1 Requirements to meet Healthy for Life premium rate continue In 2017, employees and their spouses (if enrolled in an LG&E and KU medical plan) must once again complete the following wellness requirements by Aug. 31, 2017, to qualify for the HFL medical premium. 1. Obtain and submit your biometric screening results from a 2017 company health fair, your primary care physician, or through another convenient option to ADURO, the independent wellness vendor of the HFL portal; and 2. Complete the Well-Being Assessment on the HFL portal; and 3. Complete the Tobacco-Free or Take Action requirement on the portal by indicating you re tobacco-free or, if you certify as a tobacco user, by completing either the Quit for Life tobacco-cessation program or a series of Breathe Easy webinars by Aug. 31, Please note the premiums will be based on 2018 rates; therefore, the differential New in 2017 Thank You points! More points for being well Individuals who have biometrics that fall into healthy ranges (as determined by accredited, national health recommendations) will automatically earn extra points. Individuals can earn 100 points each for healthy blood pressure, blood sugar and total cholesterol levels.

2 Preventive Care Getting regular checkups and exams can help you stay well and catch problems early. It may even save your life. Preventive care helps protect you from getting sick, whereas diagnostic care is used to find the cause of existing illnesses. For example, if your doctor suggests you have a colonoscopy because of your age when you have no symptoms that s preventive care. However, if you re having symptoms and your doctor suggests a colonoscopy to see what s causing them that s diagnostic care. For more details about preventive care, visit Anthem s website at Enter your gender and age to see what preventive services are available to you. Also, keep in mind the following recommendations from the American Cancer Society. (More details are available on Women s Cancer Screenings Breast cancer testing: At any age, report any changes in the way your breasts look or feel to a health care provider right away. Consult with your doctor about your risk related to breast cancer. If you re not at high risk, then testing is not needed from age If you are, talk to a health care provider about when you need to start getting mammograms or other screening tests. Women ages should have the choice to start annual breast cancer screening with mammograms if they wish to do so. The pros and cons of screening should be considered when making this decision. Starting at age 45, women should get mammograms every year. Starting at age 55, you should switch to getting mammograms every two years, or you can choose to continue getting one every year. Cervical cancer testing: The U.S. Preventive Services Task Force is currently updating its recommendation on cervical cancer screenings. Currently, they recommend women age have a Pap smear every three years. Please speak with your doctor about their recommendation. Colon cancer testing: Colon cancer is highly treatable when detected early. Find out if you are at higher-than-average risk for colon cancer because of family history, genetic disorders or other factors. If you are at increased risk, talk to a health care provider about when you need to start testing and what tests are right for you. If you re not at high risk, then testing is normally started at age 50. There are several testing options. Talk with your health care provider about which tests are best for you and how often testing should be done. At age 65, Medicare covers testing. Lung cancer testing: If you re age 55 or older, talk to a health care provider about your tobacco history and whether you should get yearly low-dose CT scans to screen for early lung cancer. Men s Cancer Screenings Colon cancer testing: Colon cancer is highly treatable when detected early. Find out if you are at higher-than-average risk for colon cancer because of family history, genetic disorders or other factors. If you are at increased risk, talk to a health care provider about when you need to start testing and what tests are right for you. If you re not at high risk, then testing is normally started at age 50. There are several testing options. Talk with your health care provider about which tests are best for you and how often testing should be done. At age 65, Medicare covers testing. Prostate cancer testing: Starting at age 45, men at higher-thanaverage risk of prostate cancer should talk with a doctor about the uncertainties, risks and potential benefits of testing so they can decide if they want to be tested. This includes African-American men and men with close family members (father, brother, son) who had prostate cancer before age 65. Men with more than one close relative who had prostate cancer before age 65 are at even higher risk and should talk with a doctor about testing starting at age 40. Beginning at age 50, all men at average risk should talk with a health care provider so they can decide if they want to be tested. Lung cancer testing: If you re age 55 or older, talk to a health care provider about your tobacco history and whether you should get yearly low-dose CT scans to screen for early lung cancer.

