2 Welcome Welcome to Guitar Center! As a regular, full-time associate, you are eligible to participate in Guitar Center s sponsored benefits effective the first of the month following 30 days of employment. (Refer to the Benefits Summary Plan Description for eligibility rules for transfers and rehires.) This Benefits Newsletter, along with additional documents located in your Benefits Packet, contains information to assist you in deciding which benefit plans you should participate in and how to enroll into each plan. Eligibility You are eligible for benefits if you are a full-time associate working an average of 30+ hours per week. Your eligible dependents, including domestic partners, legal spouses, and children up to the age of 26, are also eligible to participate beginning the first of the month following 30 days as a regular, full time associate. Levels of Coverage Guitar Center s medical, dental, and vision plans allow for associates to elect the appropriate level of coverage for each specific healthcare plan. For example, you can choose Family coverage for medical, Associate + Spouse for dental, and Associate Only for vision. The levels of coverage from which you can choose are: Associate Only Associate + Child(ren) Associate + Spouse/Domestic Partner Benefit Advocate Center Rock-Enroll Benefit Advocate Center (BAC) In addition to the great team we have at Guitar Center, we also partner with a Benefit Advocate Center to help address benefit questions pertaining to such issues as: Medical, Dental, or Vision Life or Disability Pre-certification, Appeals Advice, or Claims Resolution Care Coordination or Procedural Cost Estimator All your benefit and enrollment questions can be answered by calling (844) In addition, you can reach a Benefits Advocate at BAC.GuitarCenter@ajg.com. If any statement conflicts with applicable plan documents, the plan document will govern. 3 Medical Administered by Meritain Guitar Center offers two PPO medical plans: the Standard Plan and the Consumer Driven Health Plan (CDHP). The following is a summary comparison of the differences between our two medical plan options. This comparison only identifies in-network costs. Detailed plan descriptions and coverage are available on the medical section of my.adp.com. This means that if you elect the Standard or CDHP medical plan, you will automatically receive prescription coverage. CVS/Caremark is the Pharmacy Benefits Manager for your health plan which offers many in-network pharmacy options, including CVS. If you elect the Standard plan, you will be subject to the deductibles and copays indicated in the chart above. If you elect the CDHP, you will be subject to the medical plan s deductibles and coinsurance. There are no independent deductibles or copays. You will still receive the CVS network discount but will be responsible for the remainder of the cost of the prescription, which will be applied towards your deductibles and coinsurance. Note: If you are enrolled in the CDHP and have a chronic condition such as diabetes, high blood pressure, asthma, or high cholesterol, the deductible will be waived for identified generic preventive medications. This means you only pay the 20% coinsurance, even if you have not yet met your annual deductible. Guitar Center is committed to ensuring associates and their families have available medication to maintain their health. 2

4 Telemedicine Guitar Center offers telemedicine in partnership with Teledoc to all associates and their families enrolled in the Standard or CDHP medical plans. This benefit is a medical hotline answered by licensed U.S. physicians. Just like nurse hotlines, this hotline provides medical consultation. Even better than a nurse hotline, Teledoc physicians can also prescribe medication for medical and dental needs. As part of this benefit, Guitar Center pays for the cost of your hotline call! This is the equivalent of paying for your office visit when you go to see the doctor! All you pay is the cost of the prescription, if needed. You have now saved yourself an office visit! Health Savings Account (HSA) Administered by Fidelity By enrolling in the CDHP, associates are eligible to establish a health savings account that can be used to pay for qualified healthcare expenses incurred by you, your spouse, or any dependents. Contributions will be deducted from your paycheck pre-tax. The great thing about this account is that there is no use-it-or-lose-it rule, balances accumulate pre-tax and carry over year to year. You can elect the amount you would like to contribute at my.adp.com. Contribution limits set by the IRS in 2018 are: IMPORTANT! Guitar Center provides an HSA contribution match up to $750 for Associate Only coverage, or up to $1,500 for any other level of coverage elected on the CDHP. Company funding is made each paycheck based on your enrollment and contributions. You must contribute to the HSA account in order to receive the employer match! $3,450 individual/$6,900 family for combined company and associate contribution. Those over 55 years of age, can contribute an additional $1,000 each calendar year. Flexible Spending Accounts (FSA) Administered by Discovery A Flexible Spending Account is an account where you can set aside pre-tax dollars to use on a variety of expenses. See below for details on the different kinds of FSA s that Guitar Center is offering in For additional details, you can visit my.adp.com. Healthcare (HCFSA) - Allows associates enrolled in the Standard PPO Plan to set aside pre-tax dollars to pay for out-of-pocket healthcare (medical, dental, and vision) expenses incurred by you or your dependents. Maximum: $2,650. If you are enrolled in the CHDP, you are not eligible to participate in this account. Limited Purpose (LPFSA) - Allows those enrolled in the CDHP to set aside pre-tax dollars to pay for out-of-pocket dental and vision expenses incurred by you or your dependents. Maximum: $2,650. If you are enrolled in the Standard Plan you are not eligible to participate in this account. Dependent Care (DCFSA) - Allows you to set aside pre-tax dollars to pay for out-of-pocket expenses incurred during the care of your dependent children under the age of 13 or an incapacitated spouse or dependent adult who lives with you at least 8 hours per day. If you are married, you and your spouse must be working or attending school full-time. Maximum: $5,000 if married and filing a joint return or if a single parent, or $2,500 if married and filing separately. Commuter Benefit (CBFSA) - Allows you to set aside pre-tax dollars to pay for commuter expenses. You can sign up for one month at a time or schedule a recurring deduction it is your choice. Maximum: $255 per month for transit expenses (i.e., bus or metro train usage). Per-month maximum for parking expenses vary by city for a Commuter Flexible Spending Account. Note: This benefit is available to both Full- and Part-Time Associates. 3

