The prescribed tender documents with the terms and conditions may be obtained from National Training Academy, II Floor, N.R.P.O Building, EPFO Complex, Sector 23, Dwarka, New Delhi between A.M to P.M on any working day except Saturdays & Sundays against the payment of Rs. 100/-( Rupees One Hundred Only) through demand draft issued in favour of ESIC Fund, A/c No. 1 payable at Delhi/or can be downloaded from the official website of ESIC ( alongwith DD in favour of ESIC Fund, A/c No. 1 payable at Delhi. The interested agencies should submit their quotations in Prescribed Format for Technical Bid (Annexure A ) and Financial Bid (Annexure B ) in sealed envelope in two parts viz. Technical & Financial separately along with EMD of Rs /- (Rupees Fifteen Thousand Only) on or before till Hrs. Both the sealed envelopes should be placed in another sealed cover superscribing it Tender for Hiring of Staff Car and shall be dropped in tender box kept at National Training Academy, II Floor, N.R.P.O Building, EPFO Complex, Sector 23, Dwarka, New Delhi 77. The Technical bids will be opened on the at 15.00Hrs. in the presence of the Tender Committee and in the presence of the parties or their authorized representatives, if, any, who wish to participate in the National Training Academy, II Floor, N.R.P.O Building, EPFO Complex, Sector 23, New Delhi The Financial bid of only those agencies who qualify in the Technical conditions will be opened subsequently. The Insurance Commissioner NTA reserves the rights to postpone and / or extend the date of receipt/opening of quotations or to withdraw the same, without assigning any reason thereof. In addition of above, interested agencies may also give their quotations for hiring of the Contd..p/2

2 -2- above mentioned category of cars on daily basis as the National Training Academy conducts regular training programmes and required services of taxis on daily basis for trainees and faculty during training programmes. 5 OTHER TERMS AND CONDITIONS OF THE TENDER NOTICE 1. The vehicle should be registered in Delhi/NCR 2. The firm should be a registered firm/company/proprietorship. 3. The order for providing taxi on monthly basis will be given to the agency which has quoted the lowest rate while meeting all terms and conditions given in the tender document. However, to safeguard against failure by this agency to provide desired vehicle, the Head of the office may empanel other tenderers who are prepared to provide the services as per same terms and conditions as that of L-1 (firm who has quoted lowest rates) tenderer. 4. In case the quality of service by the contract agency is found wanting, the Head of the office may terminate the contract agreement after giving 15 days notice. In case of termination, services of the agencies from the panel will be utilized. 5. The vehicle should be properly and comprehensively insured and should carry necessary permit/clearance from the Transport Authority and/or any other Authority including pollution certificates. 6. The vehicle shall be for the exclusive use of the National Training Academy, ESIC and will not be used by the Contractor for any other purpose. The engagement and employment of the drivers and payment of wages to them as per existing provisions of various labour laws and regulations is the sole responsibility of contractor and any breach of such laws or regulations shall be deemed to be breach of this contract. 7. The driver deputed on duty should not be involved in more than two punch or challan for negligence driving. The firm should ensure that they have arrangement for establishing contact and round the clock service. The drivers employed should carry a mobile no. and valid Driving Licence. 8. National Training Academy, ESIC, Dwarka, New Delhi shall not be responsible for any challan, loss, damage/or any other accident of the vehicle or to any other vehicles or for the injury to the driver or to any other third party. The loss or damage or legal expenses on this account shall be borne by the Tenderer. 9. The monthly bill shall be submitted by the Tenderer in duplicate duly supported by the copy of Logbook. 10. The Tenderer should have the PAN Card/TAN and should attach a photocopy of the same. 11. The calculation of mileage should be from the reporting point to the relieving point and will not be calculated from garage to garage basis. 12. The Tenderer should give an undertaking that he/she or his/her firm has not been blacklisted by any of the Organisation/Government Department as on date of submission of the bid. 13. The Tenderer shall also clarify that he/she has read and understood the terms and conditions of the tender. He/she should sign the terms and conditions. The tenders, which are not accompanied by the requisite documents, shall be out rightly rejected. 14. The agency shall ensure that the odometer of car supplied is properly sealed so that no tampering is done with a view to inflate distance travelled. 15. The authorized officers of the NTA, ESIC, Dwarka, New Delhi may get surprise checks of odometer of the car supplied from any authorized workshop and cost thereof will be borne by the agency. 16. The vehicle will be kept in perfect running condition with full fuel everyday during the period of hiring by this office. 17. The service provider shall undertake to indemnify the department against all damage/charge arising on account of or connected with the negligence of the service provider or his staff or any person under his control whether in respect of accident/injury to the person or damages to the property of any member of the public or any person or in executing the work or otherwise and against all claims and demand thereof. 18. There should be at least two sets of white covers, towels and napkins. It should be changed every week.

