A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving the Resolutions for the Clean Ohio Trails Fund, CR24A Extension, The Frost Extension / COTF, and the Engineer s Cost Estimates with Buckley Group, Project # 15577, and authorize Commissioner Eliason s signature on all related documents. Bob Eichenberg will also draft a letter of interest to submit to ODNR along with the above mentioned Resolutions. (Copied to pages 42-43). Court St. Athens, OH Dear Commissioners, This letter-form agreement is intended to specify the details of a contractual relationship between the Athens County Commissioners, hereinafter "County" and Round River Planning & Design, LLC, hereinafter "Consultant." The County is in need of assistance with planning and administration of several aspects of the County's planning function. The Consultant is a business managed by a certified planner, qualified to provide such assistance. The work to be provided to the County by the Consultant may include the following only upon authorization by the County: Grants administration, including but not limited to: grant writing, evaluating projects for feasibility and competitiveness, securing necessary information from other governmental departments or agencies, seeking matching funds, and assistance with administration of funded projects. Floodplain management - property determinations and answering citizen questions. Assistance with management of the meetings of the Regional Planning Commission and the Comprehensive Plan Advisory Committee. Other planning tasks the County deems necessary. The Consultant will charge the following rates for work provided:

Itemized invoices will be submitted monthly. The County point of contact will be that County Commissioner chosen by the Board of County Commissioners. The County agrees to provide secretarial.staff for necessary mailings, including copying, for any meetings the Consultant will manage. The County will also provide an employee who will notify the Consultant of citizens with questions or concerns related to county planning. The Consultant will review any phone messages or s sent by the County on a daily basis and respond by the next business day from when the phone message or is received. It is the intent of both parties that the Consultant will operate as an independent contractor, maintaining its own office, facilities, and equipment. Athens County is to be held harmless as a result of any action undertaken by the Consultant in the performance of its duties. This agreement may be terminated with 30 days written notice by either party. I. Coordinate strategic planning in the following manner: a. Studies and reports relating to the physical environmental, social, economic and government characteristics of the county, i.e. update Land Use Plan. e. Floodplain Management (map interpretation, enforcement, permitting, public information, coordination with other governmental units) b. Reviewing, evaluating, and making comments and recommendations on land use, open space, transportation, public facility plans and projects affecting area development. c. Coordinating with units of local government, non-profit organizations and chamber of Commerce on land use plan issues, i.e. site development for industry, Health Department sanitation and Community Action housing and transportation. d. Promoting understanding of and recommending administrative and regulatory measures to implement plans of the county. f. Hazard mitigation planning and management. g. A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving the following contract as requested by the Athens TB Clinic Board Contract / UMA County Tuberculosis Clinic Board: Between the Athens County Tuberculosis Clinic Board and University Medical Associates, Inc (UMA). The contract period shall be from January 1, 2015 to December 31, 2015 inclusive. The total amount of this contract shall not exceed $52, and consist of the following components. 1. Fixed costs which includes: The Directors Salary to provide oversight of the program 3% time = $3, per 12 month period. The Administrative Associate s salary to assist with support of the project 5% time = $2, per 12 month period. 2. Variable Costs include: For satisfactory performance of the clinical services described in Scope of Work the Board shall pay University Medical Associates funds to supply nursing staff for the clinic and supplies. Not to exceed: nursing staff salary and benefits per 12 month period = $7,000.00; supplies - $ Physician Services - Dr. Katy Kropf, D.O. or designee (a physician licensed to practice medicine in the State of Ohio) will provide medical supervision through University Medical Associates. Dr. Katy Kropf, D.O. and/or designee s hours will be billed at a rate of $ per hour per 12-month period - not to exceed $12,

County Tuberculosis Clinic Board: Between the Athens County Tuberculosis Board and Athens City-County Health Department. The Athens City-County Health Department agrees to provide Tuberculosis services, clerical support, office space and phone service for the needs of the TB Clinic Board for the control of tuberculosis. The Athens County Tuberculosis Board, agrees to provide the Athens City-County Health Department $48,000.00, in quarterly payment, for the contract period. The contract period begins January 1, 2015 and ends December 31, In addition, on an as needed basis the Board will reimburse the Department for staff completing direct observed therapy. The reimbursement will be at the rate of $45.00 for each hour of DOT services for a maximum of $12, under the contract and amendment. This guideline supersedes all previous guidelines that relate to this subject. PURPOSE - This guideline authorizes the payment of overtime wages for any part time employee who works a holiday as defined below. For the purpose of this guideline, a holiday shall be defined as midnight to midnight on the designated days. Part time employees are only paid if they actually work the holiday.
