1 Open Enrollment Handbook Welcome to Open Enrollment 2013! Orange County is committed to supporting your good health and quality of life; and your health and welfare benefits are one way we put this commitment into action. Open Enrollment takes place from October 8 through October 26, 2012, and is your once-a-year opportunity to review these valuable benefits and make important choices for the coming year. We have good news about our benefits for 2013! First of all, there will be no increases to premiums and no major plan design changes for our health and welfare plans. Also, premiums for Additional Employee Life and Spouse Life Insurance will decrease, and there will be no medical underwriting required for life (up to the Guaranteed Issue amounts) or disability insurance this year even if you or your dependents have been denied in the past! Other changes this year are federally mandated and include an increase in the HSA contribution limits to $3,250 and $6,450; an increase in the minimum deductible of high deductible health plans to $1,250 and $2,500; and a contribution limit of $2,500 on Medical and Limited Purpose FSAs. Before you make your enrollment choices for 2013, please take a moment to review your current benefits and covered dependents on myocinfo so you are prepared to make any needed changes during Open Enrollment. We also encourage you to carefully review this Open Enrollment Handbook, along with the valuable resources available on the County s website, to learn more about your options. NEW THIS YEAR! Enroll using myocinfo This year, you will enroll in your benefits using myocinfo, the same system you use to apply for County jobs, change your address, and view and print your paystubs. TiP Is Open Enrollment Mandatory? Do you want to receive the High Plan Contribution? Will you be enrolling in a County medical plan and want to avoid the Tobacco User Surcharge? Will you be waiving a County medical plan and want to receive the Opt-Out Credit? Do you want to elect an FSA? If you answered YES to any of the above questions, then you MUST participate in Open Enrollment and submit your elections on myocinfo. TiP You must log in to myocinfo between October 8 and October 26, 2012, to complete your Open Enrollment elections. MyOCinfo can be accessed from any computer that has internet access, and you can launch it directly from the Open Enrollment page of the County website ( or from the CountyLine intranet. From the County website, click on the Open Enrollment 2013 slider. From the CountyLine intranet, scroll down to HR Corner, Employee Self Service, and click on MyOCinfo Apply for Jobs; View/Update Your Personal Information.

2 Medical Plan Highlights High Deductible Health Plan (HDHP) The HDHP is made up of two parts the Medical Plan and the Employer Contribution The Medical Plan: Annual deductible, 20% coinsurance and out-of-pocket maximum Pharmacy coverage without a separate deductible Preventive care coverage of 100%, even before you reach your deductible Preventive drugs covered outside of the deductible The Employer Contribution: Helps offset the HDHP deductible Contribution based on level of medical coverage elected during Open Enrollment $750 contribution for employee only and $1,250 contribution for employee plus dependent(s) coverage Contribution can be made into a Health Savings Account (HSA), a medical Flexible Spending Account (MFSA) or a Limited Purpose FSA (LPFSA) TiP You can only make changes outside of Open Enrollment if you have a qualified status change event such as marriage, birth, divorce, death, adoption, change in spouse s employment, etc., and request the changes within 60 days of the event. However, you will not be able to change between the medical or dental plan options (High Plan to Low Plan, for example) until Open Enrollment of the following year. Low Deductible Health Plan (LDHP) The LDHP is made up of two parts Copays and a Deductible You pay Copays year-round for the following services: Doctor s office visits Specialist office visits Outpatient surgery in a non-hospital based facility Outpatient mental health/substance abuse Urgent care Prescriptions The remaining medical services are subject to the following plan design: Annual deductible, 20% coinsurance, and out-of-pocket maximum Preventive care coverage of 100%, even before you reach your deductible Tobacco User Surcharge In response to the increasing cost of delivering employee healthcare benefits and the overwhelming evidence that tobacco use is a leading cause of serious illness, Orange County assesses a tobacco user surcharge of $25 per pay period. The surcharge is applicable only to employees enrolled in either the HDHP or LDHP. To avoid the surcharge during 2013, you must acknowledge your non-tobacco use and submit the Tobacco Certification during your online open enrollment Open Enrollment Handbook

