2 Naming the Applicatin Frms It is very imprtant that the electrnic cpy yu send is named crrectly. Carefully fllw these instructins. When the frms are ready fr submissin, and befre yu print r the final cpy: 1. Under the file menu, select save as. 2. Save yur dcument using the principal investigatr s last name fllwed by -NYS-48. The file name shuld be similar t this: Smith-NYS-48.dc 3. Clse and then re-pen the dcument. Yu shuld nw see the file name in the header at the tp f each page. (Page numbers shuld display in the fter.) NB: If the file name des nt appear in the header, there is n cause fr alarm: in mst cases, this prblem rectifies itself when the ed dcument is pened by the amfar grants administratr; if this des nt happen, we will make sure that all pages are prperly identified. 3 Sectin 1: Applicatin and Institutin Infrmatin Title f prject Titles may nt exceed 75 characters including spaces. D nt use title case r all capital letters. Applicant Institutin: Please enter the cmplete legal name and principal lcatin (street address, city, state, zip cde) f the nnprfit institutin r rganizatin t which a grant wuld be made. Emplyer Identificatin Number (EIN) and nnprfit status Under this RFP, grants will be awarded nly t U.S. tax-exempt charities and nnprfit institutins. The federal EIN must be prvided by all applicants. Organizatins that have been determined by the IRS t be a 501(c)(3) shuld check the crrespnding bx. Sectin 3: Institutin Officer This sectin must be cmpleted by all applicants and prvide the requested infrmatin fr the institutin fficer r ther individual wh, being duly authrized by the institutin, has apprved submissin f the applicatin. Sectin 4: Payment Infrmatin Please make sure the first bx in this sectin crrectly identifies t whm payments shuld be made (i.e., the pay t the rder f Page 3 f 10

Underneath (NOT IN) the gray shaded area, prvide a peer-language abstract f 200 wrds r less (12-pint Times New Rman) describing the ratinale, lng-term bjectives, specific aims, and methdlgy f yur prject, and its relevance t the HIV/AIDS epidemic in New Yrk State. The abstract is meant t serve as a succinct and accurate descriptin f the prpsed wrk when separated frm the applicatin. It is nt cnsidered by the Cnsrtium r amfar t be cnfidential infrmatin and may be used in press releases r ther dcuments made publicly available. Submissin f an applicatin is deemed acceptance f this prvisin. Please underline ten (10) key wrds, r phrases. Relevance Statement In 200 wrds r less (12-pint Times New Rman), discuss the direct relevance f the prpsal t the HIV/AIDS epidemic in New Yrk State. Leverage Statement Underneath the gray bx, describe ptential funding surces and mechanisms that will be pursuedjusing the data gathered thrugh the prpsed research. 5 Sectin I: Persnnel Grants: Prvide infrmatin fr all institutin persnnel t be supprted with grant funds. Include all key persnnel emplyed by the applicant institutin, whether persnnel csts are t be supprted with grant funds r nt. (Infrmatin abut key persnnel nt emplyed by the applicant institutin shuld be prvided in sectin 2.) Starting with the principal investigatr, prvide the name, psitin title and percent effrt t be allcated t the prject. The percent effrt shuld reflect the prtin f the whle year that the emplyee is expected t wrk n the prject. Fr example, an emplyee wrking half time fr six mnths wuld be shwn as 25%. Fr each persn, enter the amunts fr salary and fringe benefits t be supprted by the grant in the applicable clumns. Enter the sum f salary and fringe benefits fr each persn in the far right-hand clumn. Enter the subttals fr salary and fringe benefits n the subttals line, and the ttal requested persnnel csts in the bx t the right f persnnel ttal. The budget fr persnnel shuld include any cst-f-living r merit increases that, by applicant institutin plicy, are expected t ccur during the perfrmance perid. Requests fr salary csts fr individuals already emplyed full-time by the applicant rganizatin will be carefully scrutinized fr apprpriateness and need. Salary and fringe benefits paid in whle r in part with funds frm this grant must nt vary frm thse paid t ther emplyees f the institutin, regardless f the surce f funds. General