3 Get Benefits That Fit Your benefits are with you all year long. That s why Dominion wants you to have the benefits that best fit your needs. Open Enrollment is your chance to choose benefits for It s important to take action this year for a couple of reasons: You may have to enroll to get the medical coverage of your choice for (See the box to the right for details.) Being enrolled in a medical option is the third and final step toward earning your 2017 medical premium credit of up to $800. Keep reading so you re ready to make Open Enrollment count for you. Go to Your Benefits Resources (YBR) at and use the online tools to compare coverage options and premiums. Enroll for Dominion Medical Coverage in 2017 The best way to have medical coverage that fits you and your family for 2017 is to review your options and enroll by October 28, Here s what will happen if you don t go through the enrollment process: If you re currently enrolled in Dominion medical coverage, you and your covered dependents will be covered by Option B, at your current coverage level, for If you re not currently enrolled in Dominion medical coverage, you will continue to have no medical coverage for Dental, vision, long-term disability and life insurance elections from 2016 will carry over to 2017 unless you make a new election for Health Savings Account (HSA) and Flexible Spending Account (FSA) elections do not carry over from year to year, so you must make new elections for those benefits. Enroll and submit. Go to YBR and enroll in your benefits by October 28. 3

4 What s Changing for 2017 Before you can really find what fits you, you need to know what s changing for Medical Benefits. Hearing aids and exams will be covered with a $2,500 allowance over a 36 month period. Prescription Drug Benefits. Diabetic supplies will be covered under prescription drug benefits through Express Scripts. Previously they were covered through Anthem. Heath Savings Acount (HSA). The HSA contribution limit for employee only coverage is increasing from $3,350 to $3,400. This limit includes Dominion s contribution of $500 (for employee only coverage). The limit for family coverage is not changing. Visit dombenefits.com for more information. Spouse Life Insurance. Domestic partners will be eligible for spouse life insurance. No Changes to Health Care Reform As in 2016, most Americans are required to have medical coverage in 2017 or pay a penalty. Participating in a Dominion medical option meets this requirement. If you wish, you may choose coverage through the public exchange instead of your Dominion coverage. Health Care Cost Summary Tool Knowing how much you spent on health care this year can make it easier to estimate next year s expenses and choose a medical option. See what you spent on health care from January 1 through July 31, 2016 on the YBR Open Enrollment page under Step 3: Review and compare medical plans. Remember: If you are currently enrolled in Dominion medical coverage, and you do not go through the enrollment process for You and your covered dependents will be covered by Option B, at your current coverage level, for If you re not currently enrolled in Dominion medical coverage, you will continue to have no medical coverage for 2017 unless you elect otherwise. 5 Go Enroll Here s a quick look at how to enroll. See the enclosed How to Enroll tip sheet for detailed instructions. Log on to YBR. You can access YBR from DomNet from work or via any Internet connection at If you access YBR from the Internet, you ll need your user ID and your password. From the YBR homepage, click the Enroll in Your Benefits link within the Messages box at the top of the page. You can t miss it! Questions? Call the Dominion Benefit Center at ( outside the U.S.). Representatives are available Monday through Friday, from 8:00 a.m. to 5:00 p.m. Eastern Time. Between October 24 and October 28, representatives will be available until 6:00 p.m. Eastern Time. After You Enroll Be sure to print out your Submitted/Completed Successfully page from YBR so you will have a confirmation of your benefit elections for If you have an address on file, you ll get an confirmation within 30 minutes of completing your enrollment on YBR. An enrollment confirmation will also be mailed to your home address in mid-november. Your elections for 2017 take effect on January 1, 2017 and will last for the entire calendar year, as long as you remain employed by Dominion and meet the eligibility requirements. First-Time YBR User? You ll need to register. Click on Are you a new user? and follow the prompts to create your own user ID and password. Forgot Your User ID or Password? Click on Forgot User ID or Password? and follow the prompts to confirm your identity. You can choose to receive an access code by text, or phone. You can also request to receive a temporary password by or postal mail. For personal assistance, call the Dominion Benefit Center at Qualifying Life Events You