2 The Healthy Directions program provides comprehensive, valuable, and affordable benefits to help you and your family live well. When comparing health plans of similar companies, Healthy Directions consistently ranks as one of the best. Together, we have maintained this program through the shared responsibility of managing health care costs. John Deere does its part by offering a variety of health, wealth, and lifestyle benefits. You do your part by making wise health and health care decisions. Benefits Enrollment is October 2 13, There are important items to consider when making your 2018 elections: If you currently have medical and/or dental coverage through John Deere, no action is required to continue your current medical and/or dental elections for If you maintain a Health Savings Account (HSA) and/or Flexible Spending Account (FSA), you must elect contributions for 2018; your current HSA and/or FSA elections will not carry over to

3 Step-by-Step: 2018 Annual Benefits Enrollment Below is a list of step-by-step instructions for making the most of annual benefits enrollment. Step Action 1. Understand your benefit options for Read this guide and review the FAQs on page Determine the coverage tier that s right for you. 3. Estimate the cost of your health care expenses for the coming year. 4. Think about which health care and dental benefit plan options are right for you (or you can opt out of coverage if you have it elsewhere). 5. Decide how much you want to contribute to your HSA, if eligible. Think about who you want to cover (Employee Only; Employee and Spouse; Employee and Child(ren); Employee, Spouse, and Child(ren)). See page 5. Go to myuhc.com to view your past year s claims to see what you spent on health care services. You can also call myhealth at for help. See the Health Plan Selection Guide on page 4. Your 2017 HSA election will not carry over into If you are opening an HSA for the first time or making elections for 2018, you will do so through Fidelity, our HSA administrator. Deere Direct does not manage HSA elections. See pages 7-8. Fidelity offers an online modeling tool to help you decide how much to contribute to your HSA. 6. Decide if you want to contribute to a Limited Purpose Health Care FSA or Dependent Care FSA. See page 8 for more details on these accounts. If you make changes during the annual benefits enrollment period, we will mail you a confirmation letter of your 2018 election changes in November. Please make sure you review the letter. If any information is incorrect, contact Deere Direct at Reminder! You may be required by Deere Direct to submit eligibility verification documents (like birth certificates or a marriage license) if you have not done so previously. 2

John Deere uses this analysis to benchmark our benefits with similarly-situated companies. The average value of all benefit plans offered by each employer is compared and ranked in a Benefit Index. The results of this comparison show Healthy Directions is in the top twenty-five percent of health benefits offered when considering employee premium contributions, point-of-care costs, and company-provided funds. The benefit comparison is for allowed covered benefits provided by network health care providers. Dental The dental plan pays benefits for a broad range of covered dental services. Dental coverage is separate from the health plan, and maintains separate employee premium contributions, deductibles, and annual/lifetime maximums. 7 Virtual Visits The next time you or a loved one is sick, skip the doctor s office waiting room and try Virtual Visits. You can see and talk to a doctor from your phone, tablet, or computer without an appointment. Most visits take about minutes and cost approximately $40. Doctors can write a prescription if needed and you can pick it up at your local pharmacy. Virtual Visits are best suited for issues like allergies, bladder infection, bronchitis, cough/cold, diarrhea, fever, pink eye, rash, seasonal flu, sinus problems, sore throat, or stomachache. To access Virtual Visits: Go to myuhc.com or download the UnitedHealthcare Health4Me app Select a virtual provider group Register with the virtual provider group, then enter a virtual waiting room until the doctor is available FAQs: 2018 Annual Benefits Enrollment Q: How do I find out which health plan I am currently enrolled in? A: On the front of your UHC ID card you will see the name of your health plan. You can also log onto myuhc.com to see your health plan s name, verify who is covered, find providers, view claims, and more. Q: How do I know my changes have been made? A: If you make changes during the annual benefits enrollment period, we will mail you a confirmation letter of your 2018 election changes in November. Please make sure you review the letter. If any information is incorrect, contact Deere Direct at Q: I don t want to make changes to my benefits for 2018, so I don t need to do anything, right? A: It depends on your situation. If you are satisfied with your current coverage, you do not need to take action to continue receiving those benefits in If you want to contribute to an HSA or FSA for 2018, you must actively elect how much you wish to contribute. 8 Wealth The wealth benefits John Deere offers through Healthy Directions provide opportunities for you and your family to save pre-tax dollars for health care expenses, and receive contributions from the company to help you pay for health care Highlights Health Savings Account (HSA) Your 2017 HSA election will not carry-over into Go to netbenefits.com or call Fidelity at to make your HSA elections. If you are opening an HSA for the first time, you must do so by Dec. 29, 2017, in order to receive future company contributions deposited pre-tax into your HSA account. Annual IRS limits on how much can be contributed to an HSA: $3,450 (Employee Only); $6,900 Employee and Spouse; Employee and Child(ren); Employee, Spouse, and Child(ren)). John Deere still provides a contribution of $750 for Employee Only coverage or $1,400 for Family coverage. Flexible Spending Accounts (FSA) The contribution limit for the Limited Purpose Health Care FSA is $2,600, an increase from Beginning Jan. 1, 2018, any amount carried over from your Limited Purpose Health Care FSA must be used in the subsequent plan year or forfeited. In order to utilize your 2017 carryover, you must participate. The contribution limit for the Dependent Care FSA is $5,000, same as Call Deere Direct at if you want to contribute to an FSA. Did You Know? HSAs are a great way to budget for your annual health care expenses! If you are paid semi-monthly and you contribute just $98 each paycheck to your HSA on top of what John Deere contributes to your account you ll have enough money to cover the out-of-pocket maximum in for Employee Only coverage. Setting aside money each paycheck helps build an HSA safety net so you can handle an emergency medical situation or save for future health care needs. Plus, since you save $1 on taxes for every $3 to $4 you contribute to your account, your money goes further. 7

