The last change you make before the website closes will be your election for Be sure to click Finish, then print the confirmation page and keep for your records. 3
4 SHBP Website at ADP Page 2 DO NOT PROCRASTINATE! Login problems are common. The closer to November the longer the hold time at Sometimes you can t get through at all. Passwords expire after 45 days. If you can t remember last year s password, click Forgot password and follow the prompts. You can have it ed or texted to you, but be sure you have the same or phone number as last year. Check your Clutter, Spam, and Junk folders if you chose to have it ed and don t see the . 4
5 SHBP Website at ADP Page 3 Too many login attempts will lock you out for 1 hour. After that you can try again. Questions or problems with the SHBP website must be directed to FCS has no power or authority over the SHBP website. FCS can not reset your password. The call center will not speak with us about your account. 5
6 SHBP Website at ADP Page 4 Recommendations: Access the SHBP website prior to OE to ensure you will be able to log in. Check your address on the SHBP site. If incorrect, Benefits with your ID# and the correct address. The health plan and tier you have this year will rollover to next year unless you make a change. If you don t have health coverage with us and don t want it, do nothing. There are no automatic defaults to tobacco surcharge. 6
7 Employee Express Changes Page 1 Go to Employee Express to elect, drop, or change: --DENTAL --SHORT/LONG TERM DISABILITY --FLEXIBLE SPENDING ACCOUNTS --VISION --LIFE INSURANCE --LIFE LOCK Flexible Spending MUST be re-elected each year. HFSA-Medical Flexible Spending Account NFSA-Dependent Day Care Flexible Spending Account If you have Dental, you will not need to re-elect the coverage this year. 7
8 Employee Express Changes Page 2 Before making elections in Employee Express, be sure to add your dependents. Social Security Numbers, Dates of Birth, and phone numbers are required for any dependents. If electing Dental and Vision, be sure you specify which dependents should be covered. If you can t see all the fields to enter, maximize your window. Be sure to walk through all six steps and Save before you exit. Not saving = Never happened. Print and keep a confirmation for your records. 8
9 True OE Short Term Disability (STD) Plans Page 1 STD replaces your salary if you become ill or injured and have exhausted your sick leave. Without sick days or STD, you may have little or no income to live on. We offer two options depending on your needs A 15 day waiting period that replaces 50% of salary at a semimonthly cost of $1.36 per $1,000 of monthly salary A 45 day waiting period that replaces 60% of salary at a semimonthly cost of $0.96 per $1,000 of monthly salary 9
10 True OE Short Term Disability (STD) Plans Page 2 No health questionnaire required to elect STD this year if you elect it, you will have the coverage. Pre-existing clause still applies so, for example, someone already pregnant won t have the pregnancy as a covered disability. Encourage anyone who does not have STD to consider it, unless they do not need their salary to live, or have 6 months of sick leave. 10
11 No OE for Group Term Life Plans Health questionnaire required to elect or increase coverage for 2017 You must elect coverage on yourself equal to or greater than what you will elect on your spouse or child(ren). You must elect your coverage first in Employee Express, then elect for dependents 11
12 Beneficiaries If you have not reviewed your beneficiaries in 5 years, do so. Deaths, divorces, children turning 18, etc. are all reasons to review your beneficiary. Minor children should not be beneficiaries. Benefits does not have your beneficiary on file, they are kept securely at Unum, our life carrier. If you can t remember who you chose, it s time to renew. 12

35 Important Facts about the 2017 Health Coverage Page 1 Provider network will vary between BCBSGA, UHC, and Kaiser. BCBSGA network is identical for HRA and HMO plans. Rx are again managed through Express Scripts for all BCBSGA and UHC plans. Kaiser manages their own. 37 Important Facts about the 2017 Health Coverage Page 3 A single person can t satisfy more than the Single deductible (and OOP) with EE + Spouse, EE + Child(ren), EE + Family A person in acute treatment on 1/1/17 can apply for Transition of Care (continue w/non-participating Dr.) if they change carriers In/Out-of-network deductibles and OOP amounts are completely separate one does not satisfy the other New employees in October and November must make the New Hire election and Open Enrollment election at least one day apart 37
38 Going out to the Federal Exchange SHBP plans are adequate and affordable as defined by ACA Any person may purchase coverage on the exchange for any reason Access to adequate and affordable plan through employer prohibits receiving federal subsidy Any FCS employee purchasing exchange coverage will have to pay the whole cost of the coverage Exchange coverage premiums can NOT be payroll deducted and are after tax 38
39 Strategy For Making an Election Page 1 As always, look at how you use health care. Minimal users might prefer the lower cost plans. People with frequent office visits might prefer an HMO plan. Remember that premiums are always deducted, while you may not always need services. Consider the level of benefits + premium + HFSA amount. 39
40 Strategy For Making an Election Page 2 The choice is going to come down to personal preference about costs, convenience, and freedom. Pay less premium per month, but more for services when they are rendered Kaiser is self-contained only one place to go for everything, and the only plan with no deductible Having freedom to choose an out-of-network doctor means electing an HRA or HDHP plan 40
41 Health Flexible Spending Account (HFSA) Page 1 Set aside amounts to pay co-pays, meet deductibles, and pay for Rx HFSA can also be used for dental and vision services Can be used even if you don t use our health plan Can be used for dependents charges even if spouse/children are not on an FCS plan. Also think braces, glasses/contacts, LASIK, pending surgery, etc. 41
42 Health Flexible Spending Account (HFSA) Page 2 Choose from $300 to $2,550 for the year Set aside only what you know you will spend, because unused money over $500 is forfeited. Up to $500 remaining at the end of the year can be rolled over to E-claims tracker makes reimbursement almost painless. 42
43 Dependent Care Flexible Spending Account (NFSA) Page 1 IMPORTANT! HFSA is used for ALL family member s expenses. NFSA is used only to pay for day care while you work, not for medical expenses for dependents. Set aside amounts to pay daycare expenses for children under age 13 Can also be used for day care for adults unable to be left alone Maximum is $5000 for married, both spouses working or full time students; $2500 for a single person 43
44 Dependent Care Flexible Spending Account (NFSA) Page 2 IMPORTANT! Do not elect Dependent Care FSA to cover dependent medical, dental, or vision expenses. Can be used with any provider that is not paid under the table. SSN or Tax ID# required for reimbursement Can also be used for summer day camp, after school care, or late pickup fees Can be used for adult day care if the person lives with you >7 hours per day and is claimed on your Federal tax return. 44