cannot make a change to your elections during the year unless you have a qualifying life event such as marriage, divorce, birth or adoption. You have 31* days from the date of the life event to notify the Dominion Benefit Center about the change; otherwise, you ll have to wait until the next Open Enrollment to make the change. Go to dombenefits.com (click on the Choose Benefits tab, then Midyear Changes) for information about what qualifies as a life event and the changes you can make and when. If you make changes to your 2016 benefits due to a qualifying event after you have made your 2017 Open Enrollment elections, your life event election won t automatically carry over to You must contact the Dominion Benefit Center immediately to update your 2017 elections. * The enrollment period to add dependent children is 60 days in the event of the birth, adoption or placement for adoption of your dependent children. The enrollment period to enroll yourself and dependents is also 60 days in the event of eligibility for premium assistance under the plan through a state children s health insurance program (CHIP); or the termination of Medicaid or CHIP coverage due to loss of eligibility. The 31-day period remains in effect for all other qualifying life events. Remember: Health Savings Account (HSA) and Flexible Spending Account (FSA) elections won t roll over to next year. If you want these benefits in 2017, you must actively enroll for them! 5

8 How Your Medical Benefits Work Now that you ve reviewed your medical plan options, take a look at how your medical benefits work. STEP 1: Start with the Deductible The annual deductible is the amount you pay for care before you and Dominion begin sharing the cost of medical care. The higher the deductible, the lower the premium. Keep in mind that with Options B or C, prescription drugs are also subject to a separate prescription deductible. Quick Tip Most in-network preventive care services are covered at 100% and are not subject to the deductible or copayments. To find out if a particular service will be covered, call Anthem at prior to getting care. STEP 2: Next Is the Copayment The copayment is the percent Dominion pays versus what you pay, after you satisfy the deductible. For example, if the copayment is 80% Dominion and 20% participant, for every $1 of eligible medical expense, Dominion pays 80 (80%) and you pay 20 (20%). Consider Your Premium When you think about your total annual medical benefits costs, don t forget to include the amount of the premium that comes out of your paycheck. Add them all up (the deductible, copayments and premiums) to find what really fits your budget. STEP 3: Option A Use the HSA When you enroll in Option A, you can open an HSA and use it to help pay expenses that go toward the annual deductible. You can also use it for other out-of-pocket medical expenses. Dominion contributes to your HSA (under Option A only see page 7 for details) and you may make tax-free contributions too, if you wish. Also, unused HSA money is yours to keep once deposited no matter where your future takes you. Option B or C Use a Healthcare FSA If you choose Option B or C, you may use a Healthcare FSA to pay for eligible out-of-pocket expenses that are not paid by insurance. Dominion does not contribute to the Healthcare FSA; it is funded entirely with your own tax-free contributions. You must use the money in your FSA by March 15, You will lose any money remaining in your FSA after that date. New to Option A for 2017? If you plan to enroll in Option A in 2017 and you have an FSA in 2016, be sure to use up your FSA so you have a $0 balance by December 31. That way you ll receive the Company s contribution to your HSA by January 31 and you can start contributing in January too. Learn more by visiting dombenefits.com. STEP 4: Out-of-Pocket Maximum: The Safety Net The annual out-of-pocket maximum protects you against high or unexpected medical costs by limiting the amount you pay of your own money for eligible medical care during the year. Once you reach the out-of-pocket maximum, Dominion pays 100% of your eligible medical expenses for the rest of the year. 8

9 Flexible Spending Accounts (FSAs) Save money on everyday health care and dependent care expenses by contributing to a Flexible Spending Account. (Remember, medical Option A participants cannot enroll in a Healthcare FSA because they have the HSA.) Healthcare FSA Estimator and Day Care FSA Estimator this tool adds up eligible out-of-pocket expenses so you can estimate how much money to contribute to your FSA. Learn more by visiting dombenefits.com FSA Deadlines March 15, 2017 last day to incur an eligible expense that can be reimbursed from your 2016 account(s). April 30, 2017 last day to file 2016 FSA claims for reimbursement. Have a Healthcare FSA in 2016 and Enrolling in Option A for 2017? File claims as soon as possible. Your Healthcare FSA balance must be $0 on December 31, 2016 to receive the Dominion HSA contribution in January. Allow plenty of time for your claims to be processed before December 31. If you have a balance in your Healthcare FSA on December 31, 2016, you may open an HSA, but due to IRS rules your full Dominion HSA contribution and your own contributions (if any) will be delayed until April Even though your HSA contributions will be delayed, you will still receive the same total amount of HSA contributions for And you ll be able to use your remaining Healthcare FSA balance for any eligible expenses incurred through March 15,

