2 Bread fr the City Representative Payee Prgram Cnsumer Rights and Respnsibilities Clients f Bread fr the City s Representative Payee Prgram have the fllwing rights: T be treated with dignity and respect. T participate in creating the Budget & Spending Plan that will guide hw their mney is paid ut fr rent, utilities, grceries, spending mney, etc. T have their questins answered whenever pssible. T have all payments made n their behalf in a timely manner as specified in the Budget & Spending Plan r within 48 hurs f a special request. T be met n time fr appintments. If necessary, t be ntified f any changes in scheduling as early as pssible. T privacy and cnfidentiality cncerning their cnversatins and case recrds. Such recrds will be released nly with their written permissin. Hwever, Bread fr the City Representative Payee Prgram staff will share infrmatin with ne anther. T infrm the prgram staff if they are nt satisfied with their care. If they feel their cncerns are nt answered, they shuld cntact the Representative Payee Prgram Manager. T file a grievance with the Department f Behaviral Health if nt satisfied by the prcess abve. T be free frm physical, mental, and sexual abuse by any staff r ther individuals n the premises. T receive the mst apprpriate treatment regardless f age, gender, race, religin, sexual rientatin, natinal rigin, disability r incme. Clients f Bread fr the City s Representative Payee Prgram have the fllwing respnsibilities: T treat staff, vlunteers, and thers wh they may cme int cntact with at Bread fr the City with dignity and respect. T participate in the planning f their Budget & Spending Plan and t fllw the plan. T be hnest with prgram staff when discussing anything that might affect their benefits. T keep their appintments n time r ntify the prgram staff f any changes as sn as pssible, preferably giving 24 hurs ntice. T participate in any treatment r services recmmended fr them by their Clinical Team at the Department f Behaviral Health. T let us knw if they are unsatisfied with services r if they have thughts f discntinuing Representative Payee Services at Bread fr the City. T reprt any changes in their living situatin t their Clinical Team and/r t Bread fr the City as sn as they ccur. Please see the fllwing page fr additinal details

3 Changes t Reprt It is imprtant that yu reprt any changes t yur living situatin t yur Clinical Team r t yur Bread fr the City RPP Crdinatr as sn as they ccur. It is ur respnsibility t reprt these changes t the Scial Security Administratin. This will ensure that yu cntinue t receive the crrect amunt f benefits and avid any verpayments. YOU MUST TELL US IF: Yu mve r yur address changes; Yu get married r separate frm yur spuse; Anybdy mves int r ut f yur husehld; Yu start r stp wrking (even if yu earn nly a little); Yu begin receiving any ther incme, the amunt f ther incme changes, r yu get any resurces (inheritance, prize winnings, settlements frm lawsuits, etc.); Yu take a trip utside f the United States; Yu g t jail r prisn; Yu are admitted t the hspital r a nursing hme; Yu save any mney; Yu are n lnger disabled r yur disability imprves; There are ther changes t yur current situatin that yu think might impact yur benefits. Grievances / Prblems with Yur Payee If yu are having prblems managing yur budget we encurage yu t speak first with yur Clinical Team at yur Cre Service Agency. Yu and yur Clinical Team will be reviewing yur Budget & Spending Plan regularly. When yu review yur plan, yu can discuss with yur Clinical Team whether there are changes that can be made t help yu better manage yur mney. Bread fr the City will be paying ut yur mney based n the Budget & Spending Plan and ther instructins we get frm yur Clinical Team. If yu have a prblem with the way that Bread fr the City is fllwing this plan and instructins, yu can request a meeting with yur RPP Crdinatr at Bread fr the City. We may ask yur Cmmunity Supprt Wrker t attend this meeting as well. If after meeting with yur RPP Crdinatr, yu still have cncerns abut the services yu receive at Bread fr the City, yu can request a meeting with the Representative Payee Prgram Manager. Yu can als file a Grievance at any time thrugh the DMH FAIR grievance prcess. We can prvide yu with infrmatin r assistance with this prcess. Please remember that Bread fr the City is respnsible t the Scial Security Administratin (r if applicable, t the Office f Persnnel Management) fr managing yur mney. We must use the mney t pay fr yur current needs including husing and utilities, fd, medical and dental expenses, persnal care items, clthing, and rehabilitatin expenses. After thse expenses are paid we can use any mney left t pay past-due bills, supprt yur dependents, r prvide entertainment fr yu. We can als help yu save sme f yur mney r plan fr burial expenses. We must keep accurate recrds f hw yur mney is spent and reprt this infrmatin t Scial Security. Finally, we will als be prviding yu with mnthly statements t shw yu hw yur mney is being spent

