PLAN PAYS 25% OF BENEFIT AMOUNT FOR: Coronary artery bypass surgery Carcinoma in situ Amyotrophic lateral sclerosis (Lou Gherig s disease) Adrenal hypofunction (Addison s disease) Bone marrow donor Cerebral palsy Cystic fibrosis Hemophilia Huntington s chorea Critical Illness Plan Features THE CRITICAL ILLNESS PLAN PAYS A LUMP-SUM BENEFIT FOR SPECIFIC CONDITIONS, such as heart attack, stroke, cancer, transplant, Alzheimer s disease and paralysis and benefits for eligible travel and lodging expenses. See the chart below for a complete list of covered conditions. The plan is administered by Allstate Benefits. THE PLAN ALSO PAYS AN ANNUAL BENEFIT OF $75 FOR WELLNESS SERVICES. In some cases, that $75 could cover the cost of your Critical Illness Protection Plan coverage. CRITICAL ILLNESS PROTECTION PLAN BENEFITS ARE PAYABLE ONLY FOR CONDITIONS DIAGNOSED AFTER YOUR COVERAGE UNDER THE PLAN BEGINS. Meningitis Multiple sclerosis Muscular dystrophy Myasthenia gravis Necrotizing fasciitis Osteomyelitis Scleroderma Sickle cell anemia Systemic lupus Tuberculosis PLAN PAYS UP TO $75 PER CALENDAR YEAR FOR EACH COVERED PERSON FOR ONE OF THE FOLLOWING ELIGIBLE WELLNESS SERVICES: Pre Biopsy test for skin cancer Biopsy for skin cancer Oral cancer screening Blood test for triglycerides Bone marrow testing Colonoscopy Echocardiogram Eletrocardiogram (EKG, including stress EKG) Flexible sigmoidoscopy Hemocult stool analysis Lipid panel (total cholesterol count) Mammography, including breast ultrasound Pap Smear, including Thin- Prep Pap Test PSA (prostate specific antigen blood test for prostate cancer) Serum Protein Electrophoresis (test for myeloma) Stress test on bike or treadmill Annual physical examination (only for covered persons over 18 years of age) Immunizations Transportation Benefit Actual cost, up to $1,500, for round trip coach fare on a common carrier; or $.50 per mile for personal vehicle travel, up to $1,500, to a facility if more than 100 miles from place of residence. Lodging Benefit Reoccurrence Benefit $60 per day up to 60 days if facility is more than 100 miles from residence. Only applies to lodging occurring within 24 hours of, and including days of treatment. A benefit of 100% of the previously paid amount will be paid if a covered person is diagnosed for a second time with a heart attack, stroke, coronary artery bypass surgery, transplant, invasive cancer or carcinoma in situ. The second date of diagnosis must be more than 12 months after the first date of diagnosis for the critical illness, and for the cancer critical illness benefits, the covered person must have had no symptoms nor received any treatment during the 12 months after the prior occurrence. 3

4 DENTAL IF YOU ARE CURRENTLY ENROLLED IN DENTAL AND DO NOT MAKE A CHANGE, YOU WILL CONTINUE YOUR CURRENT 2015 COVERAGE. IF YOU WOULD LIKE TO ENROLL IN DENTAL AND ARE NOT ENROLLED TODAY, YOU MUST ACTIVELY ENROLL BY NOVEMBER 13 TO HAVE COVERAGE IN Dental Plan Coverage DENTAL SERVICE Annual Maximum Benefit Preventive Care TAKECARE NETWORK DENTISTS ONLY $1,500 per covered person per calendar year You pay nothing. Restorative You pay 20% Simple Extractions You pay 20% Endodontics You pay 50% Periodontics You pay 50% Prosthodontics You pay 50% Orthodontics Plan pays a lifetime benefit up to $1,000 per covered person. Coverage is limited to the active phase (Phase II) of the treatment plan and is only available to covered children up to age 17. Your Dental Coverage YOU MUST USE TAKECARE NETWORK DENTISTS OR NO BENEFITS ARE PAID! To find a TakeCare network dentist, go to or call the Customer Service department on Guam at NO DEDUCTIBLE IN THE DENTAL PLAN! The dental plan begins paying benefits immediately there is no deductible before benefits for covered dental services are paid. FREE DENTAL PREVENTIVE CARE! Two dental cleanings and checkups each calendar year are free if you use a TakeCare network dentist Dental Biweekly Payroll Deductions COSTS FOR ALL OTHER BENEFITS WILL BE AVAILABLE DURING YOUR ENROLLMENT SESSION. TakeCare Dental ASSOCIATE ONLY ASSOCIATE+ SPOUSE ASSOCIATE + CHILD(REN) ASSOCIATE + FAMILY $30.29 $60.58 $57.55 $79.96 For weekly payroll deductions, take the biweekly payroll deductions above, multiply by 26, then divide by 52. In some instances your paycheck may not be enough to cover the entire amount of your benefits premiums. In those cases, the amount of the premium above your paycheck is still owed and will be collected from your future paychecks. 4

