Now, it s time to take one of the most important steps of all: enrolling in your 2015 Stryker benefits. Inside, you ll find six easy steps for enrolling in your benefits. Annual Enrollment is Here Annual Enrollment for your 2015 Stryker benefits begins November 3 at 8 a.m. EST and closes on November 14, 2014 at 7 p.m. EST. This is your once-a-year opportunity, outside of a qualifying life event, to enroll in Stryker benefits. what s inside: Enroll in Your Benefits in Six Easy Steps Not Making Changes for 2015? 2 Enroll in your Benefits, in Six Easy Steps Here s what you ll need to do to prepare for and complete Annual Enrollment: 1. Prepare to enroll in your 2015 benefits, including recertifying your tobacco status. 2. Get familiar with what s changing for 2015, and your options. 3. Check your personal information in My Stryker Info, including dependents, and make any changes. 4. Enroll in your benefits for 2015 and complete the Tobacco Use Affidavit. 5. Print a confirmation of your elections Annual Enrollment Guide 6. Provide Dependent Status Certification and Evidence of Insurability (EOI) if required. Let s get started!
3 Not Making Changes for 2015? Even if you plan to elect the same benefits for 2015 as you currently have, there are two items that you must review during Annual Enrollment: Tobacco Use Affidavit If you or your spouse/declared domestic partner will have Stryker medical coverage, you must complete the 2015 Tobacco Use Affidavit (at to affirm that you and (if applicable) your spouse or domestic partner: (a) Do not use tobacco, or (b) Have completed the Quit For Life program or another physician-recommended tobacco cessation program. You must re-certify your (and your spouse/declared domestic partner s) tobacco status each year during Annual Enrollment, even if you completed the Affidavit last year. If you don t, you will pay a $600 Tobacco Use Surcharge. Questions about Quit For Life or the Tobacco Use Affidavit? Visit the Quit For Life tile in the Health and Wellbeing section of the Stryker Total Rewards Site ( Click or call Quit For Life at or Contact your Benefits Team. Flexible Spending Accounts (FSA) If you would like an FSA in 2015, you must elect a contribution amount during Annual Enrollment. Any FSA election you made for 2014 (Healthcare or Day Care FSA) will not carry over for Questions about Flexible Spending Accounts? Visit the Flexible Spending Accounts tile in the Financial section of the Stryker Total Rewards Site. Healthcare FSA Used for eligible out-of-pocket medical, dental, vision and prescription drug expenses. You can contribute from $100 to $2,500 for Day Care (child and adult) FSA Used for eligible child care, day care and elder care expenses. You can contribute from $100 to $5,000 for May not be used for dependent medical expenses. Review your September Pre-Enrollment newsletter. Contact your Benefits Team Annual Enrollment Guide
4 Step 1: Prepare to Enroll First, prepare to actively enroll in your 2015 benefits. Here s how: Make sure you can log into My Stryker Info, either directly at or through the Benefits Enrollment tile on the Stryker Total Rewards Site ( Health and Wellbeing section). You must be on the Stryker network or VPN in order to access My Stryker Info. If you have trouble, contact your Benefits Team for help. Review the Tobacco Use Affidavit and decide on the tobacco status that applies to you and your spouse/declared domestic partner. You ll certify it in Step 4. Step 2: Get Familiar With What s Changing The changes planned for your Stryker benefits were explained in detail in the most recent Total Rewards newsletter (the pre-enrollment edition, sent to your home in September). The newsletter is also available on the Stryker Total Rewards Site, on the Benefits Enrollment tile and via the Communication Archives link at the bottom of the page. Here is a summary of what s changing for Gather the Social Security Numbers and Certifying Documents for proof of dependent status for anyone you plan to newly add under your Stryker benefits. You'll need to enter the Social Security Number during the enrollment process Annual Enrollment Guide
5 WHAT S CHANGING FOR 2015 Your Prescription Drug Expenses Count Your prescription drug copays will count towards satisfying your annual in-network medical plan out-of-pocket maximum in all Stryker medical plans. Portability for the Life Insurance Plan If you leave Stryker for any reason other than reaching retirement age or beginning active military service, you can take your coverage with you at preferred rates without submitting evidence of insurability (EOI). Certain coverage limitations apply; ask your Benefits Team for more information. Higher Out-of-Pocket Maximums* in the UHC PPO Plans In 2015, the annual out-of-pocket maximums* in the UHC medical plans will change as shown. Remember, your medical copays, coinsurance, medical deductible and prescription copays will all count toward the applicable maximum in Out-of-Pocket Maximums*: UHC PPO Plans UHC Choice PPO & Out-of-Area Employee $2,950 Employee + 1 $5,900 Family $6,250 UHC Value PPO Employee $4,250 Employee + 1 $8,500 Family $9,250 Increase in Employee Contributions Both you and Stryker share in the cost of your healthcare. Stryker pays most of the cost and you pay a portion through employee contributions, deductible, copayments and coinsurance. Your employee contribution amounts for the 2015 plan year can be found in the September pre-enrollment newsletter. If you do not have a copy nearby, you can view and download a version on the Stryker Total Rewards Site. Go to and click on the Benefits Enrollment tile. Expanded Domestic Partner Coverage In 2015, Stryker will extend coverage under several benefit plans to declared opposite sex domestic partners and their children. Declared... Medical (UHC, Kaiser CA, Kaiser HI) Dental Delta Dental Vision EyeMed VIsion Live Well with Stryker and Quit For Life participation Spouse and Child Life Insurance The Hartford Same-Sex Domestic Partners, and their Children