Backgrund/Cntext 1.1 Purpse The Patients First Act, 2016 requires Lcal Health Integratin Netwrks (LHINs) t establish gegraphic sub-regins fr the purpses f planning, funding and service integratin. This requirement is als reflected in the 2017/18 mandate letter t the Central East LHIN frm the Minister f Health and Lng-Term Care. The mandate letter ntes that thrugh sub-reginal (cmmunity level) planning, LHINs will identify hw prviders will cllabrate t address health gaps, and imprve patient experience and utcmes. Lastly, the Ministry-LHIN Accuntability Agreement (MLAA) calls fr the definitin and implementatin f sub-reginal structures within LHINs, including the establishment f clinical and peratinal leadership within sub-regins t drive lcal perfrmance imprvements. Sub-regin planning builds n the Central East LHIN s strng fundatin f lcal engagement and planning, including Health Links, and will als advance the strategic aims and direct care pririties f the Central East LHIN s Integrated Health Service Plan 4 (IHSP4) Living Healthier at Hme. T that end, the Central East LHIN has established Sub-regin Planning Tables t fster jint accuntability fr innvative, integrated system redesign t address health and service gaps, advance quality, and imprve patient experience and utcmes. The Planning Tables will build upn the strng, cmprehensive fundatin laid dwn by the Health Links in rder t further advance the apprach t Crdinated Care Plans and physician attachments. The Planning Tables will allw the full cntinuum f stakehlders frm diverse sectrs t cme tgether with patient and caregiver representatives t imprve the health f the ppulatin within a sub-regin gegraphy. This apprach will drive imprvements which are meaningful t the lcal cmmunity and reflective f its needs. Sub-regin Planning Tables will empwer individual rganizatins and sectrs t engage in cllabrative planning fr a lcal ppulatin within a defined gegraphy. Specific sub-regin pririties will be infrmed by the patient and caregiver perspective and lcal health system data. Ultimately, the Sub-regin Planning Tables will prvide recmmendatins t the Sub-regin Steering Cmmittee fr innvative and integrated strategies t address lcal health gaps, fllwing the established funding cycles f the LHIN. The purpse f the Sub-regin Steering Cmmittee will be t prvide strategic Sub-regin Steering Cmmittee 1

2 directin and versight t supprt the wrk f the Planning Tables by advancing Planning Table recmmendatins t the Central East LHIN. In making these recmmendatins, the Steering Cmmittee will cnsider the brader health system cntext acrss the varius gegraphic clusters. This grup will als ffer recmmendatins fr lcal Sub-regin investment and reallcatin f savings/surplus, and d s fllwing the established funding cycles f the LHIN, and the Institute fr Healthcare Imprvement (IHI) Quadruple Aim apprach (Patient Experience, Ppulatin Health, Reducing Cst, and Care Team Well-Being). The Steering Cmmittee will bring tgether executive stakehlders frm varius health service prviders and sectrs, such as primary care, crss-ministry partners, public health, and municipal services. Sub-regin planning within the Central East LHIN will be anchred in the visin, missin and values f the Central East LHIN. In additin, the fllwing cre principles will guide the wrk f the Sub-regin Planning Tables: Inclusivity (f thse impacted): Planning and actin must be infrmed and guided by the patient and family caregiver experience. The develpment and maintenance f effective wrking relatinships acrss the full cntinuum f stakehlders within a sub-regin (e.g., gvernment funded, private, nn-prfit and service entities) will prvide a fundatin fr cllabratin and success. Learning Systems (within sub-regins): Fstering increased knwledge f ppulatin health needs and understanding service capacity is an essential and n-ging prcess t infrm, assess and enable actin, imprving the health and well-being f peple patients and ppulatins. Creating a learning system will drive imprvements by integrating data fr decisin-making int rganizatinal systems and drawing lessns frm small-scale tests f change t imprve safety and quality within the sub-regin. Sustainability (f system): While lcal capacities must be recgnized (human, financial, technlgical, ther resurces), cnsistency f prcesses within and acrss sub-regins shuld be pursued prir t custmizatin. Mnitring system perfrmance and n-ging quality imprvement effrts will cntribute t sustainability and the spread f best practices. Transfrmatinal Leadership (at lcal level): Sub-regin (lcal) leaders are best able t transfrm their lcal system by identifying, infrming and champining innvative, integrated strategies t address health and service gaps, advance quality, and imprve patient experience and utcmes. 3 Equity (fr all): 1.2 Scpe Bringing health care equity t the frefrnt in planning, decisin-making and actin within sub-regins will supprt excellent care fr all. Safety (f care): Underpinning sub-regin planning will be the delivery f innvative care in safe envirnments, incrprating patient/caregiver and prvider perspectives. Quality (f care): Health quality is best shaped by understanding the experiences and wisdm f patients, families, caregivers, and the public. The mandate f the Steering Cmmittee is t supprt the accmplishment f the Sub-regin Planning Table mandate t prvide lcal leadership and jint accuntability fr innvative, integrated system redesign t cllectively address health and service gaps, advance quality, and imprve patient experience and utcmes within a sub-regin gegraphy. Rles and Respnsibilities 2.1 Rle f the Sub-regin Steering Cmmittee Members The purpse and mandate f the Sub-regin Steering Cmmittee are utlined in Sectin 1 abve. This grup will versee and supprt the wrk f the Planning Tables (and select Wrk Grups) by reviewing and advancing Planning Table recmmendatins t the Central East LHIN, factring in the brader cntext f the varius gegraphic clusters. Members are expected t abide by the Central East LHIN Cde f Cnduct. Members are expected t engage in healthy debate leading t psitive imprvement. Members are expected t transcend their representative status (when they represent a specific type f service, sectr, care, r vcatin) in favr f the system. Members are selected fr their ability t cntribute t this mandate. Members are equal. Rle f C-Chair: The Sub-regin Steering Cmmittee C-Chairs will wrk t develp agendas and facilitate meetings. The C-Chairs will act as the spkespeple fr the Steering Cmmittee with the supprt f the Central East LHIN Cmmunicatins team. 2.2 Respnsibilities f the Sub-regin Steering Cmmittee Members The respnsibilities f the Steering Cmmittee are t versee and supprt the wrk f the Planning Tables and select Wrk Grups. This will enable lcal change thrugh the implementatin f innvative, integrated strategies t address health and service gaps, advance quality, and imprve the patient experience and patient utcmes. T accmplish these deliverables, Steering Cmmittee members are respnsible fr the fllwing tasks: Maintain a system- and ppulatin-wide perspective during deliberatin and decisin-making; Sub-regin Steering Cmmittee 4

