Prescription Drugs (CVS Caremark) Highlights Health Choice and Health Maintenance In-Network Retail - $0 for generic, 30% coinsurance (up to a maximum of $75 per script) for preferred;40% coinsurance (up to a maximum of $250 per script) for non-preferred; 30 day supply Mail Service: $20 for generic, 30% coinsurance (up to a maximum of $350 per script) for preferred; 40% coinsurance (up to a maximum of $500 per script) for nonpreferred; 90 day supply NOTE: There is an annual prescription out of pocket maximum of $2,600 ($5, 200 for family) which includes coinsurance and copays for both retail and home delivery. NOTE: Dependent verification documentation must be provided in order to cover dependents under Company benefits. Documentation includes items such as copy of government issued marriage certificate and recent tax return for spouse and birth certificates for children.

A Surcharge may be applicable for employees with spouses enrolled in the medical plan. You may receive a $0 biweekly credit per paycheck if you complete an online health questionnaire. If you and your covered spouse each complete the questionnaire the credit increases to $20 per paycheck In addition, if an employee is a nontobacco user they may also receive a $5 per paycheck non-tobacco user biweekly credit. Health Investment Plan: you will pay 20% of the discounted cost of your prescriptions innetwork after you have met the deductible. Generic preventive medications are covered at 00% in-network. Preventive medications that are not generic are covered at 80% without you having to meet your deductible first. Health Select Plan: : you will pay 30% of the discounted cost of your prescriptions innetwork after you have met the deductible. Generic preventive medications are covered at 00% in-network. Preventive medications that are not generic are covered at 70% without you having to meet your deductible first. In-network coinsurance is always applied to the discounted rates for the services provided. Employee Assistance Program (EAP) Dental Cigna PPO Vision Flexible Spending Accounts Employee Assistance Program is available to all employees and family Members. DHMO (Cigna Dental Care) if available in home zip code; if not available in home zip code you will be offered the Out of Area dental plan Vision Coverage (Superior Vision Services, Inc.) Even if you don t enroll in the vision program, a discount only plan is provided for all employees and their family at no cost. These optional accounts allow you to set aside beforetax dollars to pay for qualified health care and dependent care expenses. 4 Vacation Buy Sick Leave/ Sickness in Family The company allows you to purchase up to 40 hours (5 days) of additional vacation. Paid sick leave is available when personal illness or injury prevents you from working. Available during annual enrollment You may purchase up to 40 hours deducted from all pay periods on a before-tax basis. This benefit may replace part of your income if you have a qualifying disability that prevents you from working for over six months.
