1 2017 Annual U.S. Benefits Enrollment Bulletin For COBRA Participants OCTOBER Annual Benefits Enrollment Ends November 11, 2016 Welcome to the annual enrollment period for the JPMorgan Chase 2017 U.S. Benefits Program. You have the opportunity once a year to review your Health Care and Insurance Plan coverage and make any necessary changes. Now is the time to consider these plans and make choices for You and your dependents may be eligible for coverage under the JPMorgan Chase Medical, Dental and Vision Plans. Note: If you don t elect JPMorgan Chase Medical Plan coverage, you can elect access to Onsite JPMorgan Chase Health & Wellness Centers, Wellness Screenings and EAP Counseling as a combined election. Please carefully read the additional details on page 6 prior to electing such coverage. You should consider all alternatives available for coverage, including your spouse s/domestic partner s employer coverage or a public health care exchange. Either of these types of coverage may be more cost-effective than COBRA coverage. Enroll before the November 11, 2016, benefits enrollment deadline, or you and your covered family members will be assigned coverage based on current 2016 elections with 2017 costs. If changes are needed, go to My Health and click Enroll now to make your 2017 elections, or use the online tools available on the Benefits Web Center. 2 If You Are Currently Enrolled in Subsidized COBRA and Are Eligible for Retiree Coverage If you are on COBRA due to severance and your COBRA subsidy continues into 2017 due to your severance, you may continue on COBRA coverage until the subsidy runs out (up to six months after being on COBRA coverage, depending on your years of service). Once the subsidy expires, you have a one-time opportunity to elect retiree health care coverage through the JPMorgan Chase Retiree Medical Plan (if you are pre-medicareeligible) or through OneExchange (if you are Medicare-eligible) for medical, prescription drug, dental and/or vision coverage. There is no deferral of retiree coverage. After your COBRA subsidy expires, you can continue with COBRA coverage for the remainder of your COBRA period (generally 18 months), paying the full cost of coverage. However, at the end of your COBRA coverage, you will not be able to elect retiree coverage through JPMorgan Chase (or receive an HRA with OneExchange). See the As You Retire Guide on My Rewards (myrewards.jpmorganchase.com) for more details. If you have questions about the pre-medicare retiree coverage through JPMorgan Chase, contact the accesshr Benefits Contact Center; see page 14. If you have questions about the Medicare-eligible coverage through OneExchange, they can be reached at , 8 a.m. to 9 p.m., Eastern Time, Monday through Friday. You may also access the OneExchange website for JPMorgan Chase members at Important reminder about paying for your COBRA coverage Your monthly payments are due on the first day of each month. To ensure that payment is received by the due date and to avoid cancellation of your COBRA coverage, you may wish to consider having your payments deducted directly from your checking or savings account via direct debit, if you have not already done so. Please call the accesshr Benefits Contact Center for more information (see page 14 for contact information). Please note: If you fail to remit your monthly payment within 31 days of the due date, your coverage will end retroactive to the last day of the month for which payment was received. 3 What s Changing for 2017 Medical Plan You will continue to have access to the Consumer Driven Health Plan (CDHP) Option 1 and Option 2, offered through Cigna and UnitedHealthcare (UHC). The key difference between CDHP Option 1 and Option 2 is that Option 1 has higher monthly contributions and a lower deductible and coinsurance maximum, while Option 2 has lower monthly contributions and a higher deductible and coinsurance maximum. CDHPs encourage participants to become better consumers of health care services and take a more active role in managing and improving their health. To get the most out of these types of plans, participants need to be more involved for example, by researching treatment alternatives and provider quality and taking part in wellness programs to improve and maintain their health. Change in out-of-network coinsurance for 2017 Medical Plan options The out-of-network coinsurance amount JPMorgan Chase covers will change from 60% to 50% after the deductible. Keep in mind that in-network preventive care is covered at 100% before the deductible and in-network primary care office visits are covered at 90% before the deductible, while most other in-network care is covered at 80% after the deductible. We re increasing the amount you pay for out-of-network medical care due to rising medical costs and because an increasing number of participants are selecting out-of-network providers and services, which cost more for all participants and JPMorgan Chase. Selecting in-network providers and services is more cost-effective. This change is a continuation of our strategy to help drive in-network care. Additionally, to help make it easier for you to find in-network care, Cigna and UHC continue to increase the size of their networks by adding doctors and hospitals. Use the Doctor Search tools on your health care company s website to identify quality and cost-effective providers in-network available through My Health. You and your covered dependents can also call your health care company (Cigna/UHC) or