3 Required steps to qualify for Healthy for Life medical premium in 2017 To qualify for the Healthy for Life (HFL) premium rate in 2018, employees and their spouses (if on the company s medical plan) must complete these steps by Aug. 31, Individuals who choose not to complete the steps below will pay a higher premium in 2018 through the base rate. All employees hired in 2017 should refer to their Healthy for Life new-hire wellness folder for their specific requirements and deadlines. Step 1: Obtain and submit your biometric screening results. Employees and spouses have various options to submit biometric screenings to the HFL portal (see Table 1 below). Step 2: Complete the Well-Being Assessment on the HFL portal. Employees and spouses should log in to their HFL portal (lke.mywellmetrics.com) to complete their online Well-Being Assessment. (See page 5 for more details about the portal.) To qualify for the HFL rate, both employees and spouses (if on the company s medical plan) must complete the assessment. Step 3: Complete the Tobacco-Free or Take Action section on the portal by indicating you re tobacco-free or, if you certify as a tobacco user, by completing a tobacco-cessation program. While on the HFL portal, users must indicate their tobacco status in the Tobacco-Free or Take Action tile. If you and/or your spouse (on the company medical plan) indicate you are not tobacco users, then no further action is required on your part. However, if you and/or your spouse indicate tobacco use, the tobacco user(s) must complete the Quit for Life program (four mandatory phone calls) or a series of Breathe Easy webinars/surveys by Aug. 31, See page 4 for more details. Tobacco users should begin the process early to ensure they have enough time to complete the program before Aug. 31. Table 1. The results will be loaded automatically onto the HFL portal two weeks after the health fair. Health fair dates are available at Co-pay may apply: Complete a physical with your primary care physician and submit your annual screening results by: 1. Logging into the HFL portal. 2. Clicking Complete Your Health Screening tile on your home page. 3. Downloading and printing the Health Provider Screening Form. 4. Taking the form to your annual screening visit with your primary care physician. 5. Asking the physician s office to complete the form. 6. Submitting the completed form online by following the instructions on the bottom of the form. You will receive an confirming proof of submission. Employees may confirm the biometric screening results were submitted by visiting the portal seven to 10 business days after the physician submits the form. Co-pay may apply: Women who visit their OB-GYN as part of their annual yearly physical (not for exams during pregnancy) can print out a Health Provider Screening Form on the portal (see instructions above), take it to their OB-GYN and have the form completed. Free: Visit a LabCorp facility and submit your screening results by: 1. Logging into the HFL portal. 2. Clicking Complete Your Health Screening tile on your home page. 3. Downloading and printing the LabCorp form. 4. Taking the form to a LabCorp facility. LabCorp will perform all of the tests on the form and submit the screening results. Individuals may confirm receipt of the information by visiting the portal seven to 10 business days after the submission. Free: Self-testing and mail-in option allows spouses and employees on long-term disability to request a home test kit from the portal by: 1. Logging into the HFL portal. 2. Clicking Complete Your Health Screening tile on your home page. 3. Selecting Click to Order a Home Test Kit and following on-screen instructions. The kit enables individuals to provide a small blood sample for the lab and includes a self-addressed, postage-paid mailer to return the sample. The kit will be received three to five business days after submitting the request. The kit will include specific instructions on how to perform the test, and a nurse line is available for those who need additional help. Users will receive an letting them know when the results are available for viewing on the portal (typically within days after the kit is mailed back to the lab). Page 3

4 Healthy for Life premium rate FAQs Q1. Do the three steps have to be completed in the order listed on page 3? A. No. These activities can occur simultaneously or in any order. Want to complete your Well-Being Assessment before your health fair is scheduled? No problem. Want to get started on the Quit for Life program before completing your Well-Being Assessment? No problem in fact, we encourage it! Note that you can earn an extra 500 points on the portal if you complete all HFL premium requirements by June 30. Healthy for Life online portal login page. Page 4 Q2. My spouse/i recently had biometric screenings completed by my primary care physician. Can I submit these results? A. Employees and/or spouses who had their biometric screenings completed by their primary care physician on Jan. 1, 2017, or later, may submit these results by Aug. 31, Employees/spouses will need to have their doctor complete the Health Provider Screening Form. The form (including instructions) is available on the HFL portal (lke.mywellmetrics.com). Q3. How does the tobacco portion of the HFL requirements work? A. On the HFL portal, users will see a Tobacco-Free or Take Action tile. Employees and spouses (if on the company s medical plan) will be required to certify whether they are tobacco users. A tobacco user is defined as someone who has used any tobacco or smoking-related product (cigarette, cigar, pipe, chewing tobacco, snuf, snus, hookah or e-cigarette) within the last three months, regardless of frequency or location. For participants who are not tobacco users, points are awarded within hours after certifying they are not a tobacco user. If a participant is a tobacco user, the Tobacco-Free or Take Action tile remains and points are not awarded until he/she completes either the Quit for Life program or a series of Breathe Easy webinars. Remember, failure to report tobacco use is a violation under LG&E and KU s Fraud Policy. Q4. How do I complete the Quit for Life program? A. To complete the program, tobacco users must participate in four phone calls. Call 1: Employees/spouses work with the tobaccocessation vendor to create a quitting plan based on various factors (forms of tobacco use, usage amount, etc.). Once the quitting plan is created and the participant is registered, he/she is transferred to a cessation coach to complete Call 1. Call 2 occurs on the participant s scheduled quit date. Call 3 occurs approximately one week later. Call 4 occurs two to three weeks after Call 3. During calls 2, 3 and 4, the cessation coach discusses issues such as: Challenges the participant is experiencing. Any triggers and possible alternatives to help replace those triggers. How to replace a negative view (on why he/she should quit) with a positive one. Tools to help create a new routine to replace habitual behavior. For example, if a participant would smoke a cigarette immediately after eating, the coach will provide suggestions on how to alter that habit. Nicotine replacement therapy, including patches, gum or prescription medications. Tobacco users must make the initial call to the Quit for Life program ( ) and are encouraged to begin immediately to complete the program by Aug. 31. Representatives from Quit for Life are available from 8 a.m. to 9 p.m., seven days a week. After the initial call, a cessation coach will reach out to the employee/spouse. A coach will make three attempts per call to interact with a participant. After the third attempt, a letter will be mailed to the participant s home to encourage them to contact Quit for Life for a coaching call. Q5. How do I complete the Breathe Easy webinars? A. The Breathe Easy webinars will be available on the HFL portal. To complete the program, participants must complete a total of six webinars and complete the survey after each webinar. Once all six webinars and surveys are completed (along with one exit survey), the system will upload points and verify challenge completion. Each webinar ranges from six minutes to 11 minutes in length. For more details, visit the portal.

