1 Shawnee State University Open Enrollment 2019 WHAT IS OPEN ENROLLMENT? Open enrollment is the time of year that you can make changes to your benefits, such as changing plans, dropping coverage, enrolling in coverage or adding and deleting dependents. All changes made during the open enrollment election period become effective on January 1, WHAT S NEW FOR 2019? IRS Maximum limits increase for Health Savings Account Tiered Network onmedical plans: Increased savings when you use Tier 1 providers! Slight plan design changes on medical plans, with corresponding employee contribution adjustments Removal of waiver payment Spousal Exclusion: Starting January 1, full-time working Spouses who have access to other medical coverage through their employers are not eligible to remain on our plan, and must enroll with their respective employers. If required by the other employer, HR can provide documentation regarding this loss of coverage. Should their employment status change or eligibility for that plan cease, they may request enrollment as a dependent on this Plan due to a Qualifying Event within 30 days of their other coverage terminating. For employees keeping a spouse on this plan, you will be required to provide an affidavit. Please direct questions related to possible exemptions to HR. ID CARDS Anthem will have separate ID cards for medical and dental. Please watch your mail in December for a nondescript envelope, as you must use the new cards starting in January to ensure your claims will be paid appropriately. REQUIRED ACTION Please review your benefit options carefully and attend one of the following sessions to make informed decisions about your health coverage. 1:1 assistance with HORAN and Anthem is also available; please schedule a time if needed with Megan Ketter via the contact information on the back of this brochure. Open Enrollment Meetings: 1) Nov. 27: 1:30pm, MUC 215 2) Nov. 28: 10:30am and 1:30pm, MUC 215 1:1 Assistance (by appointment): 1) Nov. 27: 2:30 4pm 2) Nov. 28: 11:30 1:30 and 2:30 4pm Although benefit elections from 2018 will carry over into the new plan year, all employees are encouraged to log into BearTrax to verify elections, enroll in coverage, change plans or dependents, waive coverage, enroll in Flexible Spending Account for the new plan year, and add or update beneficiary information. If you need assistance, reference the attached Open Enrollment Instruction & Tip Sheet. Open Enrollment must be completed in BearTrax by Nov. 30.

2 Health Insurance Plans In-Network Benefits PPO Deductible (copays do NOT apply) Tier 1: $500 single / $1,000 Tier 2: $1,500 single / $2,500 $3,000 single / $6,000 Embedded deductible * Participants: Preventive Rx (as defined by ACA regulations) can be filled at no cost to you, not subject to your deductible! Network Tiering: Seeking care from Tier 1 providers will result in lower member costs! Log into your member portal at anthem.com to compare facility costs and quality. Look for the T1 designation next to the provider name. Coinsurance Tier 1: 90/10%; Tier 2: 80/20% Tier 1: 90/10%; Tier 2: 80/20% Out-of-Pocket Annual Maximum Includes deductible, medical copays, and coinsurance Office Visits: PCP sick or Specialist visit Wellness at PCP Inpatient and/or Outpatient Hospital Emergency Room Medical: $3,500 single / $7,000 Rx: $4,150 single / $8,800 Deductible, then 10% or 20% Medical: $6,000 single / $12,000 Rx: Included in the above Deductible, then 10% or 20% Deductible, then 10% or 20% Deductible, then 10% or 20% $75 copay (waived if admitted) Deductible, then $75 copay (copay waived if admitted) Urgent Care Facility Deductible, then 10% Deductible, then 10% Retail Rx $10 / $35 / $60 / 25% (max $250) *Deductible, then $10/$35/$60 / 25% (max $250) Employees enrolled in one of our medical plans will automatically be enrolled in dental and vision. 4 Ancillary & Voluntary Plans Ancillary & Voluntary Plans Life and AD&D: We provide employees a life and accidental death & dismemberment (AD&D) benefit at no cost to employees. For Administrative, Support Staff and DPS Officers, this benefit would pay your beneficiary an amount equal to 2.5x your salary (maximum of $400,000) if you should pass away. The benefit for Faculty would pay your beneficiary an amount of $50,000. Voluntary Life: If you would like to purchase additional life insurance for yourself, and also purchase coverage for your spouse and/or child(ren), you can do so through the Voluntary Life benefit. The cost of the coverage is based on your age and the amount of coverage selected. If you have declined coverage in the past and would like to enroll during open enrollment, you must submit your election in the BearTrax system and complete Evidence of Insurability (EOI). *Employee must be enrolled in voluntary life in order to elect dependent life coverage. Already enrolled? Increase your coverage amount by up to $50,000 (max election $150,000 for those under age 65) with no underwriting, and be automatically approved! Coverage that goes through EOI will not take effect, and contributions will not be withheld, until the EOI is approved by Guardian. Voluntary Long Term Disability: This benefit protects a portion of your income if you should be unable to work due to a disability. You can choose from four different plan options that protect 60% of your earnings to a maximum benefit of $6,000 per month. Cost for the benefit is based on your age, your income and the plan that you select. EOI is required for new elections or changes to a richer benefit (e.g., moving from Plan 3 to Plan 1). Update your beneficiary information anytime in BearTrax! Voluntary Accident and Critical Illness: If you elect the voluntary accident plan, it pays you a lump sum payment for off-the-job accidents you, your spouse, and your kids experience. Examples of common benefits are bone fracture and dislocation, hospital confinement, and ambulance charges. If you elect the voluntary critical illness plan, it pays you a lump sum if you receive certain diagnoses such as stroke, heart attach, transplant, coma, and cancer. Both of these plans pay the benefit directly to you, to use the money as you wish. Please see the ManhattanLife benefit outline for more information. Tax-Advantaged Plan Information Plan & Incentive Information Flexible Spending Accounts (FSA) Health Care FSA: Available to those who waive coverage or those not participating in a Health Savings Account, this FSA allows you to pay for qualified health and Rx, dental, and vision care on a pre-tax basis. You may elect up to $2,650 per year. Any unused funds are forfeited on March 15th of the subsequent plan year; they will not rollover. Limited FSA: Compatible with the medical plan, this FSA allows you to pay for dental and vision expenses only on a pre-tax basis. You may elect up to $2,650 per year. Any unused funds are forfeited on March 15th of the subsequent plan year; they will not rollover. Dependent Care FSA: Compatible with either medical plan and also available to those who waive coverage, this FSA allows you to be reimbursed for eligible dependent-care expenses on a pre-tax basis. You may elect up to $5,000 per year (or $2,500 if married but filing separately). When you choose to participate, you elect a total dollar amount to contribute to your account, funded in the form of incremental pre-tax payroll deductions. You will be reimbursed only for expenses that are covered by your current account balance. If you incur expenses that exceed your account balance, you will be reimbursed gradually as funds become available. Any unused funds are forfeited on March 15th of the subsequent plan year; they will not rollover. Health Savings Account (HSA) If you enroll in our High Deductible Health Plan (), you are eligible for a Health Savings Account (HSA). HSA funds may be used to help pay for medical and Rx, dental, and vision expenses on a pre-tax basis. Certain eligibility requirements may apply. Your contribution may not exceed the following limits set by the IRS for 2019: $3,500 single enrollment or $7,000 all other enrollments. If you are over age 55, you may contribute an additional $1,000 per year. Unused funds continue to build each year until you use them! If participating in the HSA, you will receive the following employer contribution for 2019 into your account: Single: $500 Employee + Sp or Ch: $800 Family: $1,000 Support Staff employer contributions: Single: $600 Employee + 1: $950 Family: $1,200
