The fee is $12.99 per camper fr up t a 30-day supply f their medicatin, and an additinal $7 fr campers staying lnger than 30-days, plus each prescriptin cpayment. Am I required t fill ut a new enrllment every year? Yes. Many f us are aware that a child s health changes frm minute-t-minute. With these cnstant changes taking place it is necessary t prvide a new enrllment every year t ensure that ur recrds with regards t yur child s medicatin regimen are accurate. What happens if I rder late? If yu have nt cmpleted the health infrmatin fr yur camper thrugh rckhillrx.cm and mailed/had faxed/had E-scripted us a cpy f yur camper s prescriptin at least six (6) weeks befre their sessin begins, yu will be charged a rush fee f $ We recmmend sending the prescriptins at least sixty (60) days prir t the sessin beginning, unless the prescriptin is a cntrlled substance frm a New Yrk prescriber (see PRESCRIPTIONS sectin). Please understand that yur campers health and well-being is ur primary cncern and we will d everything we can t accmmdate late rders. What if my camper is started n a prescriptin r nnprescriptin daily medicatin after the deadline date t register and submit prescriptins has passed? At Muntain Meds we realize that a child s health is unpredictable and we will d ur best t accmmdate all campers thrughut the seasn. We will wrk with yu regarding late medicatin changes. Please cntact us as sn as yu are aware f any changes t yur camper s medicatin regimen. If yu are enrlling fr the first time this seasn, and it is past the deadline, a rush fee will apply t the camper s rder. If yu have already enrlled this seasn, and are changing medicatin regimens, a $10 repackaging fee will apply. Can I supply a specific vitamin r supplement t yu fr packaging? Althugh we discurage this practice, we will wrk with yu t prvide yur child exactly what yu want while they are at camp. We can prvide 99% f yur specific requests. Hwever, if yu insist n supplying the prduct t us, there will be an additinal $25 fee charged. This fee will cver any research, and prduct verificatin, needed t package yur prduct in a safe, and effective, manner. We ask that yu send the prduct as sn as pssible and must include a written rder frm yur prescriber fr the prduct included in the shipment.

2 Will nn-prescriptin medicatins cst the same as my hme pharmacy? Our pharmacy is very cmpetitive, and ften times cheaper than chain pharmacies; hwever, we cannt guarantee that the price will be identical. Pricing n pharmacy and nn-pharmacy prducts is based n a reginal price matrix and yu will find ur pricing is very fair. Please see ur sample Overthe-Cunter Medicatin Price List t have an estimate f ur pricing. Keep in mind that this sample Over-the-Cunter Medicatin Price List is nt all-inclusive and nly represents a few f the items we carry. We are a full-line pharmacy and we can prvide 99% f yur Over-the-Cunter requests. What if I decide that my child des nt need t take the items I enrlled them with fr camp? We ask that yu us the cancelatin request as sn as pssible. If the items have already been packaged yu may be charged fr the cst f the item and be given the ptin t pay pstage t have the items mailed t yu fr future use. Enrllment fees are autmatically charged upn enrlling in ur prgram and will nt be refunded. Our prgram is mandatry fr yur camp fr any prescriptin medicatin, Over-the-Cunter items, vitamins, r supplements that yur child takes n a daily, r regular, basis OR that yu want a specific supply fr yur child while they are at camp. PRESCRIPTIONS Hw d I send yu the prescriptins? Please cmplete all f the infrmatin t setup a Muntain Meds accunt n rckhillrx.cm. Yu will then be able t print ut a Summary Sheet t take t yur camper s dctr. We ask that yu arrange with yur dctr t have all prescriptins in t us at least 60 days prir t the start date f camp, unless the prescriptin is a cntrlled substance frm a New Yrk prescriber (see belw). IMPORTANT: Faxed prescriptins MUST cme frm yur healthcare prvider s ffice. The ffice must have the header set in their fax settings t prvide the dctr s name and fax number. They may NOT be faxed by the parent/guardian as restricted per Federal Law. What shuld I d since my camper is at camp fr lnger than a mnth? Please make sure yur camper s physician includes at least ne (1) refill n the prescriptin befre it is sent t ur pharmacy. Mst prescriptins can be written with refills. ALL cntrlled substances require an additinal thirty (30) day prescriptin pst-dated with a date thirty (30) days befre the camper s sessin begins and the secnd prescriptin thirty (30) days later. Cntrlled Substance prescriptins must be written n an riginal prescriptin, signed by the physician in ink, and mailed t ur pharmacy fr a maximum f 30 day supply per prescriptin. We are als set up t receive Electrnic-Prescriptins (E-Scripts) directly frm yur dctr s ffice fr cntrlled substances. Cntrlled Substance prescriptins may NOT be faxed. ***Families with NY Dctrs*** As f March 27, 2016 all prescriptins shuld be sent via Electrnic Prescriptin (E-Script). Fr nn-cntrlled substances, and Over-The-Cunter items, please have yur camper s dctr send prescriptins in as sn as pssible. Fr cntrlled substance prescriptins, please make yur camper s dctr aware that we will require their ffice t send the Electrnic Prescriptin (E-Script) within 30 days f the start f camp. What if my camper requires a new medicatin, r refill f an as needed medicatin, while at camp? Muntain Meds is cmmitted t yur child s well-being. We will always send medicatin t yur child s camp whenever necessary. Yu will be charged the actual shipping and handling cst t ensure it is at the camp as sn as pssible.