10 Dental Benefits Your Dominion dental plan lets you decide where you want to receive care. When you see a MetLife network dentist, you pay less. The good news is the MetLife network has tens of thousands of participating dental locations, including general dentists and specialists. But, what if your dentist isn t in the network? No problem. You ll still receive the standard benefits as shown below. 12 Income Protection Benefits Money doesn t buy happiness, but if you don t have money, you can t buy anything! So protect your and your family s financial health with these Dominion benefits. Long-Term Disability Long-term disability (LTD) coverage provides an income if you are out of work for 180 consecutive days due to an approved disability. You automatically receive LTD coverage equal to 50% of your annual base pay at no cost to you. You may purchase additional LTD coverage. Visit YBR for coverage options and costs. If you increase your coverage during Open Enrollment, you ll need to complete an Evidence of Good Health form that will be mailed to your home by Unum, the disability administrator. Your increased LTD coverage will not take effect until Unum receives your form and approves the increased coverage. Note that a change in your LTD coverage made during Open Enrollment will not apply to a disability period that has already begun. Life Insurance Employee Life Insurance Dominion automatically provides you with basic life insurance coverage equal to your annual base pay (rounded to the next $1,000). You can buy additional life insurance coverage for yourself on an after-tax basis. Visit YBR for coverage options and costs. Spouse/Domestic Partner Life Insurance You may choose from eight coverage options for your spouse or domestic partner. Visit YBR for coverage options and costs. The amount of life insurance you elect for your spouse or domestic partner cannot be more than the life insurance amount you elect for yourself. A Statement of Health is needed for spouse or domestic partner life coverage of $100,000 or more. Statement of Health If you buy life insurance for yourself and/or your spouse or domestic partner that requires a Statement of Health, the coverage will not take effect until MetLife approves the coverage. Visit dombenefits.com for more information. Child Life Insurance You may elect dependent life insurance for your children under age 26. Coverage options and costs are on YBR. The coverage amount you select covers each of your children for that amount regardless of the number of children you have. It is your responsibility to contact the Dominion Benefit Center (DBC) within 31 days of your youngest child turning 26 to cancel dependent child life insurance. Tools You Can Use Use the long-term disability tool on YBR ( when you select this benefit: Estimate Your Long-Term Disability Needs allows you to determine how much money you and your family would need if you were to become disabled and unable to work. You can find the life insurance tool on YBR ( when you select this benefit: Estimate Your Life Insurance Needs allows you to see how much money your beneficiaries would need if you were to die. Free Will Preparation If you enroll or are enrolled in additional Employee Life Insurance, you and/or your spouse can have a will prepared or updated at no cost. MetLife is offering this service through its company, Hyatt Legal Plans. For more information, call Hyatt Legal Plans at When calling, provide the Dominion group number (6967) and the last four digits of your Social Security number. Update Your Beneficiary Selecting a life insurance beneficiary ensures your benefits are distributed as you intended. Review and update your designations yearly, or after a life event, such as marriage, divorce, birth of a child, etc. Accelerated Benefits Feature The feature provides you up to 80% of your life insurance benefit if you are diagnosed with a terminal illness and 12 months or less to live. Your request is subject to medical review and approval by MetLife. Payment is generally paid in a lump sum, up to a maximum of $500,000. You choose how you use your benefit payment; for example, to pay medical bills, nursing home expenses or living expenses. Upon your death, any remaining employee life insurance benefit is paid to your beneficiary(ies). 12

