5 Wellness - Modern Care Plan Vendor: A Charitable Life Health Solutions Employee Eligibility Requirements: 30 hours per week The focus of the program is preventative health with access to tools, resources and discounts to help employees save money and manage their own health conditions. INFINITI HR has partnered with A Charitable Life Health Solutions (ACLHS) to offer this new and exciting opportunity. This program meets all the requirements of the Affordable Care Act while providing a number of valuable benefits to the employees. One of the many benefits of this program is having online access to doctors who are board certified and can prescribe certain medications. This will save employees time and money and will eliminate office visits on routine medical situations. Best of all, this program is free to employees and in some cases increases their pay by reducing their taxes. 6 Dental Benefits - Guardian Vendor: Guardian Group Employee Eligibility Requirements: 30 hours per week This national dental plan is available to all INFINITI HR clients and offers two tiers of PPO dental coverage. Guardian dental insurance covers and helps you pay for routine and outof-pocket dental expenses with access to one of the largest networks of dental providers in the country. In addition to their extensive network, they also have negotiated rates that are, on average, 30% lower than other carriers. Both plans have low deductibles and cover innetwork preventative care at 100%. All premiums are contributed on a pre-tax basis. Plan Features: n Two PPO plans available ($1000 and $1500 annual coverage limits) n 100% In-Network Preventative Coverage n Low Premiums n Orthodontia Coverage n Dependent Coverage through age 26* *Non-student dependents are covered through age 25 Dental Maximum Rollover At no additional cost, Guardian will roll over a portion of unused annual maximums into a personal Maximum Rollover Account (MRA). If employees reach their annual maximum for services and have a balance in their MRA they can apply that balance towards the services to lower their out of pocket expenses!! Carry over amounts vary based on benefit option elected and time enrolled in the plan. The Plan Annual Maximum plus MRA cannot exceed $2,000 for the lower tier plan and $2,750 for the higher tier plan. 7 Vision Benefits - VSP Vendor: Vision Care for Life Employee Eligibility Requirements: 30 hours per week INFINITI HR has partnered with the only not-for-profit vision care company in the country and is available to all INFINITI clients. The plan offers comprehensive coverage for vision exams, prescription glasses, frames, contact lenses and much more. The plan has low co-pays and the premiums are affordable! All premiums are contributed on a pre-tax basis. Plan Features: n WellVision Exam ($10 co-pay covered 1 time every 12 months) n Prescription Glasses ($25 co-pay covered 1 time every months) n Up to $150 allowance on selected frames n Lenses covered with prescription n Lens Enhancement coverage n Additional Savings on glasses and sunglasses n Retinal Screening discounts n Laser Vision Correction discounts Rates (per month) Employee Only $8.79 Employee + 1 Dependent $14.81 Employee + Child(ren) $15.21 Family $24.37

8 FSA and HSA Options - Connect Your Care Vendor: Connect Your Care Employee Eligibility Requirements: 30 hours per week A Flexible Savings Account (FSA) and Health Savings Account (HSA) are benefits that allow employees to set aside pre-tax dollars for eligible medical, dental, vision and dependent care expenses that can be incurred throughout the year. These accounts can be used for expenses such as co-pays, out-of-pockets expenses, prescriptions and eyewear. INFINITI HR offers clients the flexibility to choose a FSA or HSA account based on the medical plan that is place and the additional option of the FSA Dependent Care. Plan Features-FSA: n Two (2) FSA Options n Pre-tax savings n Online employee access for account management and claim processing n Mobile app n FSA Debit card Plan Features-HSA: n Pre-tax contributions n Unused benefits can carry over year-to-year n Online employee access for account management and claim processing n Mobile app n HSA Debit card

9 Supplemental Life Insurance and AD&D - Unum Vendor: Unum Employee Eligibility Requirements: 30 hours per week Through INFINITI s partnership with Unum, employees have the option to purchase additional life insurance on themselves, their spouse and/or dependent children. The insurance plan provides financial protection in the event of death with benefits paid to the employees named beneficiaries. Coverage is provided for the time period that employees are eligible and premiums are paid. Premiums are based on attained age of application and volume requested. Employees must enroll themselves in order to enroll a spouse or dependent(s). Premiums are taken post-tax. 10 Transportation Benefits - EFlex Transit Vendor: EFlex Transit Employee Eligibility Requirements: No requirements, all employees are eligible Some states require employers to offer transit benefits to their employees. INFINITI HR has partnered with EFlex Transit to provide these benefit nationally to our clients. This benefit is perfect for employees who use work-related mass transit and/or pay for parking; it can save employees up to 30% on commuting expenses. With the account, employees set aside a portion of their paycheck to be used for qualified transit and parking expenses on a pre-tax basis. Employees can elect to contribute up to $255 toward such expenses (subject to change based upon IRS approval). 11 Retirement Solutions INFINITI HR offers 401(k) plan options through John Hancock for clients that would like to offer retirement benefits. Because of our group plan, John Hancock pools all assets which results in lower plan and participant fees for you! Our third party administer works closely with you to determine the fee structure and implementation process that fits your needs. Plan Features: n Program provided by John Hancock, an industry leader in 401(k) n Semi-Payroll Integration to reduce your administrative burden n Customized plan design n Fee flexibility with $0 billed fee and extremely low participant fee options available n Multiple fund managers available, such as Vanguard, DFA, American Funds, T. Rowe Price and Oppenheimer Funds n Customizable participant level retirement planning tools available to your participants via the John Hancock website n Access to a plan consultant for both the plan administrators and your participants seeking investment advice n Fiduciary Warranty to protect the plan trustees n Transfer and rollover options available Employee Discount Program INFINITI HR Perks is a program designed to give employees access to discounted goods and services through our group purchasing program. Eligible employees receive an after 30 days of employment with all the information and links to utilize this benefit. Additional discounts and offers are also available based on client s geographic location.

12 Legal Services INFINITI HR has partnered with Legal Shield to provide employees with access to legal services such as identity theft protection, will preparation, legal defense and much more. For as little as $15 per month, employees have access to a nationwide network of lawyers and experts to help with those difficult situations life sometimes brings. Voluntary Products INFINITI HR partners with a number of National carriers such as Unum and Aflac to offer ancillary benefits at group rates.
