As required by the ACCME, all activities designated by ACAAI fr AMA PRA Categry 1 Credit(s) must cmply with the ACCME Accreditatin Criteria, Plicies, and Standards fr Cmmercial Supprt SM including activities planned and implemented thrugh Jint Prvidership. Any activity t be cnsidered fr Jint Prvidership must be aligned with bth the missin f ACAAI, and the defining characteristics f cntinuing medical educatin (CME) as specified by the ACCME. All CME Applicatins fr Jint Prvidership will be assessed based n their individual merits. The ACAAI reserves the right t accept r deny any Jint Prvidership applicatin at its wn discretin. The Jint Prvider must agree t fllw all plicies, prcedures, and frmats as set frth by ACAAI. Prvide Jint Prvidership applicable frms, checklists, and templates t be used fr the activity design prcess and fr ACCME dcumentatin Review and apprve Jint Prvidership CME Applicatin and prvide step-by-step guidance as needed Apply fr Cmmercial Supprt in the frm f Educatinal Grants n behalf f the Jint Prvider Review, apprve, and sign Letters f Agreement (LOAs) fr Cmmercial Supprt (mnetary r in-kind) Review and apprve all advance marketing materials Review CME Cmpiled Cnflict f Interest (COI) frm and prvide guidance n acceptable mechanisms t reslve any cnflicts f interest Review and apprve all nsite prgram materials and/r educatinal cntent 1 f 5 P age

Submit the CME Applicatin fr Jint Prvidership a minimum f 4 mnths prir t the activity. Cnvene the Planning Cmmittee t: Back t Tp Identify prfessinal practice gap(s) based n internal r external data surces Determine the needs f learners (e.g., knwledge, cmpetence, perfrmance) that underlie the prfessinal practice gap(s) Analyze practice gap(s), learners needs, and determine desired result(s) f educatinal interventin (e.g., t change cmpetence, perfrmance, patient utcmes) t frmulate educatinal bjectives Identify target audience Design educatinal cntent and frmat(s) cnsidering desirable physician attributes and adult learning principles Determine if there are any barriers fr learners t accept and integrate the desired changes Determine if any nn-educatinal strategies can be used t reinfrce the educatinal bjectives fllwing the activity Determine if the activity will benefit frm cllabratin and cperatin with ther ACAAI initiatives r external stakehlders Select expert faculty Cllect disclsures, dcument the identificatin and reslutin f any cnflicts f interests (COIs), and submit the CME Cmpiled COI Frm Determine funding, fllwing the requirements f the ACCME Standards fr Cmmercial Supprt SM Develp advance marketing materials; yu must btain ACAAI apprval n all prmtinal materials prir t printing and disseminatin Develp activity agenda and ther nsite prgram materials; yu must btain ACAAI apprval f all prgram materials and acknwledgements Submit agenda fr designatin f credit Participate in initial cnsultatin meeting with ACAAI Secure venue/facility fr activity, as apprpriate 2 f 5 P age

3 Create preliminary budget Faculty management: Cnfirm faculty Request Faculty/Planner Relevant Financial Disclsure Frms Reslve COIs accrding t ACAAI and ACCME plicies Make all disclsures (e.g., financial relatinships, faculty relatinships with Cmmercial Supprter and with prducts r services being discussed, etc.) t learners prir t the start f the activity Acknwledge cmmercial supprt t the audience prir t the beginning f the activity (if applicable) Cnduct educatin activity Cllect activity attendance infrmatin Prvide attendance file t the ACAAI Draft a summary f the evaluatin data, based n the evaluatin reprt prvided by the ACAAI and using the ACAAI Final Reprt frm Cmplete and submit t the ACAAI within 45 days fllwing the activity: evaluatin summary, final budget, and any ther infrmatin required t cmplete and clse ut the activity file.
