3 Enroll Starting October 23 for 2018 Benefits It s time to make your benefits elections for 2018! This Enrollment Guide highlights the benefits available to you and your family starting January 1, Find more information about 2018 coverage options in the Medical Plan Comparison and Cost Summary, included in your Benefits Open Enrollment packet, and online at MyUSNHBenefits.net BENEFITS OPEN ENROLLMENT When You Can Enroll You must enroll, change or waive coverage during the 2018 Benefits Open Enrollment period: October 23 November 3, Benefits Open Enrollment is the only time you can enroll or change your benefits elections for 2018, unless you experience a qualified change of status during the year (such as marriage, divorce or the birth of a child). Who You Can Cover You can elect coverage for yourself and any eligible dependents. Visit MyUSNHBenefits.net for a list of eligible dependents. If you re enrolling dependents for the first time, you have 30 days from your enrollment date to provide dependent eligibility verification documents at MyUSNHBenefits.net or to your campus Human Resources Office. What Your Benefits Cost Bi-weekly payroll deductions for the Medical Plans are available in the Medical Plan Comparison and Cost Summary, included in this Benefits Open Enrollment packet. Payroll deductions for dental and vision coverage are included in this guide. Your cost will depend on the options you elect and the eligible family members you cover. Don t Default! Benefits Open Enrollment is active, which means you must enroll to have the coverage you want starting January 1, If you don t enroll in medical coverage, you will default into the lowest option plan with Employee Only coverage, subject to eligibility. If you waived coverage for 2017 and don t make coverage elections for 2018, you will continue to have waived coverage for MEDICAL AND PRESCRIPTION DRUG PLANS You can choose from three Open Access Plus Medical Plans administered by Cigna. All plans: Provide access to Cigna s nationwide network of doctors and hospitals Don t require you to select a primary care provider Cover the same services, including preventive care (generally covered at 100% in-network), emergency services, in-patient and out-patient care Include prescription drug coverage, administered by OptumRx Offer a fitness club membership benefit of up to $500 per year, provided eligibility requirements are met. For each plan, you share in the cost of services through deductibles, copayments and/or coinsurance. And, you re protected from the high cost of large medical bills by an annual out-of-pocket maximum. The out-of-pocket maximum is the most you will pay for covered services in a calendar year before the plan pays 100% of covered expenses. Review your Medical Plan Comparison and Cost Summary for coverage details and costs. Cash Back Incentives for Coverage Elections. If you re in a status 75% 100% appointment and you re not covered under another employee s USNH Medical or Dental Plan, you re eligible for cash back if you waive certain benefits, including medical and dental Enrollment Guide 1

4 Take a Closer Look at the Open Access Plus Health Savings Account (HSA) Plan If you meet eligibility guidelines and enroll in this plan, you can open a Health Savings Account to pay for eligible qualified health care expenses, tax-free! Here s how: USNH will provide a contribution to your HSA and you can contribute your own money, tax-free, up to the IRS limits (if you re eligible). You can use your account to pay for eligible medical expenses, like copays/coinsurance, your deductible and prescription drugs. You can contribute a maximum of $3,450 for Employee Only coverage/$6,900 for All Other Coverage Levels, combined employee and employer contributions. If you will be 55 by December 31, 2018, you can contribute $1,000 more to your HSA. You can take your HSA with you if you leave USNH or retire. The money in your account grows tax-free from investment returns and you re not taxed on withdrawals. To be eligible for the Health Savings Account you must be enrolled in the Open Access Plus HSA plan and not be enrolled in Medicare or any other health insurance coverage. In addition, if you have a 2017 Healthcare Flexible Spending Account, you must have a zero balance on December 31, Visit MyUSNHBenefits.net to view a short video on the advantages of the HSA! How to Decide on a Medical Plan How much health care do you expect to use? Remember the most expensive plan option isn t always the best option for you. If you don t expect to spend a lot on health care services or prescription drugs, consider the Open Access Plus HSA plan. You ll pay less out of each paycheck for coverage, and only pay for the services you use. Do you have the resources to pay for health care expenses? In the Open Access Plus HSA plan, USNH s HSA contribution will help you pay your out-of-pocket costs. However, consider whether you will have the resources to pay the full cost of medical services until you reach the deductible. Do you want to build tax-free savings for future health expenses? The HSA is the only tax-advantaged account that rolls over from year to year. Need help with your Medical Plan decisions? IMPORTANT DEFINITIONS TO KEEP IN MIND Annual deductible: The amount you generally pay each calendar year for covered services before the plan starts paying benefits. Annual out-of-pocket maximum: A limit on the total amount you pay each calendar year for covered services (deductibles, copays and coinsurance). The Medical Plan you choose may have a separate annual prescription drug out-of-pocket maximum for covered prescription drug copays. Coinsurance: A percentage of the cost of covered services you pay, generally after you meet the annual deductible and before you meet the annual out-of-pocket maximum. Copays: The flat dollar amount you pay for covered services before you meet the annual out-of-pocket maximum. 2 University System of New Hampshire