4 Yur Budget & Spending Plan Yur Budget & Spending Plan will utline hw Bread fr the City regularly pays ut yur mney each mnth. Fr example, it might say that we shuld mail a check fr $150 fr yur rent t yur landlrd each mnth and that yu will pick-up a $50 check fr spending mney twice per mnth. Every client s budget will be carefully planned t meet yur needs. Therefre, it is imprtant that yu and yur Clinical Team wrk tgether t develp yur Budget & Spending Plan. Yu shuld review yur plan regularly with yur Clinical Team t make any needed changes. A new plan shuld be submitted at least every 6 mnths. Smetimes yur Budget & Spending Plan will change unexpectedly. Yu may mve and need yur rent paid t a different persn r get cable installed and need t regularly have this new bill paid. When there is a change t ne f yur regular payments, yu must let yur Clinical Team knw as sn as pssible. They will fill ut a new budget and send it t us, s that we can make changes t the payments made frm yur accunt. Budgeting Tips: Fr weekly expenses, remember that sme mnths will have 5 weeks Ask utility cmpanies t mail bills directly t Bread fr the City s that they can be paid n time Cnsider budgeting a separate check fr grceries r requesting grcery gift cards fr fd shpping And Additinal Requests fr Funds Smetimes yu will have a special expense that is nt listed n yur Budget & Spending Plan. Fr example, yu may need extra mney t replace yur brken eyeglasses. When yu have a special expense, yu must cntact yur Clinical Team t request the funds. They will help t figure ut whether yu have mney available t cver this extra expense. If yu d, they can send us an Additinal Disbursement Request Frm t request a check. It will take 48 hurs t get yu the special check. Special requests include mney fr: birthdays, hlidays, clthing, majr items (TV, radi, etc), as well as checks paid t vendrs (cmmercial entities) fr services rendered/gds received. Yur Clinical Team can request extra mney fr yu up t 2 times per mnth. In any emergency situatin yu must cntact yur Clinical Team first, and yur Cmmunity Supprt Wrker may be able t make a request fr an emergency check (such as fr medical expenses, legal fees, husing expenses, etc) which may be available mre quickly. Additinal Request fr $ r mre require special prcessing: yu must have a specific need r intentin fr the funds and be able t return receipts t Bread fr the City that shw that yu spent the mney in that way. Yur check must be picked up by r mailed t yur Cmmunity Supprt Wrker, s that he/she can help t make sure that the receipts are returned t Bread fr the City. Tips fr Additinal Requests: If yu have a ne-time bill, such as an invice frm a dentist r ther service prvider, yur Cmmunity Supprt Wrker can simply frward the bill fr payment n Additinal Request Frm is needed. Additinal requests shuld be fr special expenses if yu find that yu need additinal funds fr everyday expenses such as bus fare, husehld r persnal hygiene supplies, r fd yu shuld discuss yur budget with yur Cmmunity Supprt Wrker. Regular mnthly bills such as cell phne r electric bills shuld als be included n yur budget and nt paid thrugh Additinal Requests. Please remember that we can nly release as much mney as yu have available in yur accunt and mney taken ut early in the mnth by Additinal Request may make it impssible t make ther payments frm yur budget later in the mnth

Legal Services Prvides assistance with Landlrd-Tenant, Child Supprt, and Public Benefits prblems. Als refers t the Emplyment Justice Center, DC Bar Advice & Referral Clinic, and ther legal service prviders.