Instructins fr Budget Sectins 2, 4, 5 and 6 In these sectins yu will enter the subttal fr each categry in the bxes n the right-hand side f the table. Detail fr each sectin is t be entered in the bxes in the center clumn, as shwn in the fllwing example. Sectin 2: Cnsultants / Subcntractrs Cnsultants and subcntractrs are nt emplyees f the grantee institutin, nr are they emplyees r cntractrs f the Cnsrtium r amfar. Key persnnel wh are emplyed by ther institutins, including thse wh are cntributing t the prject at n cst, shuld be included in this sectin. Cnsultatin with institutin faculty, staff r ther emplyees shuld be reimbursed by charging an apprpriate prtin f that individual s salary and fringe t the grant as a persnnel item (see abve). Cnsultants are expert r highly specialized and/r technical persnnel whse fr-hire services are urgently, temprarily, and infrequently needed. Cntractrs are hired t prvide specialized scientific r technical persnnel, r services that are identified as essential t the cmpletin f the prject. In the detail sectin (center clumn), list the name and subttal fr each cnsultant/cntractr. Enter the ttal fr cnsultants and subcntracts in the right hand clumn. In the justificatin, prvide the name, functin, number f days f cnsultatin r units r service t be prvided, and the applicable rate. A letter f cmmitment frm each cnsultant r cntractr, stating the scpe f wrk and estimated csts must be included in the addenda. Sectin 3: Supplies and Materials This categry is t be used fr supplies and materials necessary t perfrm the prpsed research. Sectin 4: Equipment Equipment purchase, rental, and maintenance csts are allwable nly fr special-purpse equipment required t perfrm the specific prject and nt therwise available at the applicant institutin. Purchase, rental and maintenance f ffice equipment, including persnal cmputers, laptp cmputers, furniture and similar general-purpse items are generally nt allwed fr funding. Equipment must be itemized in the center clumn and cmplete details prvided in the budget justificatin. Page 5 f 10

6 Sectin 5: Administrative Csts Up t 10% f the ttal amunt requested may be applied t specific administrative csts. The justificatin must detail the specific csts t be supprted. Include such items as institutin fees fr use f special equipment and ccupancy csts. Requests t supprt ccupancy csts (i.e., rent) shuld be included nly if implementatin f the prject requires renting facilities nt currently available t the institutin, r if the institutin charges user fees fr specialized facilities n a prject-by-prject basis regardless f surce f supprt. The supprt f administrative csts is subject t apprval by the Cnsrtium. Sectin 6: Other Direct Csts Use this sectin fr csts that d nt fall in the categries described abve. Ttal Direct Csts Enter the ttal direct csts as shwn in the example t the right. In n case are ttal direct csts t be greater than $33,000. Under this RFP, an indirect cst rate is nt allwed. Budget Restrictins Funds are nt awarded fr the fllwing: Majr cnstructin r the remdeling f facilities (minr alteratins are allwed with adequate justificatin) The direct cst f supprt services and facilities generally available at a spnsring institutin (ccupancy csts, utilities, maintenance, telephne, ffice furniture and supplies, etc.) except when directly allcable and essential t carrying ut the prpsed research Indirect csts The purchase, lease r rental f ffice equipment Funding fr dissertatin research Travel csts (ther than lcal travel necessary t cnduct the research, e.g., t cnduct interviews) Grants awarded under this RFP are nt meant t cver the ttal cst f a prpsed research prject. A grantee institutin is expected t prvide the necessary physical facilities and administrative services as well as ther supprting services nrmally available at a spnsring institutin. Budget Justificatin A cmplete budget justificatin, n the frm prvided, is required. Applicatins received withut the required budget justificatin will nt be accepted fr review. Resurces and Envirnment Briefly describe the facilities t be used at the applicant rganizatin, their nature, pertinent capabilities, and relative prximity and extent f availability t the prject. Use Other t describe