Health Advocate. JPMorgan Chase is also changing the coinsurance amount from 60% to 50% after the in-network deductible is met when you use the emergency room (ER) when it s not a true emergency. Examples of not a true emergency are non-life-threatening situations like a sprain, cold, sore throat or ear infection. These examples can be handled through your primary care physician, a virtual doctor visit, convenience care clinic or urgent care clinic for less expense than the ER. New Virtual Doctor Visits Coinsurance is the percentage of eligible medical expenses the plan pays, generally after you meet the annual deductible. Virtual doctor visits allow you to connect to a doctor in minutes anytime, anywhere using a smartphone, tablet or computer for $5 or less per virtual visit. Doctors can make diagnoses, provide advice and call in prescriptions to your local pharmacy. 4 Prescription Drug Plan update Your prescription drug coverage is part of the Medical Plan and is administered by CVS Caremark. Though it is part of the Medical Plan, it has a different plan design, with separate deductibles and a separate safety net for covered drugs in the form of per-prescription maximums and annual out-of-pocket maximums. Your prescription drug plan uses CVS Caremark s standard drug lists (Specialty and Non-specialty) of covered and excluded drugs. These lists are subject to change by CVS Caremark and can be viewed at any time on the CVS Caremark website. An independent committee made up of pharmacists, physicians and medical ethicists reviews and approves these drug lists (also known as a Formulary). Although there are no changes for 2017, the CVS Caremark Standard Formulary List will continue to exclude additional drugs from time to time. To access the Standard Formulary List, visit the CVS Caremark website or log on to My Health and click Prescription Drugs. If you choose to take a non-covered drug, you will pay the full cost of the drug. Note: Your physician can contact CVS Caremark to seek medical-exception approval for specific clinical reasons. The plan contains a mandatory generic drug program. This means generic drugs will be substituted for all brand-name prescription drugs with a direct generic equivalent. If you or your doctor requests a brand-name drug when a direct generic equivalent is available, you'll pay the difference in cost between the brand-name and the generic drug, plus the generic copay (i.e., the brand-name drug cost minus the direct generic drug cost plus your generic copay). Note: These cost differences will not be limited by per-prescription maximums or annual out-of-pocket maximum limits. Look under Important Messages on the CVS Caremark website for instructions on how to learn more about your 2017 Prescription Drug Plan design and costs. You can access the CVS Caremark website through My Health > 2017 Benefits Resources. Dental and Vision Plan benefits There are no changes to the Dental and Vision Plans for A word about dependent eligibility If you are enrolled in benefits, you may also cover eligible dependents (e.g., spouse/domestic partner, children). Children are eligible until the end of the month in which they turn age 26. 5 Important Steps to Take by November 11: Your Enrollment Checklist Enroll for your 2017 benefits by November 11. Go to My Health and click Enroll now to make your 2017 benefits elections. You can access My Health via the Internet at myhealth.jpmorganchase.com. Review your Medical Plan options, and elect the coverage that is best for you and your family. Compare your options both within the JPMorgan Chase Medical Plan and with any other plans available to you, such as through your spouse s/domestic partner s employer or other options available to your spouse/domestic partner and/or child(ren), such as through a public health care exchange. Take everything into consideration when making your choice. Go to My Health and click Enroll now to access the Benefits Web Center, where you can review the Health Plan Comparison Charts and compare the costs and coverage provided under each option. Decide whether your health care company is still the right fit for you. Both Cigna and UnitedHealthcare (UHC) offer COBRA participants the same design and monthly contributions. Check out the tools, resources and wellness programs each health care company offers, and whether your doctors are in their networks. You can access the health care company websites through My Health, or through the Benefits Web Center. Make sure your provider is still in-network. It s important to check to see if your providers are in-network to receive the most cost-effective level of benefits provided by your Medical Plan option. Go to My Health and access your health care company s website and use ELECTIONS LAST ALL YEAR Elections you make during this annual enrollment period will become effective January 1, 2017, and remain in effect through December 31, 2017, or the end of your COBRA period (whichever is sooner). The only time you can make a change during the year is if you have a qualified status change, like getting married or having or adopting a baby. In the event of a status change, any resulting coverage changes must be made within 31 days after the date of the event. Call the accesshr Contact Center right away if you experience a qualifying event. If you don t make changes for 2017, generally your elections for 2016 will continue at 2017 rates. the Doctor Search tools. You can visit My Health via the Internet at