5 Q6. What if I completed Quit for Life in 2016 for the 2017 HFL medical premium? Do I have to enroll again? A. If you and/or your spouse successfully completed the Quit for Life program in 2016 and have remained tobacco-free, you do not need to complete the program again in However, if you and/or your spouse have used tobacco within the last three months, even after completing Quit for Life in 2016, the tobacco user(s) must complete a tobacco-cessation program by Aug. 31, You are strongly encouraged to begin immediately to complete the program in time. Q7. My spouse and I are both on the company s medical plan. If I complete the three requirements and my spouse doesn t (or vice-versa), will I qualify for the HFL premium rate? A. No. If both you and your spouse are enrolled in the company s medical plan, both of you must complete each of the steps to qualify for the HFL rate. Q8. My spouse has secondary coverage through LG&E and KU. Does he/she still have to meet the three requirements despite having only secondary coverage through the company? A. Yes, all spouses who are on the company s medical plan whether for primary or secondary coverage are required to complete the steps (listed on page 3). Q9. How will HFL apply to employees who have a change in status? A. For employees who are impacted by a change in status (for example, if a spouse loses a job or medical coverage through his/her employer and wants to be added to the company s medical plan), their spouse has 60 days to complete the Well-Being Assessment and the Tobacco-Free or Take Action tile on the portal. The 60 days conforms to other required change-in-status documentation (marriage certificate, birth certificate, etc.). If you have any questions about new hires or change in status, please contact the Benefits Department at benefits@lge-ku.com. Q10. How does this impact retirees? A. Retirees are on a separate medical plan and are not eligible for the HFL rate. Q11. Does the HFL rate require action from any dependent children? A. No, to qualify for the rate, only the employee and spouse (if covered by LG&E and KU s medical plan) need to take action. Additional dependents such as children are not affected. Healthy for Life online portal FAQs Q12. How do employees access the HFL portal? A. On Feb. 1, employees and spouses who already have an account on the HFL portal received an from Healthy for Life (via support@adurolife.com). New employees who have not yet created a HFL account will receive an with a link to the website along with a personalized activation code. Do not share this code with anyone. It is your individual access code for your personal information. After signing in for the first time, employees should change their password. New spouses must create a separate account as well. Page 5

