Step 1 Dwnlad and cmplete WPA Frm 8A frm the Massachusetts Department f Envirnmental Prtectin (DEP) website: Printed cpies may be btained frm the Cnservatin Office at the Twn Offices. Yu may wish t btain a wetland scientist, cnsultant, engineer, and/r land surveyr t assist yu with preparing the permit applicatin. If the Order f Cnditins fr this prject, r the prtin f the prject subject t this request, cntains an apprval f any plans stamped by a registered prfessinal engineer, architect, landscape architect, r land surveyr, then: Attach a written statement by such a prfessinal certifying substantial cmpliance with the plans. If any deviatin exists frm the plans apprved in the Order: Include an as-built plan stamped by a registered prfessinal engineer, architect, landscape architect, r land surveyr, clearly depicting and describing all deviatins. Use the included checklist t ensure that the plan(s) include at least the minimum required infrmatin. Step 2 Plan yur submissin date based n the hearing schedule. The Cnservatin Cmmissin generally meets every 1 st and 3 rd Tuesday f the mnth. Please cntact the agent prir t submitting the applicatin t determine the next available meeting. Generally applicatins shuld be submitted tw weeks prir t the meeting.
