2 Enrolling For Your 2018 Benefits Action Needed! Welcome to Open Enrollment 2018! If you fail to enroll, your current elections will rollover EXCEPT you will not contribute to an HSA or FSA. You must re-enroll in the HSA and/or FSAs every year. NEW HIRES: Be sure to elect your 2017 and 2018 benefits by the deadline noted. Click on Enroll in Your Benefits to get started! Don t miss out. The Open Enrollment Deadline is 11/15/2017 Have you experienced a life event change? Qualified status changes must be made within 31 days of the qualified life event. To learn more about life events or make changes to your benefits due to a life event change, click here. 2

3 Step 1 Enrolling or Changing Your Benefits When you click on Enroll in Your Benefits, you will see the It s Time to Enroll Screen. Click on Continue to access your personal information screen 3

4 Step 2 Enrolling or Changing Your Benefits Review your benefit selections on the Benefits Summary page You can walk through each benefit by clicking Take Me Through Each Benefit. If you know the changes you want to make, just click on the View/Change button by those plans. You will also see your current benefits and your 2018 elections side by side. Note: You can access additional information by clicking on the options at the top of the page. A drop down box appears for each category. Be sure to re-enroll in the HSA and FSAs 4

5 Step 3 - Your Medical Benefits The following steps guide you through each benefit Make sure you have your eligible dependents selected. Unclick the box to drop coverage for a dependent currently on the plan. To add a new dependent for 2018 click on Add Dependent Note: If you have a life event and want to add a new dependent for 2017 you must go to the Life Events tab at the top of the page Click on Continue to Step 2 or click on Return to Benefits Summary if you don t elect to make a change 5

6 Step 4 - Your Medical Benefits Review your plan options and select the one that best meets the needs of you and your family. Your total cost per pay period appears on the page. Click Save and Continue to return to your benefits summary. Note: Select the Decline Coverage button if you wish to decline your medical benefits 6

7 Step 5 -Your Delta Dental Benefits Select an option for who s covered and then elect or decline coverage. You can cover different dependents for dental than medical. Click on Save and Continue to return to the benefits summary page. Select Continue to Step 2 7

8 Step 6 - Your VSP Vision Benefits Select an option for who s covered and then elect or decline coverage. You can cover different dependents for dental than medical. Click on Save and Continue to return to the benefits summary page. Select Continue to Step 2 8

9 Step 7- Electing a Health Savings Account (HSA) If you enroll in the Anthem Health Savings Plan (HSP), you are eligible to set aside pre-tax dollars in an HSA. Enroll in a health savings account (HSA) to take advantage of NetApp s 2018 contribution and save more pre-tax from your paycheck. Note: The system takes into account NetApp s annual contribution in calculating the maximum you can contribute from your paycheck. Enter the amount you want to contribute per pay period to your HSA account. You must agree to the terms and conditions before you can click on Save and Continue. Note: You can change your election at any time during the year. 9

10 Step 8 - Electing a Limited Purpose (HSA Compatible) Flexible Spending Account If you are contributing to a health savings account (HSA), you are ineligible to enroll in NetApp s general purpose health care FSA, but may participate in the limited purpose (HSA Compatible) FSA to pay for qualified dental and vision expenses. Enter the amount you want to contribute per pay period. The maximum allowed contribution for 2018 is $2,600. Click on Save and Continue. Note: Each year you are allowed to rollover $500 from this account into the next year. 10

11 Step 9 - Electing a Day Care (Dependent Care) Flexible Spending Account The Day Care (Dependent Care) Flexible Spending Account (DCFSA) allows you to use pre-tax dollars to reimburse expenses for child day-care and adult day-care for an elderly parent or disabled spouse. See important note below regarding use of a DSCFA. Enter the amount you want to contribute per pay period. The maximum allowed contribution for 2018 is $5,000. Click on Save and Continue. Note: You must reenroll in FSAs each year. Your election will NOT rollover to next year 11