3 MEDICATIONS Which medicatins am I required t have Muntain Meds Dispense fr my child s camp? All Oral/Pill-Frmed Medicatins, Over-the-Cunters, Vitamins, and Supplements Exceptin Accutane Prducts All NnPill-Frmed Medicatins such as Inhalers, Creams, Ointments, Drps, Liquids, and Injectables (EpiPens and Insulins) Exceptins Grwth Hrmnes and Allergy Shts Any As Needed Medicatins and Over-The-Cunters that yu wuld like a Specific Supply t be available fr yur camper s usage nly based n their at-hme usage. D yu supply ver-the-cunter (OTC medicatins, vitamins, and supplements)? Yes. We will need an authrizatin frm yur child s physician fr all medicatins, including verthe- cunter medicatins, vitamins, and supplements. This authrizatin shuld include dse, times t be given and any ther detailed instructins fr administratin, as well as, specific manufacturer if needed t be maintained. Authrizatin may be in the frm f a written r faxed prescriptin, r using the Over-the-Cunter Authrizatin Frm. Either methd will require the prescriber t sign ff authrizatin fr these items t be given while at camp. Will yu dispense generic r brand medicatins? Our pharmacy will dispense generic medicatins unless therwise specified by yur physician as Dispense As Written r DAW. Please ntify yur camper s physician accrdingly if yu d nt want generic prescriptins r ver-the-cunter medicatins dispensed and cnfirm with them befre sending the prescriptin t us that the wrds Dispense As Written, DAW, r fr E-Scripts that they indicate a DAW cde f 1 are clearly n the prescriptin. It is yur respnsibility t cnfirm with yur prescriber s ffice that the prescriptin is written crrectly, since fr mst campers, we will nt have the medicatin histry t reference befre dispensing these medicatins. Are yu able t supply grwth hrmne r allergy shts? Thrugh ur wrk with numerus camps, we have fund that in rder t maintain cnsistency and ensure prper balance within the narrw range f treatment, it is best fr yu t send grwth hrmne, insulin r allergy shts up t camp with yur child fr administratin while at camp. Our pharmacy will nt rutinely prvide these medicatins. Hwever, in the event f an emergency, we will d everything in ur pwer t help get the medicatin t yur camper as sn as pssible. D yu supply Birth Cntrl packs while my camper is at camp? T prevent issues with fllwing the cnsecutive rder f yur child s birth cntrl pack, we suggest that yur child brings the current pack that they are utilizing at the time f camp and cmplete that pack while at camp. We will prvide the cntinuatin f thse packs fr the remainder f time that they are at camp t cntinue their therapy where their previus pack has left ff. Can yu supply nn-pill frm medicatins such as inhalers, creams, drps r liquid medicatins? Yes, the medicatins will be included in separate, re-sealable packages with dsing instructins attached. Can yu supply Gummy vitamins t my child while at camp? Yes, Gummy vitamins will be sent t the camp in an apprpriate package fr that item. An apprpriate package may include the item nt being packaged via ur strip packaging system. This decisin will be made n an individual basis per the judgment f the packaging pharmacist t ensure a safe and efficient methd is used. Alternatives t cmmn gummy vitamins are prvided thrugh the link titled Gummy Alternatives and these items can be incrprated in t the strip package system. If an alternative is nt used, we will use ur judgment t prvide the Gummy t the camp as we see mst apprpriate, and the Gummy will be included n the Medicatin Administratin Recrd (M.A.R.).

