Sealed quotations should be addressed to The Medical Superintendent, ESIC Hospital, Ezhukone. The completed quotations shall be deposited in the tender box provided at the hospital office on or before the scheduled date and time. Last Date of submission shall be 24/06/2015 till 1.00 PM. Quotations shall be opened on the same day at 2.30 PM in the presence of representatives of the centres, if available. Quotations received late shall not be accepted under any circumstances. The undersigned certify that I have gone through the terms and conditions mentioned in the instruction supplement and undertake to comply with them & the rates quoted by me/us are valid and binding on me/us are for acceptance of the contract of Lab/Scanning Investigations for the period of one year from the date of contract order. 2. It is certified that rate quoted are the lowest quoted for any institution/ Hospital in India. 3. Earnest Money deposited by me/ us viz Rs (Rupees..)(EMD as noted in tender terms and conditions) in the form of Demand Draft/ Banker s cheque with no dtd in favour of ESIC fund Account No. 1, Ezhukone, Kollam is attached herewith. 4. I/ We give the rights to Medical Superintendent, to forfeit the earnest money deposited by me/ us on any breach of the tender terms and conditions. 5. 5 5 Terms and conditions : 1. The investigation centre shall do all the tests / investigations mentioned in the Annexures A/B for a period of one year from the date of contract agreement at CGHS rates applicable from time to time or rates quoted by the firm whichever is lower. The rates of unlisted tests shall be applicable either as per CGHS/AIIMS rates. If any tests not available in CGHS/AIIMS rates, then rates shall be calculated on the rates provided by the tenderer in their tariff list after deduction of 15%. 2. No change in rates shall be allowed after placing contract orders by the hospital. After placing contract orders if the centre hesitates to do the tests at the CGHS rates or making any change in the quality of the tests, their EMD/ security deposits shall be forfeited at any stage of the contract. 3. Those centres who cannot abide by the CGHS rates need not apply. 4. An EMD of Rs1,00,000/-( Rupees One lakh Only ) for Scanning Investigations & Rs.30,000/-(Rupees Thirty Thousand Only) for Lab investigations by way of DD drawn in favour of ESIC fund Ac No. 1 payable at SBI, Keralapuram has to be enclosed with Lab/ Scanning investigations bids separately. After placing contract orders, the contractors shall pay security deposit as per the rules based on the contract value. The contract value will be based on hospital s previous year s expenditure on the same. 5. The contract order will be placed only after inspection of the centres by the ESIC team. 6. The investigation centre will provide all the investigations as per tie up list on cashless basis to ESI beneficiaries/ staff referred by the hospital. An agreement has to be executed by the centre with ESIC Hospital, Ezhukone after getting contract order. 7. The investigation centre shall provide investigation facility only to referred beneficiaries ( as per proper prescription/ reference from doctors of this hospital. ). Identity of the patient is to be confirmed in each case before conducting the investigation with reference to the photo affixed pehchaan card issued by the ESIC. 8. The investigation centres should be willing to do the tests before or after the normal working hours and on holidays. The centre, should have facilities for transporation of the patient to and fro. 8.The results shall be furnished and issued to ESI beneficiaries at the earliest. Turn around time regarding each test should be submitted and undue delay in performing investigations and issuing reports would invite penalty action. The investigations should be done on the same date of reference and reports should be furnished within normal turn around time. Delayed performance of investigations shall attract penalty as the patient shall be sent to other centres at the contractor s own risk and expense. The extra expenditure incurred shall be levied from the contractor, from the subsequent bills/emd/security deposit. Repeated delay/ compromise in quality of the tests shall lead to cancellation of contract / forfeiture of security deposit. 9. The investigation centre should agree that any liability arising due to any default/ negligence in providing proper services shall be borne, by the investigation centre and shall alone be responsible for that. 10. Appropriate action will be initiated by the ESIC on the basis of complaints/ delay /medical audit or inspections carried out by the ESIC team. 11. Cash bill for investigations are to be submitted by the centre on monthly basis. Bills are to be submitted with proper prescription and a copy of the report. 12. TDS shall be deducted as per income tax rules.

The investigation centre has to ensure that the reference for costly investigations ( costing above Rs.3000/- each and certain specified investigations )are authenticated by the Medical Superintendent of the ESIC Hospital, Ezhukone and that eligibility certificate has been produced in such cases. 14. Tests that have been awarded to the centre and could not be done under the centre s facilities should be send to labs with National or International certification / accreditation at centre s own arrangements. In that case also the change in CGHS rates will not be allowed. QC chart for the data with instrument print out has to be given for the tests done in centre s own facility wherever applicable / required. The investigation centre must undertake to collect the sample from the hospital each day, without fail besides responding to any emergency requirements. 15. All the sealed quotations submitted must be superscribed with Quotation for.. ( must specify whether Lab or Scanning Investigations ) 16. Any breach of these terms found may attract forfeiture of EMD/security deposits. 17. The Medical Superintendent reserves the right to accept or reject any or all of the quotations without assigning any reasons thereof. All the decisions of the Medical Superintendent pertaining to the contract/tender shall be final. 18. NABL/ISO certification preferable. 19. Minimum two years experience of providing (full 24 hrs ) Lab/ Scanning services support to atleast one 100 bedded hospital under Central Govt./State/ PSU/ESIC is required for selection. 20. A status report on the infrastructure facilities available with the tenderer in support of his technical capability to execute the quoted lab investigations must accompany the tender highlighting details of equipment, type of procedure, Quality Control measures ( including document proofs ) and automation in various sections of laboratory. The tenderer will be liable for all types of responsibilities including medico legal cases also. 21. Certificate of registration of the firm and the registration for rendering medical laboratory/scanning investigation services with appropriate Govt. authorities must be enclosed with the tender. 22. ESIC being a statutory & autonomous body established under ESI Act 1948, is subject to all orders/instructions/guidelines issued by Govt. of India and ESI Corporation. All such orders/instructions/guidelines shall also form part of this tender / contract. 23. Any disputes, in this regard shall be under the jurisdiction of Kollam court. Sd/- MEDICAL SUPERINTENDENT