6 Life and AD&D Administered by Voya All regular full-time associates are provided with Basic Life insurance, which is paid for by Guitar Center. In addition to the Basic Life insurance, you have the option to purchase additional Voluntary Life insurance for yourself, your spouse, and child(ren). The table below provides a summary of the benefit plan options and maximums. Detailed plan documents are available on my.adp.com. The EOI is completed through my.adp.com. Disability Insurance Administered by Unum Disability insurance provides you with income protection if you are unable to work due to sickness or injury (non-work related). Optional Short-Term and Long-Term Disability insurance plans are available through UNUM based on your employment class. The tables below provide summaries of the benefits and waiting periods. 7 Voluntary Benefits Administered by Voya We offer three voluntary benefits that are 100% employee-paid. They are intended to bridge the gaps in medical coverage and pay you money when you need it most. If you are interested, you can enroll on my.adp.com. Critical Illness Insurance When a serious illness strikes, critical illness insurance can provide financial support to help you through a difficult time. It can pay you a lump-sum cash benefit, which you can use any way to meet your needs. To help prevent illness, this plan can also pay you an annual cash benefit when you take a covered health screening test. Accident Insurance Pays you benefits for specific injuries and events resulting from a covered accident. Accidents can happen 24 hours a day, 7 days a week. If you or a loved one suffers an accident, it can cause more than just physical pain, it can also affect your finances. Your current medical coverage helps pay for expenses caused by an accident, but won t cover all of the out of pocket expenses you may face. With Accident insurance, you can receive up to $100k in a lump sum depending upon the accident or disability. Hospital Indemnity Insurance This plan pays a daily benefit if you have a covered stay in a hospital, critical care unit, or rehabilitation facility. The Benefit Amount is determined based on the type of facility and the number of days of your stay. Voluntary Benefits Administered by MetLife Hyatt Legal Plan The Hyatt Legal Plan provides legal services from attorneys experienced in estate planning documents, civil suits, adoption, creditor issues, and much more. Sign up for a convenient payroll deduction of just $9.25 bi-weekly and save money on attorney fees. Note: Once you enroll, you must remain in the plan for the entire plan year. Auto & Home/Renter s Insurance, and Pet Insurance To get a quote and learn more about these plans, go to mybenefits.metlife.com, and enter Guitar Center, Inc. as the company name for personalized service. These benefits are direct billed and cannot be made via payroll deductions. Gym Membership Discount Guitar Center partners with GlobalFit which offers discounts for you and your family at more than 10,000 gyms nationwide! These are full-access memberships with the same privileges as full-cost retail memberships. They also offer discounts on a wide variety of products for physical activity and a well-rounded active lifestyle like videos, tracking tools, newsletters full of health tips and recipes, etc. To find out more about GlobalFit and how to join, visit their website at or call

