SCOPE The Beltn Independent Schl District ( BISD r the District ) is seeking t establish an agreement with multiple Businesses, Public r Private Organizatins, r ther entities that wish t prvide Classrm Instructinal Supplies and Equipment t the District. The estimated value f supplies purchased annually is between $100,000 and $150, DISTRICT OVERVIEW 2.1 Beltn ISD is lcated 60 miles nrth f Austin and 40 miles suth f Wac n IH-35. BISD cvers an area f apprximately 200 square miles in Bell Cunty. 2.2 BISD current has ne (1) traditinal high schl, ne (1) new tech high schl, ne (1) ninth grade center, three (3) middle schls, nine (9) elementary schls and ne (1) early childhd schl. 3. DESCRIPTION BISD is accepting Prpsal Respnses fr a multiple award cntract fr Classrm Instructinal Supplies and Equipment in accrdance with the instructins, terms, cnditins, and requirements/specificatins cntained in this Slicitatin. 2. SUBMISSION OF PROPOSALS 2.1 Prpsals are t be sealed and returned in an envelpe marked n the utside with the vendr s name, address and prpsal number. 2.2 Prpser shall submit ne (1) riginal cpy f prpsal. 2.3 Prpsals must be returned t the fllwing address in sufficient time t be received and time stamped n r befre the time and date shwn n this Slicitatin and can be either hand delivered r mailed. The Directr f Purchasing will issue any addenda t all wh are knwn t have received a cpy f this prpsal. PROPOSAL RESPONSE 3.1 The District will be accepting Prpsal Respnses n a cntinual basis frm the issue date f this prpsal until 10:00 A.M. n March 6, February 28, Prpsal respnse must cntain; The Prpsal Respnse Frm in its entirety; Certificatins/Representatins Dcuments; Vendr Catalg(s) r price list; Any additinal dcuments required by this Slicitatin. 3.3 The District reserves the right t reject any Prpsal Respnses that the District cnsiders inapprpriate. The District shall be the sle judge f acceptable Prpsal Respnses. 3.4 Withdrawal f Prpsals. Any Vendr(s) wh is extended the privilege f withdrawing a Prpsal because f having prven mechanical errr in his r her Prpsal may nt be allwed t submit a Prpsal n similar items fr a perid f ne year unless the prhibitin is waived by the Superintendent. 3.5 Prpsals submitted are encuraged t be in type-written r in print frmat. Due t high vlume f respnses, any illegible prpsals may be rejected. 3.6 Faxed r ther electrnic submissin is prhibited unless specifically authrized by the Slicitatin. 4. GENERAL TERMS, CONDITIONS AND REQUIREMENTS FOR SOLICITATIONS This slicitatin shall be gverned by the fllwing dcuments; unless an exceptin is therwise taken within this Slicitatin. TERM OF CONTRACT 5.1 Cntracts created by this Slicitatin shall be in effect frm the date f award r March 1, 2015, whichever is later, thrugh March 31, BISD reserves the right t extend the cntract at the District s sle ptin fr ne (1) additinal year. 5.3 The District reserves the right t re-evaluate the cntract during the cntract perid and cancel the cntract with a thirty (30) day written ntice. 5.4 BISD, may at its sle ptin, extend this bid fr an additinal sixty (6) days frm the date f expiratin, under the same pricing and terms and cnditins if it is determined by the District that additinal time is required t avid a cntract lapse. 6. EVALUATION, NEGOTIATIONS, AND AWARD 6.1 Each Prpsal respnse will be evaluated based n the requirements set frth in Sectin III, Special Instructins. 6.2 Vendrs are encuraged t submit prpsals as sn as pssible. 6.3 THIS IS A NEGOTIATED PROCURMENT, and as such, the District reserves the right t negtiate any terms, cnditins, r pricing with a Vendr prir t an award. 6.4 Awards will be made t Vendr(s) that have received an acceptable evaluatin rating n all criteria. 6.5 All deviatins frm the specificatins must be nted in writing by the Prpser at the time the prpsal is submitted. The absence f a written list f specificatin deviatins will hld the Prpser strictly accuntable t the District s specificatins as written. 7. TYPE OF CONTRACT Firm-Fixed Discunt. Discunts shall remain firm fr the life f the cntract. 8. FELONY CONVICTION CERTIFICATION AND NON-COLLUSION STATEMENT Prpser is required t submit a cmpleted cpy f the Felny Cnvictin Certificatin and Nn-Cllusin Statement. Cpies f the frms are included with these specificatins and must be cmpleted and returned with the prpsal. 9. REFERENCES Three (3) references must be submitted with this prpsal 10. SIGNATURES ON PROPOSALS Prpsals, t be valid, must be manually signed in ink by authrized persn in the space prvided. By such signature, Prpser agrees t strictly abide by the terms, cnditins, and specificatins embdied in this prpsal. 11. COOPERATIVE PURCHASING Beltn Independent Schl District is a participating member f the Central Texas Purchasing Alliance. The successful vendr is t state their willingness t prvide prducts and/r services, based upn the prpsal price, t any participate in the Alliance. Beltn Independent Schl District shall nt be held respnsible fr any rders placed, deliveries made, r payment fr prducts and/r services by these ther entities. 