facilities at ther perfrmance and field study sites available t the prject. If a significant prtin f the prpsed research activity will be cnducted at a lcatin ther than the applicant institutin, prvide a separate resurces and envirnment frm fr each site. Please cntact the amfar grants administratin ffice fr assistance with making duplicates f this frm (grants@amfar.rg). Belw the table, using cntinuatin pages if necessary, describe applicable cnsrtium agreements with ther rganizatins. Shuld a cnsrtium agreement exist, a cpy shuld be included in the addenda, alng with agreements frm cllabratrs, cnsultants and cntractrs. Bigraphical Sketches and Other Supprt Frm(s) Bigraphical sketch and ther supprt frms must be cmpleted fr the principal investigatr and fr each f the key prfessinal persnnel listed n page three f the applicatin. Detailed instructins are at the tp f the frm. Enter requested infrmatin in the space beneath the crrespnding gray shaded sectin headers. Cnsrtium funds cannt be used t duplicate supprt prvided by anther rganizatin. Hwever, supprt frm the Cnsrtium can be used t supplement supprt frm ther surces (i.e., supprt fr related prjects whse aims and/r methdlgies are clearly distinct and severable frm the prpsed research). Accrdingly, a prject s principal investigatr and the applicant institutin must diligently insure that all infrmatin presented in an applicatin, including ther supprt, is accurate and inclusive. Failure t d s may be cnstrued as an attempt t mislead the Cnsrtium and/r amfar and will lead t the rejectin f an applicatin, r terminatin f a grant and the required return f all funds received. Requests fr supprt f this nature are carefully scrutinized fr apprpriateness. RESEARCH PLAN FORMATTING REQUIREMENTS AND PAGE LIMITS Applicatins cntaining research plans that d nt cnfrm t these requirements, which are strictly enfrced, will nt be accepted fr review. Research plans must be written in English N mre than 1,200 wrds in length Duble-spaced 12-pint Times New Rman; d nt cndense character r line spacing 1 inch margins r 2.55 cm (tp, bttm, left, right) Page Size: U.S. letter (8½ x 11 inches). This frmatting is required fr the ed versin. Figures and/r tables are limited t ne page, which shuld immediately fllw the literature cited; figures shuld nt be embedded in the research plan itself. Figure legends are t be in 12 pint Times New Rman. The research plan is t be prepared and submitted in a separate wrd prcessing dcument. Open a new dcument, set paper size fr U.S. letter (8.5 x 11 ), set all margins at ne inch, select duble-spacing and 12-pint Times New Rman fnt. Type Research Plan r the applicatin title at the tp f the first page, starting the plan prper belw that line. Use a header t identify the dcument by file name in the upper right hand crner. Page numbers shuld be nt be cnsecutive with the frms (i.e., start the plan with page 1 and include the wrd plan in the fter), and shuld be placed in a fter. Figures and/r Tables Figures and/r tables are limited t ne page; legends are t be in 12 pint Times New Rman. B. Institutinal Apprvals All funded research investigatins are cnducted under the versight and directin f the grantee institutin. Accrdingly, the institutin bears full respnsibility t insure that research facilities, activities (e.g., experiments and prtcls), and emplyees cmply with applicable federal, state, and lcal laws and regulatins. An investigatin invlving either human r animal subjects r derivatives f such subjects, r bihazards may nt initiate research activity r make expenditures until its entire prtcl has been reviewed and apprved in accrdance with applicable regulatins and guidelines including thse established by the U.S. Department f Health and Human Services (DHHS), the U.S. Department f Agriculture (USDA) and the Natinal Institutes f Health (NIH). Human Subjects If human subjects are t be used, dcumentatin f apprval by the institutin s Institutinal Review Bard (IRB) and a cpy f the IRB-apprved patient s infrmed cnsent frm must be submitted. Plicies and Cnditins Descriptins Research grants satisfy varius financial bligatins incurred in the curse f an HIV/AIDS-related investigatin. In general, a research grant is applied t direct csts f salaries