10 How to Receive Short Term Disability Benefits Day 1: The employee must notify his/her supervisor of an absence as soon as possible but no later than 24 hours after the first absence. Day 3: The employee should notify HR Connections of the absence, and enter his or her absence in SAP as STD Employee Sick 0220 Day 7: An Employee Health Report (EHR) must be submitted to ConocoPhillips Health Services via either or fax to at no later than the 7th consecutive calendar day of absence (consecutive calendar days includes scheduled time off, weekends and holidays). Day 30: The next EHR is due. However, if the previous EHR specifies the date of the next physician s appointment, then you many contact ConocoPhillips Health Services to request an extension that may be granted at their discretion. Weekends and holidays count towards your days out. As long as you re out, your doctor will need to submit a new EHR every 30 days. If you know you ll be out for an extended period (e.g. for surgery), call HR Connections before to let them know. 10

It s completely free and confidential. EAP offers up to eight outpatient counseling sessions for you and your dependents. It can help with: Marital and family problems Parenting Interpersonal relationships Substance abuse Balancing work and family Depression Stress and anxiety Career transitions Grief and loss Coping with illness You decide if and when you need to use EAP. 14 U.S. Health Improvement Incentive Program ConocoPhillips is committed to supporting healthy and productive employees Paid Incentives You will automatically receive the $50 Healthy Weight incentive; a biometric screening is not required. Action Needed: You and your covered spouse/domestic partner must certify your Tobacco Free status during your on line benefits enrollment to receive the $12.50 per person Tobacco Free incentive Paid Incentives Employees hired before June 1, 2017 are required to take action before Sept. 30 to earn the health incentive paid in To be eligible for these incentives employees must complete a biometric screening onsite or their own physician. 14
15 U.S. Health Improvement Incentive Program Earn the Healthy Weight and/or the Tobacco Free incentive. Healthy Weight incentive: $50/month If your 2017 biometric screening results indicate your BMI is less than 30, you will earn the Healthy Weight incentive automatically. If your 2017 biometric screening results indicate your BMI is 30 or more, you must complete a qualifying activity by Sept. 30, 2017 to earn the incentive. You can participate in one of the qualifying activities available this year: o HealthyWage Energy Industry Challenge Register today o Energy in Action! Q2 o Nutrition Boot Camp Q2 o Naturally Slim Q3 Tobacco Free incentive: $12.50/month for you and/or $12.50/month for your covered spouse/domestic partner.
