1 RESOLUTION NO A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF BONNEY LAKE, WASHINGTON, APPROVE THE INTOLIGHT (PUGET SOUND ENERGY) STREET LIGHTING ORDER FOR SR A VENUE E ROAD IMPROVEMENTS PROJECT. WHEREAS, the City and Visconsi desired to amend the Development Agreement to allocate responsibility for the improvements to SR 410 and 204 t h A venue E to the City of Bonney Lake on October 18t h, 2016 (Resolution 2552); and, WHEREAS, the City Council approved the biennial budget for the budget cycle on December 13, 2016 (Ordinance No. 1563); and, WHEREAS, the budget approved in the budget cycle for the SR th Intersection Improvement Project was $3,842,000 which included all items to complete the project; and, WHEREAS, the City requested a cost estimate from Intolight to fashion and install the street lights for the SR th Intersection Improvement Project; and, NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF BONNEY LAKE, WASHINGTON HEREBY RESOLVES AS FOLLOWS: That the City of Bonney Lake Council does hereby authorize the Mayor to approve the attached estimate with Intolight in the amount of $195, which includes tax. 2 INTO T STREET LIGHTING AUTHORIZATION LETTER Rev April 7th, 2017 City of Bonney Lake John Woodcock PO Box 7380 Bonney Lake, WA Street Lighting for SR 410 at 2041h Ave NE extension Dear John, The cost for the installation of (1 B) 40' MH Black steel WSDOT slip based poles, (1 B) 285W LED black cobrahead fixtures, and applicable circuitry is $195, It is based on the description in the Custom Street Lighting Order enclosed. This estimate is valid for 90 days. After reviewing the enclosed please sign and return this Authorization Letter, and the attached Custom Street Lighting Order. Return one copy, and retain the other copy for your file. Upon receipt of the signed agreement there is a week waiting period for us to procure the necessary materials. The following are conditions that will be required before construction of this system: 1. In the area where we are placing our cables and equipment, it is necessary that area be within four (4) inches of grade. If not, you may be required to pay the cost of relocating or reburying our facilities. 2. Customer to provide trenching, back filling, pole holes, and installation of mounting tubes which are to be 36" diameter by 54" length corrugated plastic pipe. 3. Customer responsible for providing and installing conduit PSE will provide composite handholes for contractor installation. 5. PSE will pour bases and install anchor bolts 6. Customer is responsible for street light locations. 7. Please provide anticipated date lighting installation should take place. Date: By signing this Letter, Custom Street Lighting Order and returning it, you are stating that you will comply with these requirements and authorize us to perform the work. (PSE), from City of Bonney Lake (Customer) covers the Installation of custom lighting authorized by this order. Billing will be on a Monthly basis and in accordance with the terms and conditions contained in PSE's Schedule 51, and any future modifications of such Schedule as may be approved by the Washington Utilities and Transportation Commission. Ownership of all conductors, poles, fixtures, lamps and accessory equipment installed as a result of this order shall remain with PSE. The number, size and type of lights ordered are summarized below.
