1 2018 Employee Benefits Thanks for your interest in IDEX! We are proud to offer a competitive benefits package and a variety of plan options, that can be customized to meet our employees individual needs. This guide is an overview of the benefits that are available to full-time IDEX employees.

2 IDEX Healthcare Benefit Programs The IDEX ChoiceComp Plan gives full-time, active employees who work 30 or more hours a week the flexibility to create their own benefits package. Eligible dependents may participate in the Medical, Dental and Vision Plans. Eligible dependents include: Legal spouse or domestic partner Children, up to age 26, who do not have employer-sponsored coverage of their own Children, of any age, who are physically or mentally disabled and incapable of supporting themselves, and can be claimed as a dependent on a U.S. federal income tax return Medical & Prescription Drug Benefits IDEX offers four Medical Plan options, administered by BlueCross BlueShield of Illinois (BCBSIL). Prescription Drug benefits are administered by CVS/Caremark. The Plan covers nearly all dental procedures, including orthodontics. 4 Health Savings Account (HSA) Employees who enroll in Medical Plan Option C are eligible to contribute pre-tax dollars to a Health Savings Account (HSA) to pay for qualified healthcare expenses. Each year, IDEX makes an employer HSA contribution of $500 for single coverage and $1,000 for dependent coverage (contribution may be prorated based on date of hire). Flexible Spending Accounts (FSA) IDEX offers two Flexible Spending Accounts (FSAs) to help employees save for qualified healthcare and/or child day care expenses: the Healthcare FSA and the Dependent Care FSA. The Healthcare FSA is available to employees enrolled in Plan Options A, B or D. Employees enrolled in Plan Option C are only eligible for the Dependent Care FSA. Funds do not roll over from year to year, and a contribution amount must be re-elected each year during the enrollment period. Company-Paid Insurance Programs Life Insurance/AD&D Insurance: IDEX provides company-paid life insurance equal to 1x an employee s annual base salary. Dependent Life Insurance: IDEX provides dependent life insurance in the amount of $10,000 for any eligible dependents. Employee Assistance Program: IDEX offers an Employee Assistance Program through ComPsych. $100,000 Business Accident Travel Policy Voluntary Benefit Programs IDEX is proud to offer additional benefits, like Long-Term Disability, for employees to purchase to enhance their coverage at a low cost. Voluntary programs include: Long-Term Disability: Long-Term Disability benefits allow employees to insure a portion of their salary at a low monthly rate, offering financial security and peace of mind. It provides ongoing benefits equal to 60% of an employee s income, up to a maximum of $15,000 monthly, in the event that a disability prevents them from working for an extended period of time. Allstate Accident Insurance: Provides a lump sum benefit to help cover the costs associated with unexpected bills resulting from an accident. Allstate Critical Illness Insurance: Provides a lump sum benefit to protect employees and their families from a financial loss following a prolonged illness or injury. LifeLock: Identity theft protection. Personal Accident Insurance: Provides an additional life insurance benefit in the event that an employee or elected dependent dies from an accident. Supplemental Life Insurance: Employees may supplement their IDEX company-paid life insurance by purchasing additional life insurance at their own expense. 04

5 IDEX Wellness Program IDEX offers a monthly wellness credit for those enrolled in an IDEX Medical Plan $50 for employees and $25 for spouses/domestic partners who successfully meet the program requirements. New employees must receive a biometric screening and complete an online health assessment within 60 days of hire to be eligible. Additional Healthcare Benefit Programs IDEX offers the following resources at no cost to help employees navigate the healthcare system and reduce their out-of-pocket costs. MDLive is a virtual alternative to Emergency Room or Urgent Care visits for nonemergency medical care on the go. MDLive is available at no cost no copay required! Healthcare BlueBook can help compare cost and quality among medical care providers. Employees may even get cash back for choosing a lower-cost provider! The BlueCross BlueShield Blue Distinction Center finder tool helps locate high-quality specialty care centers. When employees receive care from one of these providers, IDEX s medical plan will cover the relevant charges at a higher percentage. IDEX Retirement Savings Plans IDEX offers two retirement benefit plans: The Defined Contribution Plan and the IDEX 401(k) plan. 401(k) Plan The IDEX 401(k) plan allows employees to save through pre-tax payroll contributions. IDEX matches the first 8% of employees contributions at a 50% match, up to a maximum Company match of 4%. Defined Contribution The Defined Contribution Plan is fully funded by IDEX, with no employee contributions required. The Company contributes a match based on a percentage of your salary. The match percentage is calculated based on your Age + Years of Service. Total of Your Age + Years of Service* % of Your Eligible Earnings Less than % 40 up to % 55 up to % 70 or more 5.00% *Employees age and service are truncated to whole years prior to being added together 05

6 Employee Stock Purchase Plan (ESPP) The Employee Stock Purchase Plan (ESPP) allows employees to become IDEX shareholders through the convenience of payroll deductions. Contribute as little as $5 per paycheck, up to 10% of eligible earnings. Owning IDEX shares is a great way to save for retirement or future expenses. Additional IDEX Benefits Tuition Reimbursement Program IDEX promotes future development and growth of its employees. IDEX offers a Tuition Reimbursement Program to assist with the financial burden of further education. Regular full-time employees in good standing with at least six months of service are eligible to participate in the Tuition Reimbursement Program. The program offers reimbursement of certain expenses for secondary educational institutions, up to $5,250 per year, the maximum amount allowed as exempt from tax withholdings. Matching Gift Program At IDEX, we believe that quality education is the key to developing future generations, and we want to encourage our employees to support the educational institutions that are important to them. IDEX will match gifts of at least $50 to an accredited educational institution, located in the U.S., dollar-for-dollar, to a maximum of $10,000 per employee, per year. Short-Term Disability Full-time, U.S. employees who work 30 hours or more per week are eligible for IDEX s Short-Term Disability benefit. The IDEX Short-Term Disability policy provides disability pay if an employee is unable to work due to a serious medical condition. The policy benefit pays 100% of regularly scheduled hours of pay for Days 1-90, and 60% of regularly scheduled hours of pay for Days Employees must apply for this benefit and be approved. Tru Imp