6 19. There should be an air spray in vehicle. 20. In case of any breakdown in vehicle on duty, the agency shall make arrangement for providing another vehicle. In such a case, mileage from garage to breakdown point would not be paid. 21. Toll tax and parking charges shall be reimbursed by the NTA, ESIC, Dwarka, New Delhi against production of documentary evidence. 22. All expenses relating to salary and allowances of the driver, overtime payment, maintenance of vehicle, fuel oil or any other expenditure related to the vehicle and the driver will be borne by the Service provider/contractor. 23. The Driver should be well conversant with road of Delhi/NCR. The operation and functions of the Driver shall be governed as per Motor Vehicles Act and Rules. 24. The Driver should always remain with the vehicle during entire period of duty. In case of any emergency, the driver may seek permission of the concerned officer., 25. Vehicle shall be made available on all days including Saturdays, Sundays & Holidays, in case of requirement. 26. The agency shall provide Name and address of the drivers. The agency shall ensure that the persons employed should not have any adverse Police records/criminals cases against them. The Agency should make adequate enquiries about the character and antecedents of the persons whom they are deputing. 27. The contract will be for a period of one month on trial basis which may be extended for further period on satisfactory service basis. 28. LPG Cylinders should not be used for running the vehicle in any case. 29. The agency/firm should have an adequate number of telephones for contact round the clock. The driver shall observe all the etiquette and protocol while performing the duty and shall be neatly dressed, should wear proper uniform & must carry a mobile phone in working condition, for which, no separate payment shall be made by the NTA. 30. National Training Academy, ESIC, Dwarka, New Delhi will have the sole discretion to extend the period of contract beyond the period of six months or terminate the contract permanently in case of unsatisfactory service. 31. A daily record indicating time and mileage of the vehicle shall be maintained in a Log book and the Log book shall be submitted to the concerned officer of National Training Academy, ESIC, Dwarka, New Delhi regularly for scrutiny. 32. Once the hiring of the cars commences from a particular operator, the car and driver should not be changed unless requested by the NTA, ESIC, Dwarka, New Delhi. 33. If any vehicle does not report for duty on day(s) then an amount calculated on prorata basis per day shall be deducted from the monthly bill of the vehicle. In addition, a penalty of Rs. 500/- per day may be levied if any vehicle/agency fails to meet above terms and conditions on any day. 34. Any matter during the period of this agreement, which has not been specifically covered by this agreement, shall be decided by customer (NTA, ESIC, Dwarka, New Delhi) whose decision shall be final and conclusive. 35. No additional terms and conditions over and above the conditions stipulated above shall be entertained by NTA, ESIC, Dwarka, New Delhi. 36. In case of any dispute of any kind and in any respect whatsoever, the decision of Insurance Commissioner, National Training Academy, ESIC, Dwarka, New Delhi shall be final and binding. 37. National Training Academy, ESIC, Dwarka, New Delhi reserves the right to fulfilment of other conditions, not expressly mentioned, which are consistent with use of vehicles on hire with this office. 38. Service Tax will not be reimbursed, if the service provider fails to produce proof of payment of Service Tax. 39. If the contract is awarded, the Service provider shall provide the National Training Academy, ESIC, Dwarka, New Delhi full details of the deployed driver, their address and copies