myhealth.jpmorganchase.com. And you may want to check with your provider to make sure he or she plans to stay in-network for Please note: Providers can change networks at any time. If your provider leaves a network, it does not qualify as an event that allows a change to your health care plan option or company during the year. Review the way you pay for health care through your MRA automatic claim payment or debit card to see if it s still the best approach for you. See the Helping You Choose: Automatic Claim Payment vs. Debit Card Tip Sheet on My Health for details. Take a look at your dental and vision coverage to be sure they still meet your needs. If you re happy with them, you don t need to do a thing, but it s always good to revisit them once a year. Update your dependent information. Your Personalized Fact Sheet includes the information currently on file for you and your covered dependents. Please review this information carefully, and make sure each dependent s name, address, date of birth and Social Security number are up to date. This information is important for us to have and is required by the Affordable Care Act (ACA). You can make any necessary changes by visiting the Benefits Web Center via the Enroll now link on My Health. To review JPMorgan Chase s dependent eligibility requirements, please visit My Health > 2017 Benefits Resources. Complete the Initial Wellness Activities (biometric Wellness Screening and online Wellness Assessment questionnaire) between January 1 and December 31, 2016, to earn $200 for your 2017 MRA. You can earn an additional $100 in your 2017 MRA when your covered spouse/domestic partner completes a Wellness Screening and Wellness Assessment during See page 8 for details. Starting January 1, 2017, complete Additional Wellness Activities and earn more MRA funds. You and your covered spouse/ domestic partner must both complete a Wellness Screening and Wellness Assessment to earn Additional Wellness funds COBRA Annual U.S. Benefits Enrollment Bulletin 5

6 Take advantage of tools to help you choose. See pages for details about the tools available through My Health and the Benefits Web Center to help you make your 2017 enrollment decisions. Understand the rules for mid-year changes. If your family situation changes, you may be eligible to make changes to your benefits as follows: Qualified status changes: The annual benefits enrollment period is the only time you can enroll in or make changes to your benefits during the year, unless you experience a qualified family status change (such as gain coverage from another source or in the case of divorce). Electing JPMorgan Chase Onsite Health & Wellness Centers, Wellness Screenings and Employee Assistance Program (EAP) Counseling You may elect access to JPMorgan Chase s Onsite Health & Wellness Centers, participate in Wellness Screenings and utilize EAP counseling while on COBRA. These benefits are offered as a single election. If elected, all covered dependents may access EAP counseling telephonically. Access to JPMorgan Chase s onsite Health & Wellness Centers or Wellness Screening is only available to former employees. Note: If elected, you would access the JPMorgan Chase Onsite Health & Wellness Centers only after coordinating with that facility s security personnel. Also, the Centers offer limited medical services. Therefore, give careful consideration as to whether this is a service you would use. If you are interested in participating, please contact the accesshr Benefits Contact Center and speak to a representative. Please note: If you elect COBRA coverage for the JPMorgan Chase Medical Plan, that coverage includes EAP and Wellness Screening coverage. If you do not elect the Medical Plan COBRA coverage, but would like COBRA coverage for EAP Counseling/onsite Health & Wellness Centers/Wellness Screenings, you must make the COBRA election for those services. If you elect COBRA coverage for these services, you are eligible to earn $200 in 2017 wellness incentives (a taxable incentive payable through payroll) by completing a Wellness Screening by December 31, This is the maximum amount of wellness incentives you can earn. Additionally, your covered spouse/domestic partner is not eligible to earn any wellness incentives. Need more time to make decisions? 7 Your Medical Plan at a Glance The JPMorgan Chase Medical Plan is designed to help you manage your health by providing tools and resources that help you make better decisions about your health and the care you and your family receive. The key components of CDHP Option 1 and Option 2, which continue in 2017, include: You use the money in your Medical Reimbursement Account (MRA) to pay for eligible out-of-pocket medical and prescription drug expenses. You earn funds in your MRA by completing certain wellness activities. Remember that any funds that remain in your MRA at the end of 2016 carry over to 2017 if you remain enrolled in COBRA coverage or retire and enroll in JPMorgan Chase Retiree Medical. In-network age- and gender-appropriate preventive care is covered at 100% with no deductible. This includes physical exams, recommended screenings and immunizations. Virtual doctor visits with a doctor using a smartphone, tablet or computer for $5 or less per virtual visit (with no deductible). In-network primary care office visits are covered at 90% with no deductible. This includes visits to a family practice doctor, internist, 1 pediatrician and OB/GYN, as well as visits to convenience care clinics. Other in-network medical