6 Q13. I forgot my password. How can I recover it? A. On the login page, click the Trouble signing in? link below the Sign in button. You will be asked to enter your to have a temporary password sent to you. Other trouble-shooting options appear below the field for further help with this issue. A. Employees who are not on the company s medical plan are encouraged to create an online account on the portal, complete the Well-Being Assessment and earn wellness points to receive incentives. Q17. Do my results from my biometric screenings and the online Well-Being Assessment remain private? A. LG&E and KU will not receive any clinical health information regarding specific participants. Our third-party wellness vendor ADURO is prohibited from disclosing any personal health information to anyone, including LG&E and KU. Q14. Do my spouse and I have to create two separate accounts on the portal? A. Yes. In order for your information to be specific to your health experience (including results from your Well-Being Assessment and personalized recommendations), each user must have a separate login account. In addition, to qualify for the HFL rate in 2018, both the employee and spouse (if on the company s medical plan) must complete the steps outlined on page 3 by Aug. 31, To create an account, spouses should visit the website lke.mywellmetrics.com and click the Get Started button. They will need to enter an address (a separate from their spouse), their date of birth and their employed spouse s employee ID number (E + six digits) with an s at the end. For example, if the employee s ID number is E012345, the spouse will need to enter E012345s. Q15. My spouse is not on the company s medical plan. May he/she take advantage of the HFL portal and the company health fairs? A. All spouses regardless of whether they are on the company s medical plan may attend a health fair and earn incentives (see Table 2 on page 8). All spouses also may enroll on the HFL portal. The portal vendor ADURO will receive spouse information from the company via information obtained in PeopleSoft. If your spouse s information is not in PeopleSoft, you can update it by ing benefits@ lge-ku.com. Q16. I m an employee, but I m not on the company s medical plan. Can I still create an online account and access the HFL portal? Page 6 Q18. I certified I m a tobacco user in the Tobacco-Free or Take Action tile, but the tile still remains on my homepage. Why hasn t it disappeared? A. If you certified you are a tobacco user, this tile will not disappear until you complete the Quit for Life program or the series of Breathe Easy webinars. Please note it may take several weeks for your completion to be uploaded to the portal, at which time your tile will disappear. You also can confirm that you ve met all of the HFL premium requirements by clicking My Points and then Points History on your portal. Q19. Do I have to wait until my HFL Premium Level status is 100 percent complete before starting the Silver- and Gold-level challenges to earn extra points? A. No. You may start completing challenges on the portal to earn extra points even while you re working on achieving Healthy for Life Premium Level status. Please note, however, that you cannot officially qualify for Silver Level until the Healthy for Life Premium Level is completed. For example, you may talk to a wellness coach to earn points, but you cannot earn the Gold Level incentive until all items from the Healthy for Life Premium Level and Silver Level are completed. Q20. How do I earn extra points on the portal? A. You can earn extra points in a variety of ways. Via challenges Various challenges related to exercise, healthy eating, financial wellness and others are available throughout the year. These challenges have a variety of point levels. Via coaching The portal offers one-on-one coaching or a series of webinars featuring topics such as nutrition and stress. Employees who complete any of these programs will earn 1,000 points! If your points have not been awarded within two weeks of completing your final survey, support@adurolife.com.

7 Healthy for Life online portal s tiles for wellness activities. Via meeting with Coach Donna Take part in personalized coaching with on-site ADURO Registered Dietitian. Earn 300 points per session, up to 1,500 points total. Via Your Anthem Nurse engagement For employees or spouses with complex health care needs, Your Anthem Nurse is available to help you navigate the health care system. If you receive a call from Your Anthem Nurse and participate in the program, you can earn 1,000 points that are automatically added to the portal. There s also a Future Moms program for pregnant women. As soon as you know you re expecting, you can initiate the process by calling one of Anthem s experienced registered nurses at Not only will you receive calls from a registered nurse with expertise in prenatal/postnatal care, educational materials and more, but you also will receive 1,000 automatic points for completion of the Future Moms program. Q21. How can I see the points I ve earned? A. In your portal, click the My Points tab to see points earned and challenges completed. Q22. I m having technical issues with the HFL portal. Whom should I contact? A. Resolve technical issues by calling ADURO support at from 9 a.m. to 9 p.m. (Eastern time) or by ing support@adurolife.com. Q23. Whom should I contact if I have questions about the HFL medical premium or the portal? A. If you have questions related to the medical premium, non-technical questions about the portal, or any additional questions, wellness@lge-ku.com. Incentives FAQs Q24. What incentives are available? A. Employees and spouses regardless of medical plan participation will receive incentives for completing various wellness actions, some of which are already required to qualify for the HFL premium. Employees and spouses can attain three status levels on the site, based on activities completed (see table below). While Healthy for Life Premium Level requirements must be complete by Aug. 31, 2017, employees/spouses have until Dec. 31, 2017, to meet the other levels for additional incentives. Page 7

8 Q25. I m an employee on the company s medical plan. What happens if I miss the Aug. 31 deadline for completing all three HFL premium requirements? A. You would not qualify for the less-expensive HFL premium rate because you missed the deadline of Aug. 31, However, if you complete those actions by Dec. 31, 2017, you would still be able to earn the additional wellness incentives being offered for Silver and Gold level participants. Q26. How/when will I receive the incentives? A. Information about the Amazon e-gift card incentives will be sent to the address listed on the portal account after the employee/spouse completes Silver Level status or Gold Level status. Note that gift cards for both employees and spouses will be taxed on the employee s paycheck approximately six weeks after the gift card(s) is received or claimed. You will receive a separate Did you know you can access the HFL portal while on the go? The portal can be accessed from any internet connection, so you and your spouse can keep track of your activities at home or on any mobile device.