4 Which Health Account Can You Have? In 2013, Orange County will again offer three different pre-tax, health-related accounts for health expenses: a Health Savings Account (HSA) a Medical Flexible Spending Account (MFSA) a Limited Purpose FSA (LPFSA) Use the chart below to compare the three health-related accounts and determine which you are eligible to participate in. Should you wish to contribute your own pretax dollars to a health-related account, you must make that election on myocinfo during Open Enrollment. 5 Do you want to receive up to $1,250 next year? The County offers two financial contributions based on your Open Enrollment elections. If you are a benefitseligible employee, you can receive a $750 or $1,250 contribution if you elect the High Deductible Health Plan (HDHP), or up to $25 per pay period if you decline both of the County s UHC medical plans. Review the information below to learn more about these contributions. High Deductible Health Plan Contribution Only for employees who select the HDHP on myocinfo during Open Enrollment Deposit amount is based on your Open Enrollment election as of 10/26/2012 (Employee Only = $750; Employee plus Dependents = $1,250) You must submit elections on myocinfo between 10/8/2012 and 10/26/2012 to receive this contribution Deposit will be made to your Optum HealthBank HSA in January 2013 if you meet HSA eligibility requirements and have an active HSA with the BCC group number by 10/26/2012, or to your FSA if you don t qualify for an HSA Opt-Out Credit Up to $25 per pay period when you decline both of the County s UHC medical plans Can be used to offset TRICARE supplemental plan, dental, vision and/or supplemental life insurance premiums You MUST submit elections on myocinfo between 10/8/2012 and 10/26/2012 to earn the Opt-Out Credit Are you eligible for an HSA? 7 $ Other Benefits Dental For 2013, there are no plan, coverage or rate changes in the Dental Plan. Vision There are no plan, coverage or rate changes in the Vision Plan for Life Insurance Basic Life Insurance of one times your annual salary will continue to be paid for by the County. If you are interested in Additional Life Insurance, you will be happy to know that Additional Life Insurance and Spouse Life Insurance rates are going down in 2013! Disability The County will continue to provide Long-Term Disability Insurance for all benefits eligible employees in Long- Term Disability Insurance has a 180-day waiting period, so if you wish to reduce your waiting period, you should purchase Short-Term Disability Benefits. Rates for Short-Term Disability will remain the same in 2013 and your buy-down options will be shown on myocinfo during open enrollment. Dependent Care Flexible Spending Account If you wish to contribute pre-tax dollars into a Dependent Care Flexible Spending Account in 2013, be sure to make your election on myocinfo during open enrollment. Current deductions for this benefit DO NOT roll over from year to year. The annual maximum contribution to a Dependent Care Flexible Spending Account is $5,000. This limit is not related to any of your other HSA or FSA contributions. For full plan information on these benefit offerings, please refer to the 2013 Benefits Handbook which is available on the County s website at ocfl.net. TiP A one-year exception to the normal medical underwriting rules is in effect for the 2013 open enrollment period! For Open Enrollment 2013 only, no medical underwriting is required for Additional Life, Spouse Life or Child Life Insurance when elected or increased up to the Guaranteed Issue amounts during Open Enrollment, or when you elect Short-Term Disability Insurance or choose to decrease your waiting period even if you or your dependents have been previously denied coverage. After open enrollment ends, any request to elect or increase Additional Life, Spouse Life or Child Life Insurance, or elect or decrease the waiting period for Short-Term Disability, during any future open enrollment periods will require medical underwriting. TiP Dental has a Late Entrant Penalty! If you waive dental coverage for yourself or a dependent and then choose to add dental coverage at a later date, no benefits will be payable for expenses incurred for any procedure during the first 12 months, except exams, cleanings, bitewing x-rays and child fluoride Open Enrollment Handbook 7

12 Mandatory Open Enrollment What You Need to Do Review Your Current Benefits Take a few minutes to review your current benefits and covered dependents on myocinfo so you are better prepared to make any needed changes during Open Enrollment Review the Open Enrollment Resources Visit and click on the Open Enrollment slider to review and download resources that will help you complete your 2013 benefits enrollment. Attend a Benefits Fair Do you have a question about a particular benefit? Who better to ask than the benefits vendors themselves? Attend a Benefits Fair and meet the providers! Fairs will be held at numerous locations throughout the County from 10/4/2012 through 10/22/2012. Complete the Online Enrollment Worksheet The Online Enrollment Worksheet will help you to plan ahead and gather the information you will need to complete your enrollment online. The Worksheet is available in this handbook as well as online. Attend an Enrollment Assistance Session Do you need help completing your enrollment online? Attend an assistance session with HR and get hands-on help with making your 2013 benefits elections online. Refer to the list of assistance sessions in this handbook or contact your Department HR representative for a location near you. Log in to myocinfo To enroll in your benefits, you must first log in to myocinfo. This is the same system that you use to view and print your paystubs, apply for County jobs, etc. Use your normal user ID and password to log in. You can launch myocinfo directly from the homepage of the Orange County website or from the CountyLine Intranet. Receive the HDHP County Contribution ($750/$1,250) If you elect the High Deductible Health Plan, you will receive a contribution from the County. To receive the County contribution, you must verify HSA eligibility and elect either the HSA or FSA deposit on myocinfo between 10/8/2012 and 10/26/2012. Submit Your Elections You must click the Submit button to send your final enrollment choices and receive the County contribution, Opt-Out Credit, and to avoid the Tobacco User Surcharge. Review and Print Your Enrollment Elections Summary After submitting your elections on myocinfo, be sure to print your Enrollment Elections Summary page. Review it to make sure you didn t make any mistakes and keep it in a safe place with all of your other important records. No corrections or changes will be made once Open Enrollment ends on 10/26/2012. Open a Health Savings Account Do you need to open an HSA? A link to the Optum HealthBank website is available on the Open Enrollment page of the County internet. Open your HSA no later than 10/26/2012. Submit Proof of Dependent Eligibility Are you adding dependents who were not previously covered or making changes to a dependent s name, birthdate or relationship?
