It is the declared plicy and intent f GHS t strive t maximize participatin f Diverse Business Enterprises thrugh all business cntracting pprtunities. GHS is cmmitted t ensuring that Diverse Business Enterprises are given every pprtunity t participate in cntracting pprtunities. In adherence t GHS s cmmitment t Supplier Diversity, Slicitrs f a GHS cntract must clearly as defined by GHS herein, demnstrate gd faith effrt t achieve the Supplier Diversity gal set frth. By the dcumentatin f Direct and/r Indirect Tier II gds and/r services t be purchased frm Diverse Business Enterprises certified by ne (1) r mre f the third party certificatin agencies recgnized by GHS. In cnnectin with such mnitring, Cntracted GHS Suppliers will be required t reprt Diverse Supplier Spend t GHS mnthly in a manner in GHS's sle discretin. In additin, a cpy f reprted Diverse Supplier spend, must be attached with the submissin f any invices t GHS. Failure t demnstrate the defined Gd Faith Effrt t achieve GHS s Supplier Diversity gal, bjectives, r t reprt in a manner prescribed by GHS, shall be a material breach f any cntrlling cntract between GHS and Cntractr r vendr. GHS prhibits discriminatin n the basis f race, clr, gender, religin, natinal rigin, r disability in cnnectin with emplyment f any persn, r the award f any cntract. Vendrs interested in ding business with GHS are required t sign the Certificatin belw and cmplete the Supplier Diversity Sectin in its entirety and submit it with their bid respnse. Past Perfrmance: Offerr shall (1) summarize in writing its past perfrmance fr client healthcare institutins in actively fstering the participatin f Diverse Business Enterprises utilized by the institutin, (2) prvide three (3) r mre client references fr this purpse fr whm it has prvided applicable service t within the past tw (2) years, with the name, phne number and f a specific knwledgeable cntact persn fr each such client reference. Present Cmmitment: Offerr shall submit in writing its present cmmitment and business plan t facilitate and prmte the participatin f Diverse Suppliers by cmpletin f the attached Diverse Supplier Subcntracting Plan (DSSP). Diverse Business Enterprises utilized as Tier II cntractrs and suppliers must be certified by ne r mre f the 3 rd Party Certificatin Agencies recgnized by GHS.
