3 Welcome to 2015 Open Enrollment Oct. 22 Nov. 7, 2014 ACC continues to provide a comprehensive benefits program that gives you and your family the coverage you need, when you need it. Knowing the importance of your benefits, ACC performs an extensive annual review of our benefits program to gauge our competitiveness in the market place and to ensure that we are providing our employees with a quality benefits package at the lowest possible cost. Please take the time to review this Open Enrollment Guide and review the options before you make your selections. This guide provides a summary of benefits, your 2015 payroll deductions, and information about additional wellness resources available through the year. Exciting News! ACC will continue to offer benefits through CareFirst, MetLife, and VSP with no changes to the plan summaries. There will be minimal impact to payroll deductions in Less s! All annual notices, benefits summaries and additional enrollment applications will be located on the ADP Portal versus being sent to all staff. When to Enroll The open enrollment period runs from October 22, 2014 through November 7, The benefits you elect during open enrollment will be effective from January 1, 2015 through December 31, This is your annual opportunity to review your options to make sure the plan you elect will meet your personal and financial needs. We hope the benefits ACC offers will inspire you and your dependents to take charge of your health and continue to make your wellness a priority. 4 Open Enrollment Basics Eligibility If you are an employee working 30 or more hours per week, you are eligible to enroll in the benefits described in this guide. The following family members are eligible for medical, dental, and vision coverage: spouses, dependent children, and domestic partners. Enrollment 1. Log on to the ADP Employee Portal ( The ADP site can be accessed from any computer at any location. 2. Click on the Personal Information Tab and update: a. Personal Information b. Emergency Contacts c. Dependent Information 3. Go to the Benefits Tab and click on Review and Change Benefits Tab to begin Open Enrollment. The system will provide step-by-step instructions. Upon completing open enrollment, you will receive a confirmation. Changes after Open Enrollment Once you enroll in your benefits, you will not be able to make changes to your elections until the next Open Enrollment period, unless you experience a Qualifying Life Event such as marriage, divorce, birth or adoption of a child, death, or changes with your dependent s employment status or loss of health coverage. If you experience a Qualifying Life Event you must make changes within 31 days of the event. Contact HR for additional details. 4 P a g e

5 Open Enrollment Check-List Overview Review this Benefits Guide Attend the Benefits & Wellness Fair (10/21) Attend the CareFirst Education Seminar (10/21). Attend the TIAA-CREF Educational Seminar (10/21) Begin Open Enrollment in the ADP Employee Portal ( on or after Wednesday, October 22. Upon entering the ADP Employee Portal, click on the Benefits Tab. You will see several tabs that you may use to complete open enrollment. o Review & Change Benefits: This is the primary tab you will use to complete your 2015 open enrollment. It is a step by step enrollment process and will allow you to save and return at any time during open enrollment. o Compare Plans: This tab will allow you to review and compare ACC s available plans based off of your current coverage. o Plan Details: This tab allows you to review a specific plan and benefit summary. Personal Information To ensure your personal information is up-to-date, please complete the following actions. My personal and dependent information is correct (name, social security number, birthdate, address & phone) My emergency contacts are updated My work is in the ADP Portal Update or designate Prudential Life Insurance Beneficiaries. Medical, Dental, & Vision Benefits Your benefit coverage for medical, dental, and vision will roll over to If you wish to keep the same coverage you have currently, please do not update these benefits. You should only update if you are changing plans, changing your coverage level or waiving coverage. If you choose to waive health coverage, you must provide proof of medical coverage to HR. 9 Medical ACC continues to offer two medical plans through CareFirst BlueCross BlueShield (BCBS). Both plans cover preventive care at 100% in-network. CareFirst BlueChoice Advantage HMO Plan In the BlueChoice Advantage HMO Open Access plan, your primary care provider (PCP) provides preventive care and works with you to