fr prfessinal and technical persnnel, labratry supplies and equipment, and ther csts directly related t the research. T Whm Grants Are Made Grants are awarded t nnprfit institutins in the state f New Yrk; they are nt awarded t individual investigatrs. If requested, the applicant institutin must submit prf f its nnprfit status. Applicatins are neither requested nr accepted frm fr-prfit entities. By accepting a Cnsrtium grant, the recipient institutin will affirm the cmpleteness and accuracy f the applicatin submitted fr funding. The institutin will als accept full respnsibility fr the cnduct f the investigatin and fr the acts f the investigatr(s). Bth are under the directin f the institutin and are subject t its medical and scientific plicies. Similarly, prject persnnel cmpensated in full r in part with funds awarded by the Cnsrtium/amfAR are emplyees f the recipient institutin; they are nt Cnsrtium r amfar emplyees. Applicants need nt be U.S. citizens, and there are n restrictins as t age, clr, creed, gender, medical cnditin, handicap, natinal rigin, parental status, plitical affiliatin, race, religin, marital status, r sexual rientatin. Recipient institutins agree nt t prmte r engage in vilence, terrrism, r the destructin f any state, and t take prudent measures t insure that they d nt prvide supprt thrugh sub-grants r ther financing t any entity that engages in thse activities. Surce f Funds Grants disbursed by amfar are apprved by the New Yrk HIV/AIDS Research Centers Cnsrtium. This initiative has been made Page 9 f 10

10 available by the New Yrk State legislature fr the purpse f prmting HIV/AIDS research relevant t the epidemic in New Yrk State. Funding is subject t availability and awarded withut assurance f cntinued r subsequent funding. Review and Apprval Prcess The Cnsrtium intends t encurage and supprt HIV/AIDS research f the highest quality. Therefre, every prperly prepared and submitted applicatin received in respnse t this slicitatin will be peer-reviewed. Each is subject t an verall cnfrmance review by amfar staff. Thse fund t be incnsistent with the guidelines and instructins are eliminated at that time, and the investigatr and the applicant institutin s grants fficial are ntified. Peer reviewers, a vlunteer bdy f scientists wh are experts in varius fields f HIV/AIDS research, evaluate (1) the scientific merit f applicatins; (2) the relevance f the research t the HIV/AIDS epidemic in New Yrk State; (3) the qualificatins, experience, and prductivity f the investigatr/spnsr; (4) the facilities available; and (5) the likelihd f success. The peer reviewers determinatins are cnsidered by the steering cmmittee and research wrking grup chairs f the New Yrk HIV Research Centers Cnsrtium, which hlds the sle authrity t apprve prject funding. Cnfidentiality Thrughut the review and award prcess, amfar respects the privacy f the applicant and endeavrs t prtect frm disclsure any cnfidential r prprietary infrmatin cntained in a submitted prpsal. Hwever, amfar has in place n mechanisms t maintain r guarantee cnfidentiality and, as a nt-fr-prfit crpratin, lacks the financial resurces t (1) institute such mechanisms r (2) accept liability fr the disclsure f infrmatin. At the same time, amfar des nt cnsider infrmatin n an applicatin s abstract t be cnfidential. That infrmatin may be made public as a descriptin f the prject being funded by the Cnsrtium. Submissin f an applicatin is deemed acceptance f these prvisins. Human and Animal Subjects / Bihazards Applicants are required t submit dcumentatin f institutinal apprvals fr research invlving human r animal subjects r the use r prductin f bihazards. Research activity may nt begin, nr may expenditures be made, until such apprvals are received and frwarded t amfar. The Awarding Agency (amfar) reserves the right, upn the advice r directin f the Cnsrtium r the State f New Yrk, t: 1. Reject any and all applicatins received in respnse t this RFP. 2. Award mre than ne cntract resulting frm this RFP. 3. Waive and/r mdify minr irregularities in applicatins received after prir ntificatin t the applicant. 4. Adjust r crrect cst figures with the cncurrence f the applicant if errrs exist and can be dcumented t the satisfactin f amfar. 5.
