When your apron comes off at the end of the work day, your Home Depot benefit plans, programs and services keep working for you to help you live your best possible life what we like to call the Orange Life. Now it s time for you to start enjoying the benefits of your new Orange Life. What you NEED TO KNOW You MUST enroll on the enrollment website or by calling the Benefits Choice Center before your 91st day of employment. You MUST actively enroll in Medical, Dental, Vision, Critical Illness Protection Plan and/or the Hospital Indemnity Plan or you will not have coverage in these plans for the rest of You MUST designate your beneficiary(ies) for your life insurance coverage during your enrollment session. Important Enrollment Information You ll receive your New Associate Enrollment CD at your home address within a couple of weeks from your hire date. This CD contains a link to your New Associate Enrollment Guide on the Home Depot benefits website. This guide contains complete details on all of your benefit options and an overview of the enrollment process. Even if you don t receive your CD, you must actively enroll on the Your Benefits Resources website by your enrollment deadline or you won t have health care coverage through the Company for the rest of 2012 unless you have a qualified status change during the year. You can visit any time to find your new associate enrollment guide, as well as information on the valuable resources and programs available to you as a Home Depot associate. 1
5 QUICK LIST OF YOUR HEALTH CARE COVERAGE OPTIONS Through The Home Depot To have health care coverage through The Home Depot for the rest of 2012, you must enroll by your 91st day of employment! For complete information on all of the valuable plans, programs and services available exclusively to you as a Home Depot associate, visit Action Needed! You must elect health care coverage by your enrollment deadline to have coverage in Medical Coverage Options Your medical plan options are: Aetna/SRC $10,000 Max option Aetna/SRC $20,000 Max option Under each option, you ll pay less for medical care when you use Aetna Open Choice PPO network providers. Please note that pre-existing condition exclusions may apply and coverage through the Aetna/SRC options is subject to plan limits. You can take the Simple Steps Health Assessment and receive personalized information on how to improve your health. For information about this online assesment, visit select General Information, then click Health Assessment. This plan complies with health care reform provisions. Hospital Indemnity Plan Coverage The Hospital Indemnity Plan provides a limited benefit that can help you pay the cost of an inpatient hospital stay. Please note that pre-existing condition exclusions may apply and coverage through the Hospital Indemnity Plan is subject to plan limits. Critical Illness Protection Plan The Critical Illness Protection Plan can be purchased in four benefit amount options: $5,000 benefit $10,000 benefit $20,000 benefit $30,000 benefit This plan pays a lump-sum benefit for specific conditions, such as heart attack, stroke, cancer, transplant, Alzheimer s disease and paralysis, as well as benefits for eligible travel and lodging expenses and wellness services. 2
6 Dental Coverage Each MetLife dental option provides free preventive care until you have reached your annual maximum benefit: MetLife $500 Annual Maximum option MetLife $1,000 Annual Maximum option MetLife $2,000 Annual Maximum option The $500 annual maximum option covers preventive and basic restorative care only. The $1,000 and $2,000 options cover most dental services, including orthodontia. Vision Coverage Each vision option provides free eye exams from EyeMed Select providers, which include independent optometrists and ophthalmologists and the vision care professionals at Sears, LensCrafters, Pearle Vision, Target Optical and JCPenney Optical. EyeMed Select $120 option EyeMed Select $150 option Short-Term Disability (STD) IMPORTANT: Part-time hourly associates are automatically enrolled into coverage.* If you don t want this coverage, you must decline it before your 91st day of employment or payroll deductions will begin. When you are covered under the short-term disability plan, you ll continue to receive part of your income (50% of your weekly base pay up to $125 per week) if a qualified disability due to illness or injury leaves you unable to work. Benefits begin after you have been unable to work for 14 consecutive calendar days and continue for an approved period of disability for up to 26 weeks (your 14-day elimination period plus your 24 weeks of coverage). Term Life Insurance for You and Your Dependents IMPORTANT: Part-time hourly associates are automatically enrolled into coverage. If you don t want this coverage, you must decline it before your 91st day of employment or payroll deductions will begin. Term Life Insurance provides a $20,000 benefit for you, the associate. If your death is the result of an accident, your beneficiary will receive an additional $20,000 benefit. You also can enroll your spouse/domestic partner and your eligible dependent children in Term Life Insurance. Dependent coverage provides a $2,500 benefit for your spouse/domestic partner and a $2,500 benefit for each eligible child age six months and older. The plan provides a $500 benefit for a child under six months old. * Disability coverage is not available to associates working in California, New Jersey, New York or Rhode Island these states have state disability plans. 3
7 OTHER IMPORTANT BENEFITS FutureBuilder 401(k) Savings Plan FutureBuilder can help you save money for your future through convenient payroll deductions. You can begin making before-tax and/or Roth after-tax contributions to your FutureBuilder account when you complete 90 days of service. After one year of service (at least 1,000 hours in a 12-month period) you can begin receiving a Company matching contribution. It s easy to enroll in FutureBuilder or make changes to your contributions or investments during your enrollment session. Employee Stock Purchase Plan (ESPP) The ESPP allows you to set aside up to 20% of your eligible pay (subject to certain tax limits) to purchase The Home Depot stock. Get Home Depot stock at a discount. The Employee Stock Purchase Plan gives you a unique opportunity to invest in ownership of the Company at a special associate price. Through the plan, you can purchase shares of Home Depot stock at a 15% discount. Buy stock through convenient payroll deductions. When you enroll, you ll indicate the percentage of your pay you want to invest in the ESPP through payroll deductions, up to a maximum of 20% of your eligible earnings or $21,250. On the last day of each plan, The Home Depot stock will be purchased for you. The price of the stock is 15% off the closing stock market price on the last day of the plan. You have two opportunities to enroll in the ESPP each year before each ESPP Plan deadline. You can enroll by visiting MetLaw Legal Services MetLaw provides coverage for legal services, such as wills, estate planning documents and real estate matters at a discounted price. IMPORTANT: To have coverage in 2012 you will need to enroll by your 61st day of employment. 8 Bright Horizons Back-up Care Advantage Program The Home Depot has partnered with Bright Horizons to offer high-quality back-up dependent care when you need to be at work and your regular child or adult/elder care is unavailable. Associates have access to the following back-up care programs: Center-based child care In-home child care (including for mildly ill child) In-home adult/elder care Once you have been with the Company for one year, you will receive up to 10 days of care per year through a nationwide network of quality providers at deeply discounted rates. Visit to learn more. Ayco s Money in Motion Personal Finance Program Associates have access to comprehensive financial education and counseling by phone at a highly discounted rate of $200 per year. For example, counselors can assist with budget planning and strategies for saving for college tuition. To learn more about or enroll in this program, log onto Your Benefits Resources from and choose the Savings & Retirement tab. Commuter Benefits Associates can purchase passes for work-related public transportation and parking expenses (for example, a monthly bus pass) while taking advantage of before-tax payroll deductions. Financial Engines Investment Advice Financial Engines is an independent investment advisor that provides unbiased advice to FutureBuilder 401(k) Plan participants.