12 Step 10 - Basic Life and AD&D Insurance NetApp automatically covers you for 2x s your basic salary for life insurance and accidental death and dismemberment (AD&D). You do not have to enroll for this benefit. Note: Be sure to review your beneficiary designations annually. Click Edit to add or change beneficiaries or benefit percentage. Click on Save and Continue. 12

13 Step 11 - Supplemental Life Insurance NetApp provides you the option to purchase up to 8x s your salary in supplemental life insurance for yourself. This is in addition to the basic life insurance coverage provided by the company. If you are a new hire, take advantage of your one-time opportunity to purchase supplemental life insurance of up to 3 times your pay with no questions asked. Be sure to designate your beneficiaries. You can change your beneficiaries or the benefit percent at any time. Note: EOI is required for all new elections or any increases. You can complete the EOI form on line for MetLife s review 13

14 Step 12 - Supplemental Spouse Life Insurance NetApp provides you the option to elect supplemental life insurance for your spouse. New hires can elect up to $50k for your spouse without requiring a statement of health (EOI). Any increases during open enrollment require EOI Note: You must have your spouse enrolled as a dependent in order to purchase supplemental spouse insurance. 14

15 Step 13 - Supplemental Child Life Insurance NetApp provides you the option to elect supplemental child life insurance. Coverage purchased for your child(ren) will pay a benefit to you if your child should die. Coverage amount is $20,0000 per child. Note: You do not need to have a child/children enrolled as dependent(s) to purchase supplemental child life insurance. Select No Coverage or the Flat Amount Coverage and click on Save and Continue. 15

16 Step 14 - Supplemental AD&D Life Insurance Supplemental AD&D insurance pays benefits if an accident leads to serious injury (for example, loss of limb, hearing or vision loss, paralysis) or death. NetApp provides you the opportunity to elect additional AD&D coverage. Note: Click Edit to add or change beneficiaries or benefit percentage. Click on Save and Continue. 16

17 Step 15 - Supplemental Spouse AD&D NetApp provides you the opportunity to purchase additional AD&D coverage for your spouse. Note: Your Spouse does not need to be enrolled as a dependent for you to elect supplemental spouse AD&D coverage. Any amount may be elected without a statement of health (EOI). 17

18 Step 16 - Supplemental Child AD&D NetApp provides you the opportunity to purchase additional AD&D coverage for your child(ren) Note: You do not need to have a child/children enrolled as dependent(s) to purchase supplemental child AD&D Select No Coverage or the Flat Amount Coverage and click on Save and Continue. 18

19 Step 17 - Group Legal Plan Hyatt Legal Plans offers discounted services on a wide range of legal matters, such as wills, living trusts, real estate issues, civil litigation, adoption and taxes. Note: You can only enroll in the Group Legal Plan as a new hire or during Open Enrollment be sure to elect this option if you want coverage in 2018 Select No Coverage or Group Legal and click on Save and Continue. 19

20 Step 18 - Business Travel Accident Insurance NetApp automatically provides you additional life insurance benefits when you travel for business at no cost to you. You do not need to enroll for this benefit. However, be sure to review and update your beneficiary designations. Click on Edit to add beneficiaries and change benefit percent. Click Save and Continue. 20

21 Step19 - Confirm Your Benefits You re almost there! Click Complete Enrollment to move to the final step to confirm your benefit changes 21

22 Step 20 - Enrollment Confirmation You will see a confirmation and recap of your new benefits including a cost summary IMPORTANT!! Your enrollment is not complete until you click on Complete Enrollment 22

25 Congratulations! You ve Completed Your Open Enrollment! Your first payroll deduction for your new benefits will take place on your January 5, 2018 paycheck. If you join midyear, it will be the next payroll cycle. If you did not review your beneficiaries, you can go back in and do so at anytime. Be sure to update your beneficiaries whenever you have a life event, such as getting married or divorced, or having a baby. 25