find specialty care using a large network of CareFirst BlueChoice providers. Unique to this plan, the Open Access feature allows you to visit a specialist directly without a referral from your PCP. Plus, the BlueChoice Advantage Plan offers you the flexibility and freedom to choose from both in and out-of-network providers. Receiving Care inside the CareFirst Service Area: When rendering care in Maryland, DC, or Northern Virginia, use the CareFirst BlueChoice Network to receive the highest level of coverage and pay a lower out-ofpocket expense. Note: If you receive care from a provider that is not part of the BlueChoice network, services will be covered as out-of-network. Receiving Care outside the CareFirst Service Area: Members seeking care outside of the CareFirst service area will lower out-of-pocket cost by using a national BluePreferred PPO provider. Members will still have the option to opt-out of this network but will pay a higher out-of-pocket expense. CareFirst BluePreferred PPO Plan The plan provides benefits in and out-of-network; the choice is yours. The plan provides services through the CareFirst BluePreferred PPO network of doctors, hospitals and other health care providers. Consider in-network providers for all care to benefit from CareFirst s negotiated discounts and greater plan reimbursements. Use of out-of-network providers results in greater out-of-pocket costs and potential balance billing from your provider for amounts above the CareFirst allowed benefit. 10 do you want to search? : Preferred Provider Organization (PPO) or BlueChoice Advantage If you have not registered already, or are new to the plan for 2015, please remember to register online as a member at This allows you to view plan designs and your claim history, print Explanation of Benefits (EOBs), research information on procedures, view CareFirst s prescription drug formulary list and print temporary ID cards. CareFirst Prescription Drug Plan Prescription drugs can be purchased at participating retail pharmacies for minimal copays. Some maintenance medications (e.g., birth control pills, blood pressure medication, etc.) can be purchased through CareFirst s mail-order service that will home deliver a three month supply of maintenance medication at only two times the co-pay. For more information on how to determine prescription drug tier status, search for participating pharmacy, or specialty medications, visit CareFirst s website ( or call Member Services at (202) (BluePreferred PPO) or (800) (BlueChoice HMO). CareFirst Annual Vision Exam Benefit & Discounts Program When you are enrolled in a CareFirst medical plan you also receive an annual vision exam benefit. If you utilize the services of a provider listed in the CareFirst referred Vision Provider Directory, you may receive your routine vision exams for an office visit copay. This is considered a discount program for other vision services such as frames and lenses. 10 P a g e

Discounts only available from contracted facilities. After surgery, use your frame allowance (if eligible) for sunglasses from any VSP provider. 18 ACC Commuting Benefit The ACC provides employees with a monthly commuting benefit of $105 and allows the employee to choose between the QTE Mass Transit Benefit or Onsite Parking. The benefit begins the first of the month following the employee s start date. The chart below shows ACC s commuting options. QTE Mass Transit Benefit Onsite Parking Benefit Metro, Bus, or Train Offsite Parking 2300 N St N St. Car Pool QTE Mass Transit Benefit: Flores & Associates, a third-party vendor manages ACC s QTE Mass Transit Benefit. Upon enrolling in the benefit, ACC will provide Flores & Associates with the monthly $105 benefit and additional funds you may request. If the employee chooses to add additional pre-tax dollars to the benefit, deductions will be deducted from the biweekly payroll. This benefit can be updated with HR on a quarterly basis and must be to HR by the 15 th of the month (March, June, September, November). Metro, Bus & Train Expenses* - The IRS allows individuals to add up to $130 pre-tax dollars each month to use to pay for Metro related expenses. After, ACC s contribution of $105, the employee can add an additional $25 pre-tax dollars to use each month. If the employee uses the Benny Visa Card to add funds onto their metro card which is then used at the metro, on a bus or train, Flores & Associates will subtract funds from the employee s metro fund. Offsite Parking Expenses* - The IRS allows individuals to add up to $250 pre-tax dollars each month to use to pay for parking expenses at the metro, non-metro parking lots, or garages. If the employee uses the Benny Visa Card to add funds onto their metro card which is then used to pay for parking, Flores & Associates will subtract funds from the Employee s parking fund. If the employee chooses to pay for parking directly with the Benny Visa Card, the funds will also be subtracted from the Employee s parking fund. 18 P a g e