9 Health Savings Account (HSA) An HSA offers a tax-favored way for you to pay for current and future qualified health care expenses, including your deductible, coinsurance, and eligible medical expenses not covered by our health plans. Contribute Grow Know John Deere will contribute $750 (Employee Only) and $1,400 (Family) to your HSA In addition, you can contribute up to $2,700 (Employee Only) or $5,500 (Family) to your HSA If age 55 or older in 2018, you can make a $1,000 catch-up contribution You can change your contributions any time during the year If you re enrolled in either or MAX and are not eligible for an HSA*, you ll receive the company contribution automatically through your paycheck rather than a company deposit into an HSA. The company contribution will be taxable income The money you and John Deere contribute to your HSA goes in tax free When HSA funds are used for eligible medical expenses, the money comes out tax free If your account reaches $2,000, you can invest your money with mutual fund options (just like a 401(k)) Any remaining HSA balance at year s end rolls over; there s no use it or lose it rule Account balance can continue to grow for use on eligible medical expenses in retirement To make HSA contributions, you must actively elect how much you want to contribute per paycheck HSA funds can be used for qualified health care expenses, including out-of-pocket vision and dental expenses New HSA participants must open their account by December 29, 2017 to receive a pre-tax contribution from John Deere in January 2018 Medicare enrollees are not eligible to contribute to an HSA; other eligibility requirements apply* The HSA is yours to keep; you take the money with you if you retire or leave the company *To see other eligibility restrictions, please search for Publication 969 on Flexible Spending Accounts (FSAs) A Limited Purpose Health Care FSA uses pre-tax dollars to pay for out-of-pocket vision and dental expenses in combination with an HSA. A Dependent Care FSA uses pre-tax dollars to pay for dependent care expenses*. Contribute Grow Know John Deere does not make contributions to FSAs You can contribute pre-tax dollars to a Limited Purpose Health Care FSA and/or a Dependent Care FSA To change contributions during the year, you must experience a qualifying life event, such as a marriage or birth of a child There is no opportunity to invest FSA funds The Limited Purpose Health Care FSA allows you to roll over up to $500 of unused funds. However, you must remain an active participant in the upcoming year by making contributions to your account, or you forfeit these funds. Any balance above $500 remaining at year-end will be forfeited, per IRS restrictions The Dependent Care FSA is use it or lose it ; any remaining balance does not roll over year to year, per IRS restrictions To make contributions to any FSA, you must actively elect the total annual amount to contribute The Limited Purpose Health Care FSA is for qualified out-of-pocket dental and vision expenses only The plan limit for Limited Purpose Health Care FSA contributions is $2,600 per household; the minimum annual deferral is $48 The plan limit for Dependent Care FSA contributions is $5,000 per household; the minimum annual deferral is $48 You have until March 31 of the following year to submit for reimbursement. Doing so can help you lead a healthy, productive life and make the most of your health care dollars now and in the future Highlights Fitness Center Reimbursement Beginning in January 2018, John Deere is expanding the Fitness Center Reimbursement program to include programbased activities. Examples of these types of programs include: yoga, pilates, Jazzercise, Farrell s extreme bodyshaping, and Orangetheory. Quit for Life Our tobacco cessation program is now offered through Quit for Life in collaboration with the American Cancer Society. While there is no doubt that quitting smoking is good for your health, we understand that s not always easy. John Deere provides reimbursement for a portion of fitness center memberships. The reimbursement amount is up to $120, per employee, per year.