5 TERM LIFE INSURANCE IF YOU ARE CURRENTLY ENROLLED IN TERM LIFE INSURANCE COVERAGE AND DO NOT MAKE A CHANGE, YOU WILL CONTINUE YOUR CURRENT 2015 COVERAGE. IF YOU ARE NOT CURRENTLY ENROLLED, YOU CAN ENROLL IN TERM LIFE INSURANCE ANY TIME BY CALLING THE BENEFITS CHOICE CENTER AT but you can enroll or drop coverage only once during a 12-month period or if you experience a life event. REVIEW YOUR BENEFICIARY(IES) FOR YOUR LIFE INSURANCE COVERAGE DURING YOUR ENROLLMENT SESSION! Review or change your beneficiary designations online during your enrollment session. When you reach the Enroll in Your Benefits page during your enrollment session, you ll see a link Choose a Beneficiary. YOU ALSO CAN ENROLL YOUR SPOUSE AND YOUR ELIGIBLE CHILDREN IN TERM LIFE INSURANCE. Associate Coverage Spouse or Same-Sex Domestic Partner Coverage Child Coverage $20,000 coverage amount If your death is the result of an accident, your beneficiary will receive an additional $20,000 $2,500 coverage amount $2,500 coverage amount for each eligible child age six months and older $500 coverage amount for a child under six months DISABILITY IF YOU ARE CURRENTLY ENROLLED IN DISABILITY COVERAGE AND DO NOT MAKE A CHANGE, YOU WILL CONTINUE YOUR CURRENT 2015 COVERAGE. IF YOU ARE NOT CURRENTLY ENROLLED, YOU CAN ENROLL IN THE DISABILITY PLAN ANY TIME BY CALLING THE BENEFITS CHOICE CENTER AT but you can enroll or drop coverage only once during a 12-month period or if you experience a life event. Disability benefit 50% of your covered weekly base pay (up to $125 per week) for an approved disability Benefits are reduced by other income you receive (such as Social Security and Workers Compensation) while you are disabled. When benefits are payable After an illness or injury has kept you unable to work for 14 consecutive calendar days How long benefits are payable Benefits continue for an approved period of disability up to 26 weeks (your 14-day elimination period plus your 24 weeks of coverage) 5

6 FUTUREBUILDER Enrolling in FutureBuilder You can enroll in the FutureBuilder 401(k) Plan or make other FutureBuilder changes at any time. It s easy to enroll in FutureBuilder or make changes to your contributions or investments during your enrollment session! Consider Automatic Escalations of Your FutureBuilder Contributions You can choose to have your FutureBuilder contribution percentage automatically increased by 1% each year up to a maximum target rate of 15% through the Quick Enrollment Process, or you can choose your own automatic contribution percentage increase and target maximum rate (up to 50%). You can enroll in automatic escalation during your enrollment session! How FutureBuilder Helps You Save for Retirement MATCHING HOME DEPOT CONTRIBUTIONS once you have completed one year of service (at least 1,000 hours in a 12-month period), the Company contributes $1.50 for every $1 you contribute up to the first 1% of your pay. In addition, you ll receive 50 for every dollar you save from the next 2% to 5% of your pay. A VARIETY OF INVESTMENT APPROACHES you can let the professionals invest your account by choosing a LifePath Portfolio or make your own investment decisions among the plan s core funds or through the selfdirected brokerage window. PROFESSIONAL INVESTMENT ADVICE if you would like professional advice on how much to save through FutureBuilder and how to invest your savings in the plan, consider using Financial Engines Investment Advice. Financial Engines is an independent investment advisor that provides unbiased advice to FutureBuilder 401(k) Plan participants. A LOWER TAX BILL you lower your tax bill today by making before-tax contributions to FutureBuilder. You pay no taxes on your contributions, the company matching contributions or your investment earnings until you take the money out of the plan. 7 EMPLOYEE STOCK PURCHASE PLAN Enrolling in the ESPP You have two opportunities to enroll in the ESPP each year before each ESPP Plan deadline December 17 for the January 1, 2016 plan and June 16 for the July 1, 2016 plan. You can enroll by logging on at livetheorangelife.com. Become a Home Depot Stockholder at a Discount Through the Employee Stock Purchase Program GET HOME DEPOT STOCK AT A DISCOUNT! The Employee Stock Purchase Plan gives you a unique opportunity to invest in ownership of the Company at a special associate price. Through the plan, you can purchase shares of Home Depot stock at a 15% discount. BUY STOCK THROUGH CONVENIENT PAYROLL DEDUCTIONS! When you enroll, you ll indicate the percentage of your pay you want to invest in the ESPP through payroll deduction, up to a maximum of 20% of your eligible earnings or $21,250. On the last day of each plan, Home Depot stock will be purchased for you. The price of the stock is 15% off the closing stock market price on the last day of the plan.