19 Receiving Funds from Flores & Associates - The employee has two options to receive monthly commuting funds. See chart below for details. Receiving funds Benny Visa Card Reimbursment through Flores & Associates 1. Benny Visa Card The employee can order the Benny Visa card when completing the enrollment form which has a $1 monthly administraive fee and will be deducted in a one-time fee in payroll. The Visa card will hold the monthly funds until the employee uses the card to add funds to the Metro card. ACC does not supply staff with a Metro card. 2. Reimbursement - The employee can use personal funds to add dollars onto their Metro card and request reimursment directly with Flores & Associates. Reimbursement forms are located on the Intranet and in the HR office. Direct deposit is also available with this option. Onsite Parking at ACC: ACC manages the onsite parking benefit at ACC s building (2400 N St.) and at the parking garage at 2300 N St. ACC pays the monthly parking fee on the employee s behalf and deducts additional cost from the biweekly pay N St. Garage - The monthly cost of parking at the 2300 N St. Garage is $185. ACC will provide $105 towards the parking cost; therefore, the employee will see a pretax deduction of $36.93 per pay (26 pays/year) N St. Garage - The monthly cost of parking at the 2400 N St. Garage is $205. ACC will provide $105 towards the parking cost; therefore, the employee will see a pretax deduction of $46.15 per pay (26 pays/year). Carpool Option See Jerry White for details. 19 P a g e

20 Life Insurance Basic & Voluntary Life and AD&D Insurance ACC provides full-time employees with 1.5X salary up to $450,000, group life and accidental death and dismemberment (AD&D) insurance, and pays the full cost of this benefit. Please find the enrollment application on the ADP Employee Portal. Employees who want to supplement their group life insurance benefits may purchase additional coverage. You can purchase coverage for yourself, your spouse and/or your dependents. When you enroll yourself and/or your dependents in this benefit, you pay the full cost through bi-weekly payroll deductions. Voluntary Employee, Spouse, and Dependent Life Insurance Coverage Guaranteed Issue Amount Employee Spouse Dependent Children Multiples of $10,000 to $500,000, maximum amount is the lesser of 7x annual earnings of $500,000 Multiples of $5,000 to $250,000, not to exceed 50% of employee coverage amount Multiples of $1,000 to $10,000, not to exceed 50% of employee coverage amount $20,000 $20,000 $10,000 Legal Resources Through Legal Resources, employees will receive advice, consultation and courtroom representation for common legal services 2015 NEW enrollees: Upon enrolling, you will receive an enrollment form from HR to complete enrollment. CURRENT enrollees: Unless you waive Legal Resources during open enrollment, this benefit will automatically rollover to Legal Resources Enrollment Employees will pay an initial enrollment fee Cost $20.00 Monthly Employees will see biweekly payroll deductions for the monthly cost of the Legal Resources Benefit. 22 Questions & Answers Where do I go to complete Open Enrollment? Open Enrollment is located on the ADP Employee Portal ( If you have trouble logging on to the ADP Employee Portal, click on password reset to automatically reset your password, or contact Trashaun Herring for a password reset which can take up to 24 hours to reset. What changes can I make for 2015? Enroll or terminate individual and/or dependent coverage in the medical/dental/vision plans. Enroll/Re-enroll in the Flexible Spending Account Plan (Medical and/or Dependent Care). Add or make changes to the Voluntary Life plan. Enroll in or terminate the Legal Resources benefit. Update your QTE Commuting Benefit. Please note this benefit may be updated on a quarterly basis after the 2015 open enrollment period. What additional applications should I complete? If you are a new enrollee with Legal Resources, you will need to complete the Legal Resources application on the ADP Employee Portal and return to Crystal Grandstaff. If you are a new enrollee OR increasing your Voluntary Life Policy, you will need to complete the Prudential Voluntary Life Application on the ADP Employee Portal and return form to Crystal Grandstaff.
