5 What is a Flexible Spending Account (FSA)? Flexible Spending Accounts allow you to set aside money to pay for out-of-pocket expenses on a pre-tax basis. There are two types of accounts: Healthcare to a maximum of $2,600. This includes healthcare expenses for your spouse and children up to age 26. Dependent care to a maximum of $5,000 per household and $2,500 for employees who are married and file taxes separately. This is primarily used for daycare. Discovery Benefits is the company that administers our FSAs. They maintain your account(s), pay your claims and issue you a debit card to use when you purchase qualifying services. Learn more at or

9 How can I enroll in benefits or change my benefits elections? As long as you are within the 30-day enrollment period and eligible to make changes, log into Employee Self-Service and choose Benefit Enrollment. If you have a mid-year change due to a life event, click the life event guide for guidance. Remember, you must hit "Submit" to make sure the changes go through.

10 Do I have to sign up for Jefferson s medical benefits? No, benefits are offered to you but you are not required to enroll. However, you do have to actively waive medical coverage online via Employee Self-Service. If you do not, you will be defaulted into the employee-only lowest-cost option.

11 When does my coverage begin? If you have questions, contact your HR Business Partner. You will see deductions the 1 st full pay period after effective date once you become eligible.

If you are on Jefferson's benefits, you must enroll the baby via Employee Self Service (under Benefits and Life Events ) and send supporting documentation, such as a birth record from the hospital, to the HR Operations Center within 30 days of the baby s birth. 16 I m leaving Jefferson when will my benefits end? Jefferson benefit coverage ends for you upon the following events: Medical, vision & dental benefits end on the last day of the month in which your employment ends Life insurance, disability and FSA benefits end on the date your employment ends For example, if your last day worked is September 15 th, your medical coverage will end on September 30 th. Likewise if your last day worked is September 29 th, your coverage will end on September 30 th.

72 I ve resigned from Jefferson how do I get paid for my unused ETO? To be reimbursed for unused ETO, you will need to provide advance notice greater than or equal to your basic ETO or vacation allowance per the Voluntary Resignations policy. Once notice is provided, you are expected to work during the notice time to be eligible to receive payment of unused ETO time. You will receive accrued ETO up to the max payout or vacation at the time of your next scheduled pay. It will be a live check that will be mailed to your home address of record. Please ensure your address is up-to-date in Employee Self Service (using Google Chrome) before your last day of work.

73 I am an ETO-eligible employee. How much time off am I allowed to take when I have my baby? Depending on your situation, below are the types of leave that may be available to you. You can also reference this FAQ, which addresses preparing for medical leave for birth of a child. Family Medical Leave (FMLA) Eligible if employed for at least 12 months and 1,250 hours worked in the 12 months immediately preceding the commencement of leave. Eligible for no more than 12 workweeks of FMLA in any 12 month rolling period. Medical Leave of Absence MLOA may be granted to employee who either exhausted available leave under FMLA or has not qualified for FMLA Dependent Care Leave of Absence (Baby Bonding) Must have completed at least 1 year of continuous full or part time service. Eligible for up to additional 12 weeks beyond disability time covered under FMLA. May not exceed 24 weeks when combined with FMLA. Runs concurrent to Family Medical Leave. Must complete Non-FMLA application once FMLA exhausts or if not eligible for FMLA.

74 How will I be paid while I m on short term disability? All employees must contact Cigna for short term disability approval. Faculty, clinical faculty, senior administrators, and house staff are eligible for up to 6 months of salary continuation while on an approved leave of absence. For all other employees: The first seven calendar days of leave will be paid from ETO time based on your normal work schedule. If Extended Illness Bank (EIB) hours are available to you, you may access that time until the balance is exhausted or until the medical portion of your leave is over. Once EIB hours are exhausted or if you do not have EIB hours, full time employees will begin receiving Short Term Disability (STD) benefits. STD benefits are 50% of base salary paid by Jefferson. Additional STD benefits of 66 2/3% of salary are available as a benefit option at time of hire or during open enrollment each year. Pre-existing conditions apply to the optional plan. If you have received medical treatment, consultations, diagnostic services or have taken prescription drugs for a condition in the three month period before your effective date, the optional STD will be paid for the first 4 weeks of disability; then basic STD benefits will be paid. ETO hours will be used to supplement short term disability to make you whole until ETO is exhausted.