5 DENTAL PLANS USNH offers two Dental Plans administered by Northeast Delta Dental USNH Dental Plans Overview Feature/Service Basic Option High Option Diagnostic/ Preventive Covered at 60% Covered at 100% Basic Restorative Covered at 50% after deductible Major Restorative Covered at 40% after deductible Calendar Year Benefit Maximum Per Covered Person Deductible $25 per person, per calendar year Orthodontics Covered at 50% for dependent children age 19 or under; lifetime maximum of $1,000 $1,000 $1,500 Covered at 80% after deductible Covered at 50% after deductible $25 per person, per calendar year Covered at 50% for adults and dependent children; lifetime maximum of $1,500 Your dental coverage cost depends on the plan you choose and whether you cover yourself only or yourself and one or more family members: 2018 Bi-Weekly Payroll Deductions* Employee Two Person Family Basic Option $0.00 $11.69 $27.16 High Option $11.63 $33.35 $64.10 *Assumes 100% time and 26 pay periods. If you want to contribute to a Healthcare Flexible Spending Account and/or a Dependent Care Flexible Spending Account for 2018 you must enroll! Your FSA elections will not carry over to 2018, even if you have an FSA in FLEXIBLE SPENDING ACCOUNTS USNH offers two Flexible Spending Accounts administered by WageWorks. Healthcare Flexible Spending Account (FSA) You can participate in a Healthcare FSA (unless you re enrolled in the Open Access Plus HSA plan) to pay for eligible health care expenses, such as your deductible, copays and coinsurance. You can contribute up to $2,600 for the 2018 calendar year. Dependent Care Flexible Spending Account (FSA) You can put aside pre-tax money to pay for qualified dependent day care expenses, such as preschool, before/after-school programs, or child/elder care for eligible dependents. You can contribute up to $5,000 per calendar year if you are married and filing jointly or single, and up to $2,500 per calendar year if you are married and filing separately for VISION PLAN Your vision coverage is provided by Vision Service Plan (VSP) and includes coverage for exams, as well as an allowance for eyeglass lenses, frames or contact lenses. The allowance is increasing to $200 for You can see any provider you choose but your out-of-pocket costs will be lower if you see a VSP preferred provider. Your vision coverage cost depends on whether you cover yourself only or yourself and one or more family members: 2018 Bi-Weekly Payroll Deductions* Employee Two Person Family $2.82 $5.64 $9.06 *Assumes 100% time and 26 pay periods. Your voluntary vision benefits are improving for next year! The allowance for lenses, frames and contact lenses is increasing to $200, plus you ll pay less in bi-weekly payroll deductions for Enrollment Guide 3

6 LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) INSURANCE Life and Accidental Death & Dismemberment (AD&D) Insurance, offered through The Standard Insurance Company, provides a lump sum payment to your beneficiaries in the event of your death or to you if you experience certain injuries. If you are actively working, you can increase your coverage one level or decrease coverage to any level during Benefits Open Enrollment or when you have a qualified change of status. Coverage Options $10, times regular annual budgeted salary 3 times regular annual budgeted salary* 4.5 times regular annual budgeted salary* *Rates for additional Life Insurance are subject to change throughout the year due to age and/or salary changes. LONG TERM DISABILITY INSURANCE Long Term Disability Insurance, offered through MetLife, provides an income benefit to you should you become disabled due to a non-work-related illness or injury. If you are actively working, you can increase your coverage one level or decrease coverage to any level during Benefits Open Enrollment or when you have a qualified change of status. Percent of Regular Budgeted Annual Salary Maximum Monthly Benefit 50% Option $5,000 60% Option $5, /3% Option* $15,000 *Rates for the 66 2/3% option are subject to change throughout the year due to age and/or salary changes. ADDITIONAL VOLUNTARY BENEFITS Benefit Voluntary Short Term Disability Insurance Voluntary Hospital Indemnity Insurance Information Replaces 60% of weekly income for up to 26 weeks, after you ve been unable to work for one day because of a non-work-related accident or seven days because of a non-work-related illness Helps ease the financial impact of a hospitalization Works with your other medical coverage to help pay for out-of-pocket expenses that result from a hospital stay NEW! ENROLL FOR VOLUNTARY SHORT TERM DISABILITY INSURANCE DURING 2018 BENEFITS OPEN ENROLLMENT. Voluntary Short Term Disability Insurance, administrated by MetLife, can be your safety net for continued income if you can t work because of a non-work-related injury or illness. For 2018, you must elect or waive coverage for voluntary Short Term Disability Insurance during Benefits Open Enrollment.