4 Hw are my camper s medicatin refilled while at camp? If yur camper will be at camp greater than thirty (30) days, we will refill the medicatin and cntinue t prvide the medicatin while at camp. D nt refill yur camper s medicatin while at camp, as this will cause yur insurance t reject ur insurance claim. If this happens, yu will be respnsible fr the full cst f the medicatin if the insurance denies ur claim due t the same medicatin being filled while yur camper is still at camp. What if I use a Mail Order Pharmacy? After registering with Muntain Meds fr the summer sessin, we ask that yu cntact Member Services t Cnfirm: That yur Rx Plan is NOT a mandatry Mail rder fr the medicatins we will be dispensing. Rx Plan des nt have a limit n hw many times yu are allwed t fill utside yur Mail Order Plan If yur Rx Plan allws a Vacatin Override, If Needed If yu use Mail Order r get 90 Days Supply: Please make every effrt t avid filling/refilling the medicatin within 60 days f the start f camp If medicatin is required within that 60 day windw, please fill fr the number f days needed until the start f camp If yur Mail Order Rx Plan des nt give yu the flexibility t use Muntain Meds, please us and we will help find the slutin!!! If yu use an Autmatic Refill Prgram (Mail Order r at yur Lcal Pharmacy) please ask that a temprary hld be put n the Autmatic Filling f these medicatins. INSURANCE Hw is my insurance billed? Yur child s health is ur #1 cncern. Our pharmacy will send yur camper s medicatins t camp prir t their arrival at the camp t ensure that n dses are missed. We will bill yur insurance cmpany directly shrtly befre camp begins. If necessary we will request Vacatin Overrides and wrk with yur insurance cmpany t submit the claims fr payment at the apprpriate times. Hw am I billed fr c-pays and ut-f-pcket expenses? Any ut-f-pcket csts (e.g. cpays, deductibles, ver-the-cunter medicatins, nn-cvered medicatins) will be charged t yur credit card that yu put n file during enrllment. Please nte that credit card charges will appear under ur pharmacy name Rck Hill Health Mart Pharmacy. Please nte that we d accept FSA/HSA cards fr the prtin f yur ut-f-pcket csts that qualify under the guidelines f these prgrams. We d ask that yu put a secnd, regular credit card, n file during the enrllment prcess fr the enrllment/packaging fee that is nt cvered as part f an FSA/HSA prgram. Please ntify us via ur website f any change f payment that is required, as there will be a $10 prcessing fee assessed fr payments rejected due t insufficient funds, canceled accunt numbers, r ther declined payments as a result f nn-pharmacy related rejectins. Will my insurance cmpany pay fr my camper s medicatins if I recently filled their prescriptins? Typically they will. Often times we will need t request a vacatin verride frm yur insurance cmpany fr a camper being away frm hme. We will cntact yu if we have any questins r prblems with yur insurance and discuss the ptins that are available.

5 Are yu able t accept my insurance? What if the pharmacy des nt accept my insurance? We are cntracted with all majr and mst minr prescriptin carriers. Once yu submit yur registratin we will review the infrmatin and ntify yu if ur pharmacy is nt cntracted with yur plan. In mst situatins we are able t cntact the plan and arrange ur cntract befre camp even starts. We are als happy t check and make sure we accept yur insurance in advance. What are PCN and BIN r IIN? PCN stands fr Prcessr Cntrl Number and BIN stands fr Bank Identificatin Number, smetimes referred t as an IIN that stands fr Issuer Identificatin Number. If yu have prescriptin insurance cverage, then yur plan must have a BIN r IIN and PCN. If yu are nt able t lcate these numbers n yur insurance card, yu will need t call either yur lcal pharmacy where yu have received prescriptins befre, r yur insurance cmpany s custmer service number t btain this infrmatin. PACKAGING Hw are the medicatins packaged? Medicatins are packaged in a sealed strip package that include a detailed label with yur camper s name, date f birth, date, time f dsing, medicatin names with descriptin, as well as any special instructins. Camp health staff members simply tear pen the packet and administer yur camper their medicatins. Only medicatins in pill frm are included in the dse strip. All ther medicatins will be packaged and dispensed in apprpriate packaging fr repeat administratin by health staff. Can yu package prescriptin medicatins my child uses n as needed basis? Yes, bth prescriptin and ver-the-cunter medicatins can be packaged as needed. They will be packaged separately and labeled with instructins fr administratin. Hw can I be sure medicatins will be packaged exactly the way my camper takes them? It is yur respnsibility t check that the prescriptin is written clearly and crrectly. The prescriptin shuld be written fr the time f day (i.e. breakfast, lunch, dinner, bedtime) yur camper shuld take the medicatin. If a prescriptin is written as nce a day with n specific time, the medicatin will be packaged as a breakfast dse. If the medicatin is taken as needed please make sure that is specified n the prescriptin. We strngly encurage yu t keep bth ur pharmacy, and the camp, ntified f any changes in medicatin therapy t allw the camp nursing staff t crss-reference the regimen against the package being prvided by ur staff. Can a half pill dse r ther special requirements be packaged? Yes, yur camper s physician must specifically write yur camper s prescriptin t be packaged as a half tablet r ther clear instructins fr preparatin f each dse. Yu must indicate yur request upn registering s we can determine if the specific medicatin is durable and able t be packaged in the manner requested r if alternate arrangements are required by us. What time are medicatins dispensed at camp? Mst camps dispense medicatins at mealtimes and bedtime. If yur camper s medicatin is time specific please make sure yur dctr indicates the specific times taken with the medicatin. Prescriptins written as ne pill per day will be administered at breakfast unless therwise indicated. Medicatins taken multiple times per day will be dsed arund the camp s meal schedule unless therwise indicated. Please indicate n the enrllment frm specific dsing times that yu use fr administratin.
