3 3 Open Enrollment includes: Medical, Dental, Vision, Flexible Spending Accounts and Voluntary Benefits It is the one time each year you are able to make changes to your coverage (add or drop dependents, change plans, etc.) unless you experience a qualified mid-year status change such as birth, marriage, divorce, death, etc. and notify HR Services within 31 days If you do not make new elections in November your current elections will continue into 2016 except for FSA which requires an active election each year.

UHC introduces Virtual Visits Co-payment change ($25 / $45) Co-insurance change (80%/20%) Choice Plus Family deductible $1250 Opt Out / waive medical - $24 bi-weekly incentive eliminated First time HSA members receive incentive No plan design changes Rx, dental or vision New ID cards for UHC Choice Plus

5 5 HSA Health Savings Account Medical Plan Company Contributes to the Employee s HSA Bank Account Each pay check $20 (individual) or $40 (family) each bi-weekly pay check One-time contribution to first time members in January $250 (individual) or $500 (family) Employee can contribute through pre-tax payroll deductions IRS HSA annual contribution limits from both company and employees: A total annual contribution max of $3,350 (individual)/$6,750 (family) If over age 55, contribution limit increases by $1000 Preventive medications no cost to employee (updated list available to verify your Rx) No change to annual plan deductible $1,300 individual $2,600 family

6 6 UHC Introduces Virtual Visits Log into Available 24/7 effective 1/1/16 See and talk to a doctor using your mobile device or computer Non-emergency issues Most visits will last minutes 2 Networks: Doc-on-Demand & Amwell Not all providers can write prescription in all states

7 7 FSA Flexible Spending Accounts Medical FSA Must elect annually, per the IRS; limit is $2550 Use to reimburse you for eligible out of pocket medical, dental, vision expenses Estimate carefully use it or lose it by December 31 Limited FSA for those who elect the HSA plan option Use a Limited FSA for eligible dental or vision expenses ONLY Dependent Care FSA Must elect annually Used for eligible day care expenses Minimum per household $200; maximum $5000

8 8 Life and Long Term Disability If currently enrolled in supplemental coverage, you may increase coverage by one level (one times your base salary) You may apply for an increase of more than one level* of life insurance coverage If you have spouse /domestic partner life insurance coverage, you may increase one level ($10,000) up to $40,000 Any elections / changes are initiated at the Aetna website or by calling Aetna at * Proof of Good Health will be required

9 9 Life and Long Term Disability (con t) Spouse/ domestic partner life insurance coverage that increases more than one level and/or above $40,000 * If applying for employee supplemental life or spouse/dp supplemental life for the first time * You may enroll for child supplemental life insurance coverage of $5,000 or $10,000 You may change how your Long Term Disability coverage is paid, by you or by the company (if paid by you any benefit you receive would be tax free) * Proof of good health will be required
