2 Sectin 2: PROJECT PROPOSAL NARRATIVE: Please prvide clear and cncise answers fr yur prpsal type (chse 1 f 3 utlined belw). Yur narrative shuld be n mre than five pages; please number the pages. Grant Type A: Prgram r Prject Prpsals 1. What is the missin f yur rganizatin? 2. In the past three years, what tangible results has yur rganizatin achieved fr the peple yu serve? By results we mean nt yur activities r services, but what peple gain frm them. 3. What is yur prject? Define in summary frm what yu plan t undertake. What prblem r pprtunity are yu addressing and hw many individuals within yur service area cvered by the Fundatin (Berlin, New Britain, Plainville and Suthingtn) are affected by it? 4. Wh are yur participants? What are their characteristics and are they in any way different frm the full set f peple wh have the prblem within yur gegraphic fcus. 5. What results are yu cmmitted t achieving? Please be very specific abut the level f change that will cnstitute success. 6. What methd r apprach will yu use t achieve these results? 7. Why did yu chse this apprach and what evidence supprts using it? 8. Halfway thrugh yur prgram, what predictive factrs will yu lk fr t tell yu that yu are n curse t achieve yur results with the time and mney remaining? Please fcus n the prgress r the gain that yur participants are actually making rather than yur wrk plan. 9. Will the use f ur funds create an nging additinal expense f any kind in yur rganizatin? If s, what is the nature and the amunt f that expense and hw will yu cver it lng term? 10. What are the key factrs that will make yur prgram sustainable ver time? Grant Type B: Capacity Building Prpsals We chse t invest in the infrastructure f rganizatins that deliver valuable services t the cmmunity. An attractive candidate fr this type f investment is a grup that shws lng term, high prmise where enhanced capacity and sustainability will create increased human gain. Prpsals shuld fcus n the needs f the rganizatin that will strengthen its ability t achieve its missin. Please prvide cncise and clear answers t the fllwing questins: 1. What is the missin f yur rganizatin? 2. In the past three years, what results have yu achieved fr the peple yu serve? By results we mean nt yur activities r services, but what peple gain frm them. 3. Describe the challenge r pprtunity yu face that, if addressed, wuld have a clear impact n the gains yu achieve fr thse yu serve. 4. What is yur prject? Please define in summary frm what yu plan t undertake.

3 5. What will yu be able t achieve with this new capacity that yu cannt achieve nw? 6. What are the results yu cmmit t achieving with this new capacity (e.g., reducing time and csts, increasing revenue, increasing the gains frm yur prgram and/r the persns wh get them r decreasing tme t achieve gains)? 7. Define the 4-5 mst critical steps t building this new capacity and describe what must be achieved at each step t reach the desired result. 8. Halfway thrugh yur prject, what predictive factrs will yu lk fr t tell yu that yu are n curse t achieve yur results with the time and mney remaining? 9. Describe the changes that will be needed fr the new capacity t be successfully implemented (e.g., rganizatinal changes in plicies and practices). Will it require any adjustments in prgram r staffing t be fully successful? If s, please detail these changes and nte cmmitments made t make them. 10. If the new capacity is dependent n either a new staff persn r cnsultant, please define the rles and results needed frm each and attach a jb descriptin. 11. T the extent the new capacity brings net increase in perating csts, hw will yu pay fr them? Grant Prpsal C: Capital Grant Prpsals The Fundatin will cnsider funding capital campaigns r building renvatins. A capital request is a prpsal fr funds t cnstruct a new facility, renvate r add t yur current facility, r purchase and/r renvate anther existing facility fr use by yur rganizatin. Requests t supprt the purchase f general equipment als fall int this categry. As an investr, we view every capital grant applicatin in terms f the ways in which the imprvement yu seek can enhance yur ability t achieve strnger results fr thse yu serve. We want t knw hw the imprvement will be used t increase efficiency r effectiveness in yur rganizatin. Please prvide cncise and clear answers t the fllwing questins: 1. What is the capital imprvement yu want t make? (Be specific in such terms as kinds f equipment, square feet f space, etc. Please d nt send blueprints r specificatins. We will ask fr detail when needed.) 2. What are the results yu cmmit t achieving with this imprvement in place? Please specifically detail the ways in which the imprvement makes yu mre efficient (e.g., reducing time and csts) and/r effective (increasing the gains frm yur prgram and/r the persns wh get them.) 3. What steps have yu taken t insure that the imprvement is high quality and being acquired at the best pssible cst? List all cst savings ver a cnventinal retail cst that yu have realized. Please include a brief descriptin f yur bidding prcess; we may ask t see bids. 4. What are the implicatins f this imprvement n peratinal revenues and csts? If the imprvement creates higher new csts than new revenues, hw will yu pay fr them? 5. What is yur lng-term plan fr financially maintaining the new space r imprvements? 6. Will the new space require any adjustments in prgram r staffing t be fully successful? If s, please detail these changes and nte cmmitments made t make them.

4 Sectin 3: KEY PEOPLE AND GROUPS 1. Wh are the key persnnel that will lead the prject? Please include infrmatin n their backgrund and experience that mst predict their ability t achieve the stated results. 2. Fr capital cnstructin prjects, wh in the rganizatin is mst directly respnsible fr making building cnstructin decisins and cntrlling csts assciated with them? Please name the persn and indicate what previus success in this area r persnal qualificatins r attributes best predict they can d it. (If key persnnel have nt yet been hired, please prvide a jb descriptin.) 3. Wh are the key peple wh deliver the service and what factrs mst suggest that they can help peple get the intended results? 4. Is yur prgram dependent n any partners r intermediary grups? If s, wh are they, what must they d, and what assurance d yu have that they will d it? Sectin 4: FINANCIAL INFORMATION 1. Please cmplete and submit the Grant Request Budget as well as the Organizatinal Budget fr Grant Applicatin. These dcuments can be dwnladed frm 2. Please dcument the status f ther surces f funds being pursued in supprt f the prject. List the name f the entity, classificatin (gvernment, fundatin, crprate, individual, special event, r ther) and amunt. Indicate whether the funding request is pending, r has been declined, apprved, r received. 3. Unless this is a ne-time expense, please prvide us with a specific plan fr cntinuing the prject r prgram nce the grant ends. Sectin 5: ADDITIONAL MATERIAL: Fr all prpsals, please include three cpies f these additinal attachments with yur applicatin: IRS Determinatin letter; Bard f Directrs list with addresses and affiliatins; Mst recently cmpleted audit (r mst recent 990 if yur rganizatin des nt cmplete an utside audit); Organizatin s current perating budget fr current fiscal year which includes an additinal clumn with the previus year s budget and actual figures fr the previusly cmpleted fiscal year; Signed Certificate f Nn-Discriminatin (attached)
