered at 100%. With 2 cleanings per year. NO $5 COPAY Basic Restorative Care is covered at 70%. Major Restorative Care is covered at 50% - 6 month waiting period NO ORTHO Visiting an In-Network Dentist saves you money. Out-Of-Network benefits are reduced Maximum dependent age is 26

8 Short Term Disability by Lincoln Financial Group Coverage is guaranteed issue up to 60% of your salary. Benefit Period is 90 days You may choose elimination (waiting) periods in days of: 8/8 or 15/15 Premiums you pay are based on the income benefit and elimination period you choose. 15 day elimination period includes 1 st Day Hospital Benefit - If you are hospital confined as an inpatient for at least 24 hours because of your disability, benefits begin on the first day of inpatient hospital confinement. Those employees who have been enrolled in disability at Friendswood ISD for at least 12 months will not be subject to pre-existing condition limitations New coverage and increases to coverage are subject to a 12-month pre-existing condition exclusion. 9 Long Term Disability by Lincoln Financial Group Friendswood ISD pays for the first 40% of covered earnings for all eligible district employees Employees may voluntarily elect to cover an additional 26.67% of their monthly earnings Those employees who have been enrolled in long term disability at Friendswood ISD for at least 12 months will not be subject to pre-existing condition limitations New coverage and increases to coverage are subject to a 12-month pre-existing condition exclusion with a 3 month look back period. 10 Medlink by APL Medical Supplement with High/Low Plan Options Hospital Admission - $1500/$2500 per occurrence per insured Out Patient - $500 per occurrence per insured o Includes ER, Urgent Care, MRI and Major Diagnostic exam, Out Patient Surgery Center, and Physician Treatment Physician/Specialist Office Visit - $25/$50 per visit up to 4 visits per individual or 8 max per family per plan year. 12 mo Pre-existing Condition Exclusion NOT applicable if not considered pre-ex under major medical If ActiveCare covers, APL will cover. 13 Critical Illness by UNUM Coverage is Guarantee Issue Employee $30,000, Spouse up to 100% of Employee amount, Children automatically enrolled at 100% of employee amount for no additional cost!! Provides a Lump Sum Benefit upon the Occurrence of Conditions such as: Heart Attack Stroke Blindness Major Organ Failure Paralysis Coma Alzheimers/Dementia ALS, MS, Parkinson s Childhood Conditions Cerebral Palsy, Cleft Lip or Palate, Cystic Fibrosis, Down Syndrome, Spina Bifida
14 Voluntary Life by One America Friendswood ISD provides eligible employees with $20,000 employerpaid life insurance. Voluntary Life Group Guarantee Issue: $250,000 for employees and $50,000 for spouses and $10,000 for children. Employees and spouses are eligible for up to $500,000 in $10,000 increments. Evidence of insurability (EOI) is required for amounts over GI Employee must cover themselves to cover a spouse or dependent child. Premiums are age-banded, or based upon your age. Coverage is convertible and portable. OneAmerica will honor any voluntary life amounts inforce that are above the guarantee issue amounts. Coverage can be increased by $10,000 during each subsequent open enrollment without EOI to GI amount.
15 AD&D by One America Accidental Death and Dismemberment pays a benefit for a critical or fatal injury caused by a covered accident and pays in addition to most other insurance available including disability or life insurance. Employees are eligible for up to $500,000 of Accidental Death and Dismemberment insurance in $10,000 increments. Spouse benefit is up to 100% of employee election if elected Children can be covered for $10,000. Individual rate is $0.2/$10,000 and Child rate is $0.10/$10,000 Extra Benefits: EAP, Travel Assistance, Life Event and Trauma, Seatbelt and Airbag, Childcare and Higher Education Benefits
16 Permanent Life with QOL with 5 Star Employee, Spouse, Children and Grandchildren can get coverage. Guarantee Issue for Employee only is 100K and Spouse is $30K Children and grandchildren coverage max is $20K Quality of Life Benefit Following a diagnosis of either chronic illness or cognitive impairment, you can receive 4% of your death benefit on a monthly basis, for a maximum period of 18 months, if you are unable to perform 2 of the 6 Activities of Daily Living without significant assistance. May be used for Home or Facility based long term care. The Quality of Life rider on the plan is only available if the policy is in force prior to age 66. An individual can still get a policy from 66-70, however, it would only be the life benefit without the QOL rider. You can keep this coverage, even if you leave the district, at no increased cost. Pay directly to 5 Star. Within 24 hours after receiving notice of death, an emergency death benefit of 50% or $10K is mailed to the beneficiary unless the death is within the 2 year contestability period. War and terrorism exclusion apply. Coverage stays in place until you are 100 years old! Premiums do not increase!

With MASA, you will have ZERO out of pocket expenses for any emergent air or ground transport from ANYWHERE in the U.S., REGARDLESS who transports you! Coverage includes you and your dependents.
18 Flexible Spending Accounts AN FSA IS A TAX FREE ACCOUNT FOR YOUR OUT-OF-POCKET MEDICAL EXPENSES Annual Contribution Limit is $2,650. You can enroll even if you do not enroll in any medical plan. All participants of FSA receive 2 flex card at NO COST! The NBS Flex Card makes using your FSA quick and easy just swipe it as a form of payment for medical, dental, vision and prescription expenses. Your full annual contribution loaded onto your debit card at the beginning of the plan year and is available to use as a form of payment until the funds are depleted. Friendswood ISD FSAs offer a 60 day grace period, meaning if at the end of the plan year you have a balance, you have an additional 60 days to use it up. After 60 days, any remaining balance is forfeited. You will receive your new NBS debit card mid-september.
19 Dependent Care Reimbursement Account A DEPENDENT CARE FSA IS A TAX FREE ACCOUNT FOR YOUR CHILDCARE OR ELDER CARE EXPENSES PAYABLE TO A LICENSED FACILITY. Tax Free via payroll deduction vs. deduction on income tax. Annual Maximum - $5,000 for married couple filing jointly or $2,500 if filing single.