8 How To Complete Your PHP Administered by Health Advocate Get a snapshot of your current health status and risk of certain diseases to help you set personal health goals. Steps to participate in a Personal Health Profile (PHP) 1. Go to HealthAdvocate.com/gc 2. Register as a first time user by clicking Register Now. 3. Enter your information exactly as it is documented in ADP. 4. Create a unique username/password, provide an active address, select security questions, and then click Save and Continue. 5. Log in with your username and password. 6. Under To Do List, click Complete Your Personal Health Profile. 7. Indicate your Gender, Date of Birth, and click Start a New PHP. 8. Read and answer ALL questions. 9. Once finished, click Finish and Generate Report. 10. Take the time to review your report for ideas on how you can begin to make changes in your daily habits that will have a positive impact on your health. Spousal Surcharge A Spousal Surcharge will be applied to spouses that have access to their own employer s group medical plan, yet choose to be covered under Guitar Center s medical plan. The intent of this surcharge is to help control medical plan costs for associates and their dependents who do not have the option of group medical coverage elsewhere. The spousal surcharge is $50 per pay period. Note: Answering this question falsely is considered fraud and subject to discipline including termination. These surcharges only apply to associates participating in a GC medical plan. Smoking Cessation Administered by Health Advocate Guitar Center understands how difficult it is to quit and provides a robust Smoking Cessation program. You and your spouse are eligible to participate at no cost to you! To start your tobacco-free journey, call (888) ! How to Enroll In Benefits Your have 31 days from your date of eligibility, which is on the first of the month following 30 days of employment as a regular full-time associate, the enroll in benefits. Benefits enrollment is only available online at my.adp.com Registration 1. Go to my.adp.com and click Register Now. 2. Enter the registration code GUITAR and click next. 3. Click Next. 4. Enter your name and other requested information. 5. Click Confirm then Register Now. 6. Enter your contact information. 7. View your USER ID. 8. Create a PASSWORD. 9. Select and answer security questions. If prompted, read the terms and conditions. 10. Click Register (or Register Now). 11. You will receive two s, the first will be to welcome you to the site and the second will be to activate your address should ever need to have your password reset. 12. You can now log in to the ADP Portal and will be able to view/edit your Benefit enrollment. Important: You should make a note of your User ID and Password and keep them in a safe place, as you will need them to log on to ADP services. Complete your Benefits Enrollment 1. Click on the Benefits icon located on the left hand side of your screen. 2. Click ENROLL NOW in the enrollment window. 3. Add any dependent information to the Who is Covered page; or click on the Benefits button on the top left-hand corner if you do not have dependents to cover. 4. Scroll to the Medical section and click on the SELECT button to begin the enrollment process. The system will guide you through each benefit option. Make sure to click the checkbox next to each dependent you wish to cover. Important: Confirm and submit your enrollments once you have made all your selections. 7

Benefit deductions are only processed on the first two paychecks of each month. In months when there are three paychecks, the third paycheck will not have any benefit deductions. For Guitar Center associates who are paid on a weekly basis, you will have 52 pay periods each year. Benefit deductions are only processed on the first four paychecks of each month. In months when there are five paychecks, the fifth paycheck will not have any benefit deductions. 8

10 401(k) Retirement Savings Administered by Fidelity As a Full-Time or Part-Time Associate, you are eligible to participate in the Guitar Center 401(k) retirement plan administered by Fidelity. Whether you plan to retire years down the road or just around the corner, the time to start saving for retirement is now. 401(k) Match Guitar Center provides an employer match into your 401(k) $1-for-$1, up to 2% of your eligible wages per pay period. Associates are eligible to begin receiving the matching contributions after a 1 year vesting period. Contributions You can begin contributing into either the standard pre-tax 401(k) or Roth 401(k) plans immediately. Depending on the timing, it may take two pay periods before the payroll deductions will begin. Guitar Center matching funds are always 100% vested. This means your entire account balance is yours to keep, even if you part ways with Guitar Center. No Fees As an active Guitar Center Associate, fund administration fees and the annual recordkeeping fees are all covered by Guitar Center. Annual Contribution Max 2018 IRS Limit: $18,500 Age 50 catch-up rule: $6,000 Register If you already have a Fidelity account, you can use the same login credentials. Otherwise, follow the easy steps below to Register and create a login: Go to Fidelity s website at and click Register. Verify your identity by putting in the last 4 digits of your SSN, Name, and Date of Birth. Next, create a Username and then a new Password using the criteria listed. The next screen will state that you have successfully registered and can now login and setup your contribution (%) which will feed over to payroll in 1-2 payroll cycles. If you experience any issues registering, please contact Fidelity at You will now see your 401(k) & GC match contributions on every paycheck in my.adp.com as well as online on Fidelity s website 401k.com under the Summary tab. Employee Assistance Program Employee Assistance Program (EAP) you and your dependents can receive up to three face-to-face counseling sessions per issue with Masters-level clinicians at no cost. Counselors can address diverse issues such as pre-marital and post-marital counseling, legal services, financial services, parenting services, and substance abuse. A toll-free number of (877) is available 24 hours a day, 7 days per week. Tuition Reimbursement Program Guitar Center offers an educational assistance program that covers tuition, lab fees, and books up to a maximum of $1,500 per calendar year. You are eligible if you are a full-time active associate with no performance issues who has been with the company for at least 6 months. For additional information, visit my.adp.com. 9

Note: Temporary cards can be obtained online at If you enroll in the Health Savings Account (HSA), you will receive a debit card from Fidelity once your account is approved and opened.