care has a deductible. The Plan generally pays 80% of costs for other care, such as specialist visits, hospital stays, labs and outpatient facilities after an annual deductible is met. Out-of-network care does not require a referral, but you may want to check with your health care company to assure services are covered. You will pay 100% of the cost for services for out-of-network doctors until you reach a separate, higher out-of-network deductible. After this deductible is met, out-of-network eligible medical services are covered at 50% up to the reasonable and customary (R&C) limit. These charges are based on average claims data in your area and are determined by your health care company to be appropriate fees for medical services. You are responsible for the full amount above the R&C limit. The Plan s coinsurance maximum provides you a financial safety net that limits the amount you have to pay in coinsurance each year. Note: There are separate coinsurance maximums for in-network and out-of-network charges. Prescription drug benefits are part of your medical coverage and are provided through CVS Caremark. This benefit has a separate retail deductible for brand-name drugs and a separate safety net for covered drugs in the form of per-prescription maximums and annual out-of-pocket maximums. Eligible generic preventive drugs are covered at 100% (no deductibles or copayments/coinsurance). You pay flat copayments for generic drugs and different coinsurance percentages if you use preferred or non-preferred brand-name drugs. There is a mandatory generic drug program whereby generic drugs will be substituted for all brand-name prescription drugs with a direct generic equivalent. If you or your doctor requests a brand-name medicine when a direct generic equivalent is available, you'll pay the difference in cost between the brand-name and the generic medicine, plus the generic copay. JPMorgan Chase uses CVS Caremark s lists of covered and excluded drugs. If you take a non-covered drug, you will pay the entire cost of the drug. The Employee Assistance Program (EAP) is part of the Medical Plan. 1 Internists must be contracted with Cigna or UHC as primary care physicians (PCP). 8 Learn more about your Medical Plan and how it works on My Health, where you can access tools and resources, such as the Health Plan Comparison Charts through the Benefits Web Center at My Health and/or link to the health care companies websites. How You Earn Funds for Your 2017 MRA A key part of our Medical Plan and wellness strategy is to engage employees in specific wellness activities by providing Medical Reimbursement Account (MRA) funds. JPMorgan Chase contributes money to your MRA, if you elect COBRA Medical, to help you pay for eligible out-of-pocket medical and prescription drug expenses (deductibles, coinsurance and copayments). Complete both a biometric Wellness Screening and an online Wellness Assessment between January 1 and December 31, 2016 When you complete both a biometric Wellness Screening and online Wellness Assessment between January 1 and December 31, 2016, you will earn $200 in your 2017 MRA. If your covered spouse/domestic partner completes both Initial Wellness Activities (i.e., Wellness Screening and Wellness Assessment) in the required time frame, you will earn another $100 in your 2017 MRA. If the Wellness Screening was completed at your doctor s office, make sure the Wellness Screening Results Form (found on My Health) is submitted as soon as possible, but no later than January 31, 2017, to ensure timely processing of any applicable wellness incentives. Note: If you terminate employment during 2016, you must elect COBRA to be eligible to earn Wellness Rewards for Initial Wellness Activities plus continue as a COBRA participant through the award date in mid-january Those employees who terminate before the award date in mid-january 2017 and do not elect COBRA will not receive funds for completing Initial Wellness Activities during 2016, even if you are retirement-eligible. If you terminate employment with an MRA balance and do not elect COBRA, you can still submit claims to your health care company for reimbursement of eligible medical and prescription drug expenses that were incurred before the end of the month in which you terminate. What are a biometric Wellness Screening and an online Wellness Assessment? A biometric Wellness Screening provides overall key indicators of your health. Screenings measure your blood pressure, blood sugar, cholesterol, triglycerides and body mass index (BMI). The Wellness Assessment is a 15-minute online survey that asks you questions about your biometric Wellness Screening results, diet, lifestyle, sleep patterns and health goals. For details, go to My Health. Together, your Wellness Screening and Wellness Assessment results provide you with helpful information about what you re doing well, recommendations for improving your health, and potential issues to discuss with your doctor. JPMorgan Chase does not receive the data from your Wellness Screening and Wellness Assessment. That information goes directly to your health care company. See privacy information on page 11 for more details. Complete Additional Wellness Activities during 2017 while enrolled in COBRA Medical Complete up to four Additional Wellness Activities between January 1 and December 31, 2017, and earn $200 for each (maximum $800) in your 2017 MRA. Your covered spouse/domestic partner can complete up to three Additional Wellness Activities during 2017, and you will receive $100 for each completed activity (maximum $300) in your 2017 MRA COBRA Annual U.S. Benefits Enrollment Bulletin

