THE FOLLOWING EXCEPTIONS APPLY: If you are covered under a Home Depot Medical plan, you will need to elect your Healthy Living Credit, complete your Tobacco-Free Certification and confirm your 24/7 Care election. You must actively enroll for Health Care and Dependent Day Care Spending Accounts. Prior year elections will not roll over. WHAT IS CHANGING FOR 2016? Last year I went on a cruise and like everyone I know who was onboard, I came home with a nasty cold. Rather than hoping my doctor could squeeze me in before I went back to work and spending more on an office visit, I went online and requested a call. Within 45 minutes, I spoke to a doctor and was able to pick up a prescription. The whole thing cost me $ call and medication. 10 A CLOSER LOOK AT STEP 2: KNOW YOUR NUMBERS THE HIDDEN HEALTH RISK SCREENING GIVES YOU THE INFORMATION YOU NEED TO LIVE A HEALTHY LIFE. Why does The Home Depot offer Screenings? Your Hidden Health Risks can be identified by participating in the 10 minute Hidden Health Risk Screening. By taking an active role in your health and understanding your health risks, you will be taking an important step to improve your health and potentially reduce health care costs. What is Measured and Why? HIDDEN HEALTH RISK FACTORS 5 RISK FACTORS DESCRIPTION AT-RISK LEVEL HDL Cholesterol Lower than normal HDL ( good ) cholesterol raises your risk of heart disease. Triglycerides Higher than normal triglycerides, the level of fat found in the blood, can lead to hardening of the arteries, which can cause a heart attack. Blood Pressure High blood pressure can lead to heart attack or failure, stroke and kidney failure. Glucose Blood sugar level of 100 or higher Waist Circumference may be an indicator of diabetes. Excess fat in the abdominal area is a greater risk factor for heart disease than excess fat in other parts of the body. Those with 3 or more of the above risk factors are: 2 times as likely to develop cardiovascular disease that can lead to heart attacks and strokes 5 times more likely to develop diabetes lower than or higher 130 over 85 or higher 100 or higher Larger than 40" lower than or higher 130 over 85 or higher 100 or higher Larger than 35" HEMOGLOBIN A1c TEST If your glucose is 100 or higher, the Hemoglobin A1c test automatically will be run. Sign-up sheets will be posted at work locations about 3-4 weeks before the date of the event(s). 14 MEDICAL PAYMENT PLAN THE MEDICAL PAYMENT PLAN PAYS A CASH BENEFIT IF YOU HAVE AN INPATIENT HOSPITAL STAY. A Quick Look at the Medical Payment Plan IF YOU RECEIVE THIS SERVICE: Hospital Admission (1 per year for the initial day one hospital stay) Daily Hospital Benefit (up to 100 days per coverage year) HOSPITAL ONLY YOUR CASH PAYMENT IS: $1,000 $100 A hospital stay means a period during which a covered person is confined in a hospital, treatment facility, hospice facility, skilled nursing facility or rehabilitation facility and charged for room, board and general nursing services. Stay does not include any period of such a confinement due to custodial or personal needs that do not require medical skills or training. Benefits are payable for pregnancy-related stays, visits and services provided on the same basis as for disease. Medical Payment Plan Features CASH BENEFIT GOES DIRECTLY TO YOU. If you are hospitalized after your Medical Payment Plan coverage begins, you will receive a lump-sum cash benefit. You also have the option of having your Medical Payment Plan benefit paid directly to your health care provider it s up to you. Only applies to lodging occurring within 24 hours of, and including days of treatment. A benefit of 100% of the previously paid amount will be paid if a covered person is diagnosed for a second time with a heart attack, stroke, coronary artery bypass surgery, transplant, invasive cancer or carcinoma in situ. The second date of diagnosis must be more than 12 months after the first date of diagnosis for the critical illness, and for the cancer critical illness benefits, the covered person must have had no symptoms nor received any treatment during the 12 months after the prior occurrence. 16
