There are n targets t meet fr these measures, but yu d have t reprt n them In rder t d that, yu need t run the EP Clinical Quality Measure Reprt frm CRS fr yur reprting perid (see separate instructin dcument) Yu will need this reprt n hand at the time f attestatin because yu ll have t enter in the measures yu are reprting n t the apprpriate website (State fr Medicaid, r CMS fr Medicare Prgram) Yu may print it ut frm RPMS (r sessin lg it int anther file if yu d like) File the reprt in the EP s audit file. Clinical Decisin Supprt Rule: Implement ne clinical decisin supprt rule during EHR reprting perid. This is just a Y/N questin asking if yu implemented this There are any number f items in EHR yu culd use t d this, s yu have many ptins t chse frm t serve as dcumentatin f this measure. The easiest may be t simply g t yur Immunizatin sectin f EHR and take a screen sht that includes the frecasting. Paste int a Wrd dcument, print and place in the EP s audit file OR, if yu have Clinical Reminders installed and natinal reminders cnfigured, pull up a patient, click n the Reminders, take a screen sht, and paste int a Wrd dcument OR, if yu have any f these n the EHR Reprts Tab, generate ne f the reprts, print it, and place in file: Diabetes, Pre-Diabetes, Asthma, Anti- cagulatin, r Wmen's Health Supplement Example f Immunizatin Frecast screensht:

3 Electrnic Exchange f Clinical Infrmatin: Perfrmed at least ne test f certified EHR technlgy's capacity t electrnically exchange key clinical infrmatin. There nly needs t be ne analysis per site. Drug-Frmulary Checks: The EP has enabled this functinality and has access t at least ne internal r external frmulary fr the entire EHR reprting perid. Yu just answer Y/N fr this measure. T dcument this, take a screen sht f the rder dialg fr a Nn-Frmulary medicatin, shwing the NF in the name f the drug.

5 Patient Lists: Generate at least ne reprt listing the EP s patients with a specific cnditin during the EHR reprting perid. Yu can run any number f reprts frm RPMS r icare including reprts frm the Wmen s Health Package, the Diabetes Audit, the Immunizatin package, CRS, and QMAN and VGEN reprts Run a list f patients fr the EP that are due fr immunizatins r that have a diagnsis f asthma r anything like that. Print it and place it in the prvider s file. Cntact Katie if yu are having truble finding a list t print. These next 2 measures are knwn as the Public Health measures. Yu can chse either ne, but yu d have t chse ne f them. The exceptin here is the Oregn Medicaid prgram, they chse t make the Immunizatin ne mandatry. Immunizatin Registries Data Submissin: Perfrmed at least ne test f certified EHR s capacity t submit electrnic data t immunizatin registries and fllw- up submissin if the test is successful. Fr Washingtn and Idah check with yur immunizatin crdinatr. If things are exchanging smthly, they shuld be getting a cnfirmatin . Ask fr a cpy f ne f thse s and place it in the prvider s file. This is met by the RPMS ILI/H1N1 Surveillance Exprt t the IHS Divisin f Epidemilgy and Disease Preventin. If yu are using this, smene at yur site shuld be getting an receipt that the exprt was cmpleted. Get a cpy f ne f these s and place it in the prvider s file.
