6 qualifying life events What is a qualifying life event? Open Enrollment selections are generally locked for the plan year, but certain exceptions called Qualifying Life Events (QLEs) can grant you a special enrollment period in which to make midyear changes. 7 how to make midyear QLE changes 1. On your Employee Navigator homepage, select the second box from the left, labeled Benefits. 2. Select the type of change you want to make. You can add new coverage, edit existing elections, or drop coverage. 3. Select your reason for making the change from a list of the most common Qualifying Life Events. 14 medical bridge plan SUPPLEMENT THE CORE This plan supplements your medical coverage by paying you cash to help meet your deductible and other out-of-pocket expenses. 18 vision plan Your yearly eye exam is 100% covered in the VSP network, for just a $10 copay. The plan reimburses $39 out of network. eye exams Lenses are 100% covered in the VSP network after a $25 copay, once per year. Out of network, the plan reimburses between $23 and $64, depending on lens type. lenses If you use the VSP network, the plan pays you up to $130 for frames every two years. Out of network the plan pays $46. frames The plan pays you $130 per contacts (instead of glasses) every year if you use the VSP network. 19 vision plan The Thomasville City Schools vision plan offers you the flexibility of using VSP, a trusted vision network with providers nationwide. national network The Thomasville City Schools vision plan takes the guesswork out of eye care by offering low, consistent copays on common services and materials when you use the VSP network. low copays In addition to great coverage on eye care services, the Thomasville City Schools vision plan offers you 15-30% discounts on upgrades and sunglasses when you use the VSP network. extra discounts 19

20 disability income don t get detoured by disability Thomasville City Schools provides disability income plans to keep you on the road to retirement by ensuring that you and your family can count on an income if you get hurt or sick and are unable to work. Retirement 67 waiting period If you get sick or hurt and can t work, where will the money come from? Your waiting period is the time you ll be without income until disability benefits begin. short term disability After a waiting period of 14 days, Voluntary Short Term Disability begins paying you 60% of your income. These payments will last up to 150 days. long term disability On the 151st day, Long Term Disability begins paying you 60% of your income, and lasts until retirement (age 65 or 67) if necessary. *This benefit is paid for 100% by Thomasville City Schools. 20

21 disability income Your Long Term Disability policy is paid for entirely by Thomasville City Schools. Supplement this core benefit with Voluntary Short Term Disability insurance. DI key features Maternity is covered by the Short Term Disability policy. Most people will receive 4 to 6 weeks of payments. maternity If your doctor says you are only able to work part time, you will be eligible for partial benefits. See online for details. partial disability You pay for Short Term Disability so your benefits will be received tax-free. Because Thomasville City Schools pays for 100% of your Long Term Disability, that benefit is taxable. tax savings 21

22 supplemental plans Aflac supplemental plans pay you tax-free cash to help cover out-of-pocket expenses when you re sick or hurt. Thomasville City Schools offers the Colonial Medical Bridge plan to help cover your deductible and other out-of-pocket medical expenses. For more details on this plan, see page 14. Thomasville City Schools offers the Unum Long Term Care plan to pay you a monthly benefit during times when you are unable to perform activities of daily living and require assistance. 24 tax savings strategies Your Flexible Spending Account is currently managed by WageWorks. You can access your accounts online 24/7 at Subject to change. HOW IT WORKS The Healthcare Reimbursement Flexible Spending Account allows employees to pay for certain IRS-approved medical care expenses not covered by their insurance plan with pre-tax dollars. Some examples include: Deductibles, coinsurance, prescription copays, and hearing services, including hearing aids and batteries Vision services, including contact lenses, contact lens solution, eye examinations and eyeglasses Dental services including Orthodontia Maximum $2,500 per year may be pre-taxed. Those dollars avoid Federal and State income taxes. Healthcare Reimbursement FSA 21% There is a $5 per month cost for this plan. At the end of the year you may roll over your unused balance, up to $500, for another 12 months. Otherwise this is use it or lose. 24 Minimum tax savings based on 15% federal and 6% Georgia taxes

25 tax savings strategies KEEP MORE OF YOUR MONEY Thomasville City Schools offers several ways for you to save on medical expenses by reducing your taxes and increasing your take-home pay. HOW IT WORKS Dependent Care FSA The Dependent Care Flexible Spending Account allows employees to use pre-tax dollars for qualified dependent care such as caring for children under age 13, or for elders. The annual maximum amount you may contribute is $5,000 (or $2,500 if married and filing separately). Those dollars avoid Federal and State income taxes. The cost of child or adult dependent care The cost for an individual to provide care either in or out of your house Nursery schools and preschools (excluding kindergarten) ADDITIONAL TAX SAVINGS PREMIUM PRE-TAX PLAN Money you spend on benefits (except for Life and Disability) is sheltered from Federal and State taxes, for a tax savings of 21-34% There is no charge for this plan. HEALTH SAVINGS ACCOUNT Reduce your taxable income while saving on out-of-pocket medical expenses. (Requires the UHC HDHP plan, see page 12 for details). 25
