The signs pointing to a critical illness are not always clear. Critical illnesses may not be preventable, but our coverage can help offer financial protection in the event you are diagnosed. Group Comprehensive Critical Illness Insurance can help offer peace of mind when a covered critical illness diagnosis occurs. Below are two examples of how benefits might be paid. James Smith chooses a $15,000 face amount during his enrolment Six months later he suffers a heart attack, he is hospitalized for three days, and then he is released. He is expected to make a full recovery. Our Group Comprehensive Critical Illness benefit provided James with the following: Heart Attack (100%) $ 15,000 Total Benefits: $15,000 Jane Adams chooses a $15,000 face amount during her enrolment One month later, Jane s doctor tells her she needs aortic surgery to repair a diseased section of her aorta. She has aortic surgery right away. One year later, she undergoes a coronary artery bypass surgery to unblock an artery to allow blood to flow to her heart. Our Group Comprehensive Critical Illness benefit provided Jane with the following: Aortic Surgery (100%) $ 15,000 Bypass Surgery (100%) $ 15,000 Total Benefits: $30,000 James received a $15,000 cash benefit for his Heart Attack. Jane required Aortic Surgery which provided a $15,000 cash benefit, and then received a $15,000 cash benefit for her Coronary Artery Bypass Surgery procedure. Coverage for remaining covered critical illnesses and Waiver of Premium will continue for both James and Jane. The example shown may vary from the plan your employer is offering. Each covered critical illness is payable only once per covered person, with at least 90 days separating each covered critical illness. Once the basic-benefit amount has been paid for each covered critical illness, no more benefits are payable for that covered person. Kidney Failure (100%) Pays a benefit when you have kidney failure. Carcinoma in Situ (25%) Pays a benefit when you are diagnosed with cancer in situ. Invasive Cancer (100%) Pays a benefit when you are diagnosed with invasive cancer. Alzheimer s Disease (100%) Pays a benefit when you are diagnosed with Alzheimer s Disease. Multiple Sclerosis (100%) - Pays a benefit when you are diagnosed with multiple sclerosis. Paralysis (100%) - Pays a benefit when you suffer a complete and permanent loss of use of two or more limbs. Deafness (100%) - Pays a benefit when you are diagnosed with total and irreversible loss of hearing in both ears. Blindness (100%) - Pays a benefit when you are diagnosed with total and irreversible loss of sight in both eyes. Aortic Surgery (100%) - Pays a benefit when you require surgery to replace a diseased thoracic or abdominal aorta with a graft. Benign Brain Tumour (100%) - Pays a benefit when you are diagnosed with a brain tumour by biopsy, surgery or examination. Coma (100%) - Pays a benefit when you are unconscious for more than 14 consecutive days. Severe Burns (100%) - Pays a benefit when you are diagnosed with third degree burns over more than 20% of your body. Loss of Speech (100%) - Pays a benefit when you are diagnosed with total and irreversible loss of the ability to speak due to injury or disease. Waiver of Premium (Employee/Member only) - Pays your premium if you are disabled for a minimum of 90 days in a row due to a critical illness (for which a benefit has been paid) for as long as the disability lasts, for up to 2 years. CERTIFICATE SPECIFICATIONS Employee/Member/Dependent Eligibility - Your employer determines your eligibility. Family members eligible include your spouse and children, domestic partner and domestic partner s children. Employee/Member/Dependent Termination of Coverage - Coverage ends when the policy is terminated, premiums aren t paid, last day of active employment, you are no longer eligible, a false claim is filed, or when all critical illness benefits have been paid. Child(ren) coverage ends at age 21; 25 if in full-time attendance at an institution of higher learning. Spouse, upon valid decree of divorce or your death. Domestic partner, when the domestic partnership ends or your death. If coverage ends for any reason other than non-payment of premiums, a false claim filed, or all critical illness benefits being paid, you may be able to continue your coverage. Refer to the Portability Privilege below and in the issued certificates. Portability Privilege - Coverage may be continued when coverage under policy ends. BENEFIT CONDITIONS Benefits are not payable for any critical illness diagnosed prior to the effective date; subject to exclusions and limitations. All critical illnesses must meet the definitions and dates of diagnoses (must be separated by 90 days) for each illness stated in the policy and be diagnosed by a physician while coverage is in effect. Emergency situations outside Canada will be considered by a Canadian physician on foreign soil or when you return to Canada. Exclusions and Limitations We do not pay benefits for: (a) war, participation in a riot, insurrection or rebellion; (b) intentionally self-inflicted injury; (c) engaging in an illegal occupation, felony or attempted felony; (d) attempted suicide while sane or insane; (e) injuries while under the influence of alcohol, narcotics, or controlled substances or drugs, unless doctor prescribed; (f) participation in aeronautics unless a fare-paying passenger on a licensed common-carrier aircraft; (g) alcohol abuse or alcoholism, drug addiction or controlled substance dependency. Not included under benefit: Heart Attack - An incidental finding of ECG changes suggesting a prior myocardial infarction, in the absence of a corroborating event, and elevation of cardiac markers to coronary angioplasty unless there are diagnostic changes of new Q wave infarction on the ECG; Stroke - Transient ischemic attacks (TIAs), intra-cerebral vascular events due to trauma, and lacunar infarcts which do not meet the definition of a stroke; Kidney Failure - Does not include the failure of both kidneys due to a traumatic event, such as surgical trauma(s); Carcinoma in situ - Skin malignancies other than melanoma not invading the dermis; Invasive Cancer - Skin cancer other than invasive malignant melanoma in the dermis or deeper or skin malignancies that have become metastatic; Alzheimer s Disease - All other dementing organic brain disorders and psychiatric illnesses; Parkinson s Disease - All other types of Parkinsonism; Coronary Artery Bypass Surgery - Balloon angioplasty, laser embolectomy, atherectomy, stent placement, or other non-surgical procedures; Benign Brain Tumour - Pituitary adenomas less than 10mm, tumours of the skull, or germinomas; Coma - A medically-induced coma, a coma which results directly from alcohol or drug abuse, or a diagnosis of brain death.
