29 Life Insurance NetApp automatically provides basic coverage at no cost to you Basic You 2 times your pay, up to $1.5 million* How much coverage do you need? What s the cost of supplemental coverage? Details at netappbenefits.com You can elect to pay for supplemental coverage during enrollment (approval subject to medical proof of insurability)** Supplemental You 1-8 times your pay, up to $4 million Your spouse $10,000 to $500,000 (in increments of $10,000) Up to 100% of your coverage amount or $500,000, whichever is less Your child(ren) $20,000/child to age 26 Children can be covered by only one employee in the plan + Will preparation and estate planning at no extra cost * As required by the IRS, you will be taxed on any company-provided life insurance coverage that exceeds $50,000.
