2 Welcome to your voluntary benefits program. If you have a question or need assistance, you can speak with a YouDecide advisor by calling :00 a.m. to 7:00 p.m. (EST), Monday - Friday, or advisor@youdecide.com. Starting on January 1, 2018, YouDecide will offer the Voluntary Benefits Program; a platform providing you with a single destination for enrolling in voluntary insurance products and accessing hundreds of new discounted consumer offers. We are excited to offer this optional enhanced benefit to you and your family. What are Voluntary Benefits? Voluntary Benefits are a new suite of associate-paid benefit programs that provide CarMax associates with discounted group-rate pricing not available to the consumer market. This suite of programs has been selected to complement the core benefits provided by CarMax as optional tools to help you protect your earnings and savings. CarMax has selected YouDecide to provide you with a single destination portal for enrolling in voluntary insurance products. This portal is accessed outside of Workday and will be available starting January 1, How can they help me? Programs such as Critical Illness, Accident, and Hospital Insurance all provide a cash payment, paid directly to you, when you experience covered event. These programs provide you with a financial safety-net that helps pay for your out-of-pocket costs associated with a covered event. You have the option to cover just yourself, a spouse and/or dependents. Coverage is guaranteed, which means you never have to answer medical questions. To make this even easier, your premiums are paid through a convenient payroll deduction so you don t need to worry about monthly payments. Should you decide to change jobs or retire, you can take the program with you! How easy is that? The insurance programs and discounts described here are not endorsed, offered or sponsored by CarMax. These programs are entirely voluntary. If you choose to enroll in one or more of these voluntary programs or otherwise purchase the vendor s product, all terms and conditions, including discounts, are between you and the insurance company or discount provider. Participation is completely voluntary. No contributions are made by CarMax and CarMax has not engaged these providers. Instead, CarMax has selected YouDecide to provide you with access to these programs and discounts. All questions regarding the terms and conditions of these programs should be directed to the insurance or discount provider and not to CarMax. All premiums and fees for programs in which you choose to enroll are due from you, but CarMax will permit applicable premiums to be paid through payroll deduction on an after-tax basis. YouDecide is permitted to publicize the programs to you. 3 YouDecide gives you access to programs that help you manage out-of-pocket expenses for planned life events (and for life s surprises that you don t always expect). VOLUNTARY BENEFITS OPEN TO ENROLL - JANUARY 15-31, 2018 COVERAGE EFFECTIVE STARTING MARCH 1, 2018 ACCIDENT INSURANCE Provides you with a cash payment to cover out-of-pocket expenses you may incur as a result of an accident. CRITICAL ILLNESS INSURANCE Provides you with a cash payment to cover out-of-pocket expenses when related to a heart attack, stroke, cancer, kidney failure, or other covered conditions. HOSPITAL INDEMNITY Provides a cash payment to offset out-of-pocket expenses associated with a covered hospital stay complementing your health insurance coverage. COMING SOON - MARCH 1, 2018 COMMONBOND Payoff your student loans faster with a lower interest rate. CommonBond provides you with a dedicated customer service team who is committed to making your student loan experience as painless as possible. INFOARMOR IDENTITY THEFT A few simple steps can reduce your exposure and protect you and your family. 4 Accident Insurance What are the odds? Accidents are stressful enough on their own. Knowing that you have secured additional coverage above and beyond your medical plan makes accidental injuries less stressful. Benefits for Accident Insurance are distributed directly to you in a cash payment once you have incurred expenses related to a covered injury. This gives you the ability to choose where the money needs to be applied to cover your most urgent expenses. Payments are distributed once a claim has been submitted. Coverage is available for spouses/domestic partners and eligible dependents. Premiums are based on the tier of coverage you choose, (associate only, associate plus spouse or domestic partner, associate plus children or family coverage). Coverage is guaranteed, so you never have to undergo medical underwriting, exams or tests to participate. This program is only available for enrollment during the CarMax Open Enrollment window, January 15-31, Coverage is effective starting March 1, What s covered? 