9 Your MRA adds up When you complete Initial and Additional Wellness Activities during the appropriate time frames, you can earn up to $1,000 in your 2017 MRA. If applicable, your covered spouse/domestic partner can also complete wellness activities during the appropriate time frames, and you can earn an additional $400 in your 2017 MRA for a total of $1,400. See page 10 for details on earning Wellness Rewards. Don t forget MRA carryover funds Any unused funds from your 2016 MRA will carry over into your 2017 MRA. Unused MRA funds are forfeited upon termination of COBRA coverage unless you are eligible to retire. More information can be found in the As You Leave/As You Retire guides on My Rewards. Track completion of your Wellness Screening and Wellness Assessment for 2017 To track the status of completion of your 2017 Wellness Screening and Wellness Assessment on your health care company s website, go to My Health > My MRA Tracker, or go directly to mycigna.com or myuhc.com. Your covered spouse/domestic partner can access his or her health care company website through My Health (myhealth.jpmorganchase.com) or by going directly to Cigna (mycigna.com) or UnitedHealthcare (myuhc.com). If your spouse/domestic partner hasn t already created a username and password on their particular health care website, they will need to do so. Once logged in, your spouse/domestic partner can follow these steps: Cigna: Visit mycigna.com and click on the My Wellness tab. From there, click the View Your Wellness Activities button, which will take you to the Activities page where your spouse/domestic partner will be able to view completed wellness activities. UnitedHealthcare: Visit myuhc.com. Your covered spouse/domestic partner should log in with their own username and password. On the UHC landing page, select Health & Wellness Tools. At the top of the Rally dashboard, there is a customized message box, where you can link to the activities that have been completed. It takes approximately two to three weeks for your health care company to receive and process the Wellness Screening Results, so be sure to allow time for processing COBRA Annual U.S. Benefits Enrollment Bulletin 9

If you did not enroll in JPMorgan Chase medical coverage, Cigna has been designated as your health care company to administer your Wellness Rewards.

11 Important note about your MRA funds if you change your health care company If you decide to change your health care company for 2017 (from UHC to Cigna or vice versa), you ll also be changing the company that administers your MRA. It s important to know that there will be a delay in transferring your unused MRA funds to your new health care company. This delay is designed to allow your current health care company to access MRA funds to pay any late-year 2016 medical and pharmacy claims that are processed in the first four months of If this causes you a financial concern, contact your new health care company, which can help to move those funds faster. You will need to enter your Standard ID number, shown on your enclosed Personalized Fact Sheet, and your JPMorgan Chase Single Sign-On password (see How to Use Single Sign-On on page 16). If you are not a former JPMorgan Chase employee, you cannot enroll through My Health. You may request any of the information in this bulletin by calling the accesshr Contact Center at You must enroll through the accesshr Benefits Contact Center; representatives are available to assist you from 8 a.m. to 7 p.m. Eastern Time, Monday through Friday, except certain U.S. holidays. Benefits Reminders HIPAA privacy rights and protected health information The Health Insurance Portability and Accountability Act (HIPAA) requires employers to communicate how certain protected health information under employee health care plans may be used and disclosed, as well as how plan participants can get access to their protected health information. Accordingly, JPMorgan Chase will distribute once every three years a Privacy Notice of Protected Health Information under the JPMorgan Chase Health Care Plans to you that describes in detail how your personal health information may be used and your rights with regard to this information. (This notice was distributed in December 2014.) Newborns and Mothers Health Protection Act In accordance with the Newborns and Mothers Health Protection Act, group medical plans and health insurance issuers may not, under federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a normal delivery, or to less than 96 hours following a cesarean section. Further, the plan cannot require that any medical provider obtain authorization from the plan or any insurance issuer for prescribing a length of stay less than these periods. Medical Plan post-mastectomy benefits All options under the JPMorgan Chase Medical Plan cover certain breast reconstructive benefits in conjunction with a mastectomy for eligible participants. Coverage under the Medical Plan is available for: Reconstruction of the breast on which the mastectomy was performed; Surgery and reconstruction for the other breast to produce a symmetrical appearance; and Prosthesis and treatment of physical complications for all stages of mastectomy, including lymphedema. This coverage is subject to the terms of the Medical Plan option in which you participate, including relevant deductibles and coinsurance provisions.