5 Cnsider fremst the health and scial care systems and what is best fr patients and ppulatins; Bring frward wn and sectr/rganizatin perspectives in a nn-biased and respectful manner; Declare perceived and actual cnflicts f interest related t self r ther members in advance, t the C-Chairs and/r membership; Supprt fulsme, accurate and apprpriate cmmunicatin with peers and within wn rganizatin r persnal circles f influence; and Individual members will remain accuntable t their wn respective Bards fr achievement f wn/agency-specific accuntabilities. 3. Membership f Sub-regin Steering Cmmittee 3.1 Membership The Sub-regin Steering Cmmittee is an versight bdy f the Planning Tables, cnsisting f a decisin-making, actin-riented team f 8-10 members. C-Chairs: Vice President, Clinical, and Vice President, Health System Strategy, Integratin, Planning and Perfrmance Membership: 1. Patient and Family Advisry Cmmittee C-Chair 2. Central East LHIN Chief Executive Officer (CEO) 3. Vice President, Hme and Cmmunity Care 4. C-chair representatin frm each Sub-regin Planning Table (rtating schedule f either System and Sub-regin Directr r Sub-regin Primary Care Lead) 5. Directr, Strategic Cmmunicatins and Stakehlder relatins Other key Central East LHIN staff will supprt the Sub-regin Steering Cmmittee as apprpriate (e.g. Decisin Supprt). 6 3.2 Reprting Relatinships The Sub-regin Steering Cmmittee will serve as an advisry bdy t the Central East LHIN; The Sub-regin Planning Tables will prvide recmmendatins t address lcal health system pririties t the Sub-regin Steering Cmmittee; Wrk Grups may have jint accuntability t the Sub-regin Planning Tables and the Sub-regin Steering Cmmittee, depending upn the lcal/system pririty r area f fcus; The Central East LHIN System and Sub-regin Directrs and/r Sub-regin Physician Leads will wrk with the Sub-regin Steering Cmmittee t adhere t the mnitring and reprting expectatins as set ut by the LHIN and/r prvince; and Members f the Sub-regin Steering Cmmittee will prvide regular updates t their respective gverning bdies (e.g., health service prvider Bard f Directrs). 3.3 Duratin f Service It is recgnized that a lnger term is necessary t accmmdate the time required t develp relatinships, prcesses, tls and plans. As such inaugural members f the Subregin Steering Cmmittee will be appinted fr a tw r three-year term, subject t review and mutual agreement t cntinue at end f year ne. Members will be eligible t serve tw terms, and these terms need nt be sequential. Terms will be staggered t supprt successin planning. 4. Lgistics and Prcesses 4.1 Frequency f Meetings The Sub-regin Steering Cmmittee will generally meet bimnthly and n fewer than six times per year. Additinal meetings will be held at the call f the C-Chairs. 4.2 Decisin-Making Prcess Whenever pssible, the Sub-regin Steering Cmmittee members will make decisins by cnsensus. Where cnsensus cannt be reached, a vte can be called and qurum requirements will apply. Unreslved matters may be referred t the Central East LHIN CEO fr decisin. Sub-regin Steering Cmmittee 6

7 4.3 Qurum Requirements All Cmmittee members are required t attend Sub-regin Steering Cmmittee meetings. T cnstitute a frmal meeting, a majrity f members must be in attendance. 4.4 Review The Terms f Reference will be reviewed annually by the Central East LHIN and updated as required t reflect mdificatins r additins.