Opposite-Sex Domestic Partners, and their Children Already eligible Already eligible Already eligible Already eligible NEW: Eligible, effective 1/1/15 NEW: Eligible, effective 1/1/15 NEW: Eligible, effective 1/1/15 NEW: Eligible, effective 1/1/15 NEW: Eligible effective 1/1/15, under the applicable plan (Spouse Life or Child Life) You will need to complete the enrollment via My Stryker Info, and complete and return the Domestic Partner Affidavit by November 21, In addition, declared same-sex and opposite-sex domestic partners will now be eligible for The Hartford spouse and child life insurance benefits. Coverage for Your Adult Dependent Child(ren) Beginning January 1, 2015, Stryker will cover your adult dependent child(ren) through the end of the month in which they turn 26 years of age. In previous years, coverage was discontinued one day prior to their 26 th birthday under some of the Stryker plans. *The out-of-pocket maximums shown are in-network only. Separate out-of-pocket maximums apply for out-of-network care. Also, remember that your out-of-pocket maximums differ from the above if you are in a Kaiser or BCBS of Alabama plan. 6 Step 3: Check Your Personal Information in My Stryker Info Visit and click View and Update Personal Details. Correct any errors you find and verify: Your address and phone number: Periodically Stryker may try to contact you with important benefit information. Incorrect personal information can result in unnecessary delays. Since various life events can affect the people you would choose to list as primary and secondary beneficiaries in your life insurance and 401(k) plans, you should review these once a year and make changes if necessary. Annual Enrollment is a perfect opportunity to check your listed beneficiaries. To update your life insurance beneficiaries, go to My Stryker Info. To update your 401(k) account beneficiaries, go to the Vanguard website at For questions, contact your Benefits Team. Your dependent Social Security numbers: The Affordable Care Act (ACA) requires Stryker to report all individuals covered in its medical plans in Stryker will protect all confidential information and will only use it for required purposes. See the screenshots and step-by-step guidance below to check your personal information on My Stryker Info. 7 Next, check your personal information and confirm Stryker has the correct data on file for you. Click View and Update Personal Details to view and update your name, address, phone number, and marital status Annual Enrollment Guide
8 Step 4: Enroll in Your Benefits Now, it s time to enroll in your 2015 benefits. Remember you must do so by 7 p.m. EST on November 14. a. On the My Stryker Info home page, click on Benefits Enrollment to choose your benefits for b. Read and accept the Legal Disclaimer. If you decline after reading the Legal Disclaimer, you won t be able to enroll in Stryker benefits for Annual Enrollment Guide
9 c. Confirm and, if applicable, add your dependents for healthcare coverage by doing the following: Click on Add Another Person (circled in red, above). Enter the basic information for the person you d like to enroll in coverage (their name, date of birth and their relationship to you). For Annual Enrollment, the qualifying event date should be entered as the current date. Enter a Social Security number for each dependent if you are adding them to your healthcare coverage. Click the Apply button. Once all of your dependents or beneficiaries are listed correctly, click "Next" to continue the enrollment process. d. Review your benefit information. Click on the Benefits Enrollment tab at the top of the page to review your 2015 elections. To update your 2015 elections, click on the "Update Benefits" link Annual Enrollment Guide
10 Step 4: continued... e. Choose your 2015 benefits and complete the Tobacco Use Affidavit. Remember, your Tobacco Use Affidavit for 2014 will not carry over to Select the appropriate benefit elections. f. This is where you will choose which dependents you would like to cover under your medical, dental and vision plans Annual Enrollment Guide Check the box if you would like to cover this person. Uncheck the box if you would like to remove that person from coverage.
11 g. Designate beneficiaries for life insurance. Enter the percentage of your benefit you would like each beneficiary to receive in the Primary % field. Enter a percentage amount in the Secondary % field, if applicable. Step 5: Print a Confirmation of Your Elections Review your confirmation statement to ensure all of the information is correct and print a copy for your records. Once you get to this screen, your changes have been saved. Pay close attention to any warning messages and Benefit Selection messages at the top of the page. 12 Step 6: Provide Dependent Status Certification and EOI (if applicable) Your final step is to provide any additional information requested by Stryker or The Hartford. Proof of Dependent Status If you add a new dependent to your Stryker benefits coverage during Annual Enrollment, you must provide proof of their dependent status by November 21, 2014, to your Benefits Team. If proof of dependent status is not received by that deadline, your new dependent will not be added to your Stryker coverage. Birth certificates or marriage certificates and current tax returns are examples of acceptable proof of dependent status. Contact your Benefits Team if you have questions. Did you know? Evidence of Insurability (EOI) Depending on your Annual Enrollment elections for supplemental or Spouse life insurance, you may be required to provide evidence of insurability (EOI). If EOI is required, The Hartford will notify you directly and send you an application by mail in January. You must return the completed application and any required EOI by February 28, The Hartford will make the final decision about your coverage, and will inform you when your application is approved until then, you will be covered at the maximum level available to you without providing EOI.
