3 Benefit Open Enrollment #2 Open Enrollment #2 for Part-Time Faculty is from Monday, October 15th through Thursday, November 15 th. Dental and Vision plans go into effect on January 1, You may make changes via the ebenefits online enrollment system or via paper enrollment forms. Paper forms are due to HR/Benefits no later than 12:00 pm PST on Thursday, November 15 th. 3

6 Dental Plan Options Delta Dental DHMO o Plan effective January 1, 2019: HMO dental plan though Delta Dental. The DHMO dental plan covers IN-NETWORK benefits only. Out-of-network benefits are not covered. The plan offers enhanced orthodontia coverage. You will know your cost beforehand since the plan costs are based on a scheduled copay amount. Works like an HMO plan where you have to elect a primary care dentist. Three tiers available: Employee Only, Two-Party, and Family PT Faculty pay 100% of dental premium via post-tax payroll deduction 6

7 EyeMed Vision Plan EyeMed Vision Plan o Plan effective January 1, 2019: PCCD is excited to offer a new vision plan though EyeMed. The new coverage will provide a frame of (in-network there is a $150 allowance + 20% off balance) Policy covers routine eye exams and other procedures, and provides specified dollar amounts or discounts for the purchase of eyeglasses and contact lenses. EyeMed will not cover vision services through Costco. To find a provider, visit go to Find a Provider and choose Insight Network Four tiers available: Employee Only, Employee + Spouse, Employee + Child(ren), and Family PT Faculty pay 100% of vision premium via post-tax payroll deduction 7

8 Part-time Faculty Dental & Vision Deduction Schedule Dental and Vision benefits are effective January 1, 2019 through March 31, Due to the short enrollment period, one post-tax deduction will be taken during the month of February to cover January, February and March. During the spring and fall semester, post-tax payroll deductions will be taken over a three month period. 10 Who is Eligible for Benefits? If you re a Part-Time Faculty member, you and your family are eligible to enroll in dental and vision. Your eligible dependents for coverage include: Your legal spouse and registered domestic partner Natural children, adopted children and step-children to age 26 Fully disabled children defined as (a) incapable of self-sustaining employment by reason of mental or physical handicap and (b) chiefly dependent upon the eligible employee for economic support and maintenance (c) included on the employee tax filing Legal guardianship with court filed documents (must be shown as a dependent on most recent federal income tax return) to age 18 When adding a spouse/domestic partner or a child to PCCD plans you will be required to provide a proof within 30 days from your request or your dependent will be removed from coverage immediately and retro cancelled. 10

11 Part-Time Faculty Medical Frequently Asked Questions These are some frequently asked questions and answers regarding the coverage: The District will pay 75% of the employee-only (single member) annual premium for HMO coverage through Kaiser Permanente for qualifying part-time faculty members. NOTE: This is not participation in the Kaiser Plan utilized by permanent Palomar College Employees. Spouses, family members and dependents will not be covered. Methods have been established to prioritize participation should enrollment exceed funding. COBRA coverage will be available to qualifying part-time faculty at their expense should their teaching schedule fall below the 45% load. Who is eligible? o Part-time faculty members who have completed a minimum of a 45% load for at least three of four consecutive regular semesters or an average of 45% load for four consecutive regular semesters. o Part-time faculty members wo are covered under Medicare, Medi-Cal, or any coverage provided by a spouse, domestic partner, current or previous employment are NOT eligible to participate in this plan. What is the coverage period? o Six months per eligible semester, contingent upon maintaining workload eligibility. 11

12 Part-Time Faculty Medical Frequently Asked Questions What is the premium and when are deductions taken? o The employee portion of the premium is currently $ per pay period for three pay periods for each eligible semester. o Fall Semester Coverage is effective between October 1 and March 31. o Springer Semester - Coverage is effective between April 1 and September 30. When Coverage Ends o If you will not be teaching in the spring semester, your coverage will end March 31. If you will not be teaching in the fall semester, coverage ends September 30. Continuation coverage (COBRA) will be offered on a self-pay basis for up to 18 months. SISC will send a COBRA notice and full information to you when your coverage ends. 12
