4 4 Employee Assistance Program (EAP) Your mental and emotional wellbeing are essential to your overall health and NetApp s free, confidential Employee Assistance Program provides support at your fingertips, 24/7/365: In-person counseling You and/or your family can receive six free, confidential in-person counseling sessions each year per issue Resources and referrals The EAP can also direct you to resources for issues such as parenting and childcare referrals, eldercare services, financial counseling and legal consultations Online support You and/or your family can access an online library of EAP resources. Use the EAP for a variety of personal issues NetApp pays the full cost of this benefit for employees and their family members.
5 5 Time Away Renew, relax and recharge with our generous time away programs. Family First NetApp is here for you when you need to care for the important people in your life. Our Family First program allows you to spend time with your family when it matters the most through the joys and the challenges. Whether you re bonding with a new child or caring for a child, spouse/domestic partner or parent with a serious medical condition, you can take up to six weeks of time off per year all at once, or in one-week increments paid at 100% salary. Volunteer Time Off (VTO) We know how important it is to find your passion, and VTO is at the heart of how NetApp gives back to the communities where we live and work allowing you to take off up to five paid days each year to volunteer with a cause that inspires you. Holidays Enjoy 10 paid holidays each year. Vacation Take off three to five weeks of paid vacation time each year, based on your years of service. Years of Service Paid Vacation Days Per Year Sick Time Accumulate up to 10 days of sick time each year to recover from illness or injury or to attend to the illness of a sick child, parent or spouse.
6 6 Additional Benefits Adoption Assistance covers qualified adoption expenses (e.g., adoption fees, court costs, legal fees and travel expenses), up to $13,840 per calendar year and a lifetime maximum of $20,000. Educational Assistance covers expenses of up to $5,250 per year for accredited coursework, with approval from your manager, to provide further development relevant to your NetApp career path. NetApp also offers a wide range of free professional development resources, including various training courses through NetApp University. VPI Pet Insurance helps you save money on your pet s health care. This is a voluntary benefit, which means you pay the full cost of coverage, but can take advantage of group discounts and convenient payroll deductions. Home and Auto Insurance, offered through MetLife, protects some of your largest investments. This is a voluntary benefit, which means you pay the full cost of coverage, but can take advantage of group discounts and convenient payroll deductions. Hyatt Legal Plans cover a wide range of legal matters, such as preparing wills, living trusts, real estate issues, civil litigation, adoption and taxes. You can enroll annually during open enrollment. This is a voluntary benefit, which means you pay the full cost of coverage.
7 7 Savings Your financial wellbeing is important, and NetApp offers different savings plans to help you achieve your goals. 401(k) Plan Save for the future with a retirement savings program that offers: Opportunity to contribute through convenient paycheck contributions including pretax contributions or Roth after-tax contributions. NetApp s 401(k) Plan allows you to contribute a maximum of 50% of your earnings per pay period, up to the IRS maximum of $18,500 in 2018 ($24,500 for age 50+). Company match of 100% on the first 2% of pay you contribute, then 50% on the next 4% of pay you contribute (up to a maximum of $6,000 per calendar year). Immediate vesting on NetApp s matching contributions. Employee Stock Purchase Program (ESPP) Become a part owner of NetApp by buying NetApp stock through convenient payroll deductions. The plan offers a series of 24-month Offering Periods that begin every six months (the first business day of June and December) and overlap on an ongoing basis, allowing new employees to participate. Enroll in the first Offering Period for which you are eligible or any Offering Period after that. You can participate in one Offering Period at a time.
8 8 Income Protection You and your family are covered in the event of your illness, injury or death. Disability Short-term Disability (STD) coverage protects your income if you re temporarily unable to work due to injury or illness. STD (Voluntary Disability Insurance, or VDI, for California employees) provides a 67% tax-free weekly benefit. Long-term Disability (LTD) provides benefits if you re disabled longer than 90 days. LTD coverage starts on your 91st day of disability. The NetApp LTD plan offers a 60% tax-free benefit of up to $25,000 monthly for all employees. Life Insurance Basic Life and Accidental Death and Dismemberment (AD&D) insurance is provided by NetApp through MetLife at no cost to you. Both programs cover two times your pay, with a maximum of $1.5 million. Supplemental Life and AD&D insurance is available for you to purchase at low rates through MetLife, in amounts of one to eight times pay (up to $4 million). Be sure to enroll when you join NetApp it s your one opportunity to purchase supplemental life insurance up to three times your pay with no questions asked. Amounts above three times your pay and future increases require you to submit a statement of good health and be approved by MetLife. Business Travel Accident Insurance provides life insurance benefits for domestic and international travel of three times your annual pay, with a maximum of $750,000.
9 9 Medical NetApp offers competitive, comprehensive medical plans that enable you to get healthy and stay healthy, with: Access to quality health care providers Coverage for your medical and prescription drug needs 100% coverage for in-network preventive care An annual out-of-pocket maximum that protects you from really large bills We offer three plans that vary in terms of cost, coverage and provider access, so carefully consider your options to discover the right fit for you: Anthem Health Savings Plan (HSP), a highdeductible PPO linked to a tax-advantaged health savings account (HSA) Anthem Traditional Plan, a conventional PPO HSP: Your Best Value NetApp gives you $800 (individual) / $1,600 (family) each year, automatically deposited in your HSA Lowest employee paycheck contributions Opportunity for pre-tax savings through the HSA that: Is yours to keep Can grow year over year making it a valuable savings tool for retirement You can invest, once your balance reaches $1,000 Kaiser HMO, which offers coverage only within the Kaiser network View plan details and costs.

Best Doctors provides access to the best medical minds in the world, so you can make the best possible medical decisions. Services include expert medical reviews, advisory services, physician search assistance, treatment decision support and more. The program is offered to you, your dependents, parents and parents-in-law at no cost. Anthem LiveHealth Online LiveHealth Online is your way to access live, face-to-face doctor consultations, including behavioral health benefits with a psychiatrist or psychologist, on your computer or mobile device. It s convenient, confidential, available 24/7 and costs the same (or less) than a regular office visit.