12. CONFLICT OF INTEREST 12.1 Disclsure f Certain Relatinships with Lcal Gvernment Officials Any individual r business entity that cntracts r seeks t cntract fr the sale r purchase f prperty, gds, r services with Beltn ISD must file a Cnflict f Interest Questinnaire with the BISD Purchasing Office in accrdance with Texas Lcal Gvernment Cde Chapter 176, n later that the 7 th business day after the recipient becmes aware f facts that require filing This requirement applies t a persn wh is an agent f a vendr in the vendr s business with the District All prpsers must disclse the name f any Beltn ISD emplyee wh wns, directly r indirectly, an interest in the Prpsers firm r any f its branches Frm is included in this prpsal r can be dwnladed at W-9 INDENTIFICATION NUMBER CERTIFICATE Prpser shall submit with their Prpsal Respnse a cpy f their W-9 Identificatin Number Certificate t expedite the payment prcess if awarded a cntract. The District will nt be respnsible fr any purchases made prir t the issuance f a purchase rder. 16. SECTION III- SPECIAL INSTRUCTIONS The fllwing infrmatin is t prvide the Prpser with the needed infrmatin n hw t cmplete and submit their Prpsal respnse. 1. RULES OF PREPARATION 1.1 Discunts prpsed in the Prpsal shall remain fixed and binding n the Prpser fr the life f the cntract. 1.2 BISD expects Prpsers will cmply with the stated requirements f the prpsal in develping their respnse. 1.3 Any exceptin t the terms and cnditins f this prpsal must be included in writing in the Prpsers Respnse. 2. PRICING 2.1 It is the intent f this slicitatin t establish a discunt frm catalg r published price list. 2.2 Prpser must indicate a primary discunt (even if the discunt is 0% r list price), but may ffer multiple discunts by categry. 2.3 The percentage discunt ffered will be based n the current published catalg r price list and pricing will remain firm until a new catalg is published and delivered. 2.4 Only catalgs widely distributed and nt specific t the District shall be used. 2.5 Prices fr this prpsal cannt be increased fr thirty (30) days after the cntract begins. In rder t change a price list r catalg, a new r amended price list r catalg must be submitted t the Purchasing Department fr apprval. The Purchasing Department will ntify the vendr f either acceptance r rejectin f the new price list/catalg. The last BISD apprved price list r catalg remains in effect until such time that BISD apprves the price change. 2.6 Price reductins shall be ffered immediately upn becming available t a vendr after award. 3. CATALOGS 3.1 Prpser shall submit with prpsal a cpy f each catalg t be used. 3.2 Awarded vendrs shall mail catalgs directly t each campus. The catalg shall be labeled with the Beltn ISD prpsal number, the length f the agreement, discunts, shipping terms and any exclusins. Campus addresses can be fund n ur website at Vendrs are nt allwed n campuses withut the written cnsent f the Directr f Purchasing 4. AWARD 4.1 This prpsal will be awarded n the basis f best value. Sectin (b) f the Texas Educatin Cde requires schl districts t cnsider the fllwing criteria when awarding a prpsal: the purchase price; the reputatin f the vendr and f the vendr s gds and services; the quality f the vendr s gds and services; the extent t which the gds r services meet the district's needs; the vendr s past relatinship with the district; the impact n the ability f the district t cmply with laws and rules relating t histrically underutilized businesses; the ttal lng-term cst t the district t acquire the vendr s gds r services; and any ther relevant factr specifically listed in this slicitatin. N cntact with departments shall take place until receipt f apprval. 5. NO GUARANTEE OF ORDERS 5.1 All expenditures are estimates nly based n previus purchases and future expectatins. BISD is nt required t purchase any minimum r maximum items. 5.2 Shuld the District nt have sufficient funds apprpriated fr the purchase f the items mentined in this prpsal, after timely ntificatin t the supplier, the District is fully and expressly released frm all bligatins under this agreement and any incidental agreements heret. 6. ORDER AND DELIVERY 6.1 Departments will be issuing individual purchase rders fr items purchased. 6.2 BISD will nt be respnsible fr any gds delivered r services perfrmed withut a prperly executed purchase rder signed by the Directr f Purchasing. 6.3 All rders shall be packaged and delivered t the address indicated n the purchase rder during nrmal schl hurs and shall be plainly marked with the prper BISD Purchase Order number. 6.4 All exchanges and returns will be handled by the rdering department. 6.5 The District expressly reserves all rights under law, including but nt limited t the Unifrm Cmmercial Cde, t inspect the Deliverables at the time f delivery, and t reject defective r nn-cnfrming Deliverables.
