2 WHEN TO ENROLL FOR 2017 ANNUAL ENROLLMENT Enroll at livetheorangelife.com anytime Nov. 2 Nov % of associates saved time last year by enrolling online. You also can enroll by calling the Benefits Choice Center at , between 8 a.m. and 6 p.m. CT, Mon. through Fri. Call volume may be heavy, so please be prepared for long wait times benefit elections will carry over to 2017 if you do not take action. Exceptions include: If covered under a Home Depot Medical Plan: You will need to evaluate your new Medical carrier, Imagine Health, Elect your Healthy Living Credit, and Complete your Tobacco-Free Certification. If you want coverage in a Health Care and/or Dependent Care Spending Account, you will need to actively enroll. 3 TOP THINGS TO KNOW NEW! NEW MEDICAL CARRIER We are excited to announce that we will be partnering with Imagine Health as the primary Medical carrier in your area for If you are covered in the Medical Plan in 2016, you will automatically be enrolled in an Imagine Health option in See page 2 to learn more. RX Since the FDA no longer requires a prescription for certain heartburn and allergy medications, and they are now available over the counter, they will no longer be covered under the prescription drug plan. See page 6 to learn more. TOBACCO-FREE STATUS Don t forget to make your annual Tobacco-Free Certification. 1 Also for 2017, the Tobacco-Free definition has been updated to align with the FDA s Center for Tobacco Products definition. The Medical Plan recognizes the use of tobacco products as any of the following: Cigarettes Cigars Pipes Electronic Delivery Systems (such as e-cigarettes/vape pens) Smokeless tobacco Roll-your-own tobacco Dissolvable tobacco products NEW for 2017! If you do not certify that you are Tobacco-Free, you will automatically receive Tobacco-User Medical payroll deductions. HEALTHY LIVING CREDITS Don t forget to make your annual Healthy Living Credit election during your enrollment. To begin receiving your Healthy Living Credit in your first 2017 paycheck, you will need to complete the OrangeLife Questionnaire within 15 days of your enrollment. See page 7 to learn more. PLEASE NOTE: After the 2017 Annual Enrollment Period you will not be able to make a change unless you experience a Qualifying Life Event. This is a requirement by the Federal Government. 1 To qualify for the Tobacco-Free Medical payroll deductions, you must actively certify during your enrollment that you are at least 4 months Tobacco-Free (or have never used tobacco) and will continue to be Tobacco-Free in If you think that you are unable to meet the Tobacco- Free standards, contact the Benefits Choice Center prior to 4/1/17 at and we will work with you (and, if you wish, your doctor) to find a reasonable alternative to allow you to earn the lower payroll deductions. 1

The Imagine Health Medical Plan options will be offered to those who live in certain ZIP codes located in Chicago, IL and Dallas-Fort Worth, TX. How do I find in-network Imagine Health providers? To find in-network doctors and facilities, go to livetheorangelife.com/ih or call an Imagine Health Concierge at Imagine Health Concierges are available between 8 a.m. and 6 p.m. CT, Mon. through Fri. What happens if I am traveling outside of the Chicago or Dallas-Fort Worth areas and need care? EMERGENCY CARE Any hospital ER for emergency care will be considered in-network, regardless of where you are in the United States. MINOR ILLNESSES If you are outside the Chicago or Dallas-Fort Worth area and need to seek care, you can use the provider look up tool at livetheorangelife.com/ih or call an Imagine Health Concierge at to find the nearest walk-in clinic or urgent care clinic that is in-network. ALSO REMEMBER... You have 24/7 Care available that provides quick access to nationwide board certified medical expertise from the comfort of your home or hotel room. 9 HEALTHY LIVING CREDIT PERSONALIZED ACTION STEPS FOR BETTER HEALTH IN THE $25 BIWEEKLY CREDIT IS AVAILABLE TO ASSOCIATES AND SPOUSES ENROLLED IN A GOLD OR SILVER PLUS HOME DEPOT MEDICAL PLAN. STEP1 Take the Questionnaire STEP2 Know your Numbers STEP3 Take Action Elect the Healthy Living Credit during your enrollment session. Then complete the short online OrangeLife Questionnaire within 15 days of your enrollment to receive your $25 biweekly credit as of Jan. 1. The link to the Questionnaire will be available after you complete your enrollment. Participate in the Hidden Health Risk Screening by Feb. 3 to continue earning your Credit. You ll receive a Healthy Living Checklist with action items personalized to you. Complete two items by the deadlines to continue your Credit. COVERING YOUR SPOUSE? If your spouse will be covered under a 2017 Home Depot Medical plan, he/she will receive information about earning the Healthy Living Credit and it will be available at livetheorangelife.com. As a Credit participant, your spouse will need to create his or her own account at livetheorangelife.com. If your physician believes that participating in the Screening or completing the Checklist items is medically inappropriate in your particular situation, you can complete a Physician Waiver Form. A physician s signature is required, and the form is available at livetheorangelife.com > Health & Wellness > Healthy Living or by calling the Benefits Choice Center at Participation in the Healthy Living Credit steps is completely voluntary. Choosing not to participate will only result in individuals not receiving the Credit. 7

In some instances your paycheck may not be enough to cover the entire amount of your benefits premiums. In those cases, the amount of the premium above your paycheck is still owed and will be collected from your future paychecks.