5 How does it work? Aflac s Accident Insurance pays cash directly to you to cover out-of-pocket expenses you may incur as a result of an accident, such as deductibles, copays, transportation to/from medical centers, childcare and more. It s insurance for daily living. You injure your leg playing pick-up basketball and go to the hospital via ambulance. You have an X-ray and the emergency room doctor diagnoses you with a fracture and treats you. You leave the hospital on crutches and will require 3 follow-up visits with a doctor. $1,675 Here s a sample of benefit payments you would receive if you should trip while playing basketball. Ambulance $200 Emergency Room and X-rays $125 Short observation period $25 Fractured Leg (Closed) $1,200 Three Follow Up Visits $25 $75 Appliances (Crutches) $50 Receiving a cash payment gives you the ability to choose where to apply the money to cover your most urgent expenses. Benefits are distributed once a claim has been submitted and paid out based on your schedule of benefits and the services received. Coverage is also available for eligible dependents. Premiums are based on the coverage tier selected. If you are out of work for a period of time, will your disability payments cover your living expenses? 4
6 Critical Illness Insurance Focus on your health, not your medical expenses. Critical Illness Insurance pays a lump sum cash benefit, up to $15,000, upon the diagnosis of a covered illnesses including, Cancer, Heart Attack, Stroke, Major Organ Failure, Renal Failure and Coronary Artery By-Pass Surgery. This allows you to choose where the money needs to be applied for your most urgent expenses. Associates may elect to extend coverage to eligible family members as well. Premiums are based on age and coverage tier. Spouse and Domestic Partner premium rates are the same as associate rates, and children up to the age of 26 are covered at 100% with no extra cost. This program is only available for enrollment during the CarMax Open Enrollment window, January 15-31, Coverage is effective starting March 1, Is this program for me? Do you have family history for cancer, heart disease or other critical conditions? Do you have funds readily available to offset your medical expenses? If you became disabled from a covered critical illness, would you need to dip into savings to pay for your day-to-day living expenses (rent, mortgage, car payments, insurance, child care)? 7 $ How does it work? Aflac Critical Illness is an insurance plan designed to pay you up to a $15,000 post-tax cash benefit if you or a covered family member are diagnosed with a covered illness. Annual Wellnes Benefit Aflac will pay $50 for health screening tests performed as the result of preventive care, including tests and diagnostic procedures ordered in connection with routine examinations. This benefit is payable for the covered associate and spouse, once per calendar year. This benefit is not paid for dependent children. Second Occurrence Benefit: A Second Occurrence Benefit is payable when an insured is diagnosed with a separate covered condition under the Critical Illness plan $6.27 $ $8.32 $ $13.12 $ $16.89 $ $25.27 $ $45.76 $ $45.76 $91.52 INSIDER TIP: It s helpful to have Critical Illness in addition to short term disability insurance because the benefit is payable in a lump sum upon diagnosis, so instead of relying on a weekly or bi-weekly disability check, you are receiving a lump sum payment that you can use for incurred but unpaid medical expenses, car payments, rent or insurance. Also, since you pay the premium with after tax dollars, the benefit you receive is tax-free. This voluntary policy is intended to supplement (not replace) promary medical coverage. 8 Hospital Insurance Peace of mind and real cash benefits Aflac s Hospital Insurance is designed to pay you cash benefits when you or a covered family member are admitted to the hospital due to a planned or unplanned event. There are no pre-existing condition limitations. For example, if you are currently pregnant and enroll in this plan, you will be eligible for payments for giving birth in a hospital after the 3/1/18 effective date. Any trip to the hospital can present you with unexpected costs. This plan pays an initial admission benefit of $1,000 and a daily confinement benefit of $150 for every day you, or a covered member, are hospitalized. Each hospital confinement will pay up to 31 days. This program complements your medical plan to help pay for out-of-pocket costs incurred during a hospital stay. You can use the cash amount to pay for your general living expenses, such as gas, food, or everyday bills. This program is only available for enrollment during the CarMax Open Enrollment window, January 15-31, Coverage is effective starting March 1, What s covered?