13 Eligibility You re eligible for the benefits in this guide if you re a regular, full-time or part-time non-union employee who is scheduled to work at least 1,000 hours during the year. Temporary or leased employees (sometimes known as contract employees ) are not eligible. Family Members You can enroll these members of your family in your medical, dental and vision benefits: Your spouse, defined as the person to whom you are legally married and for whom you have a valid government-issued marriage certificate. Your children under age 26 even if they are married or have access to other health coverage. Your children are your natural children, legally adopted children, children placed with you for legal adoption, stepchildren and foster children. Your children age 26 or older who are not able to support themselves because of a physical or mental disability. For more information, visit dombenefits.com. You may be required to provide proof of the disability from time to time. A person of the same or opposite gender who qualifies as your domestic partner. For more information about domestic partner coverage, visit dombenefits.com. Your legal ward under age 26 for whom you are appointed legal guardian, provided that he or she qualifies as your dependent for tax purposes. Your dependents serving in the military who are covered by military medical are not eligible unless they are your children under age 26. When you add a new dependent, you ll need to provide proof of his or her eligibility such as a marriage certificate, birth certificate or adoption certificate. You ll be notified of the documentation needed and the deadline for providing it via U.S. mail to your home address. When Your Spouse or Domestic Partner Is a Dominion Employee If you and your spouse or domestic partner are both employed by Dominion, you cannot be covered as both an employee and a dependent for medical, dental and vision benefits. Also, your children cannot be covered under the medical, dental and vision plans by both employees. Both parents can have child life insurance coverage for the same child(ren). When enrolling, you have two options: One employee can sign up for coverage with the other as a dependent; or Both employees can sign up for coverage separately (with only one employee enrolling eligible children as dependents). 13

14 The Fine Print This guide constitutes a summary of material modifications. This material, along with the most recent edition of the summary plan description, is only a summary of your benefit options and does not create a contract between the Company and any employee. Details of the Dominion benefit plans and programs can be found in the plan documents or HR policies that govern all aspects of the plan or program. In the event of a discrepancy between the information contained in this guide and the applicable HR policies, agreements or plan documents, the relevant HR policies, agreements or plan documents shall be controlling. The Company reserves the right to amend or terminate the plans or programs at any time for any reason. Reconstructive Surgery After a Mastectomy As required by the Women s Health and Cancer Rights Act (WHCRA) of 1998, the Dominion medical plan provides coverage for: All stages of reconstruction of the breast on which the mastectomy has been performed; Surgery and reconstruction of the other breast to produce a symmetrical appearance; Prostheses; and Physical complications of mastectomy, including lymphedemas, in a manner determined in consultation with the attending physician and the patient. Such coverage may be subject to annual deductibles and copayment provisions as may be deemed appropriate and are consistent with those established for other benefits under the plan or coverage. Written notice of the availability of such coverage shall be delivered to the participant upon enrollment and annually thereafter. Newborns and Mothers Health Protection Act (Newborns Act) Group health plans and health insurance issuers generally may not, under federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a vaginal delivery, or less than 96 hours following a cesarean section. However, federal law generally does not prohibit the mother s or newborn s attending provider, after consulting with the mother, from discharging the mother or her newborn earlier than 48 hours (or 96 hours as applicable). In any case, plans and issuers may not, under federal law, require that a provider obtain authorization from the plan or the issuer for prescribing a length of stay not in excess of 48 hours (or 96 hours). 14 Health Insurance Portability and Accountability Act (HIPAA) Dominion maintains the privacy of your health information in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). As part of our compliance with HIPAA, this is a reminder that you have access to Dominion s notice describing our legal duties and privacy practices related to how medical information about you may be used and disclosed, as well as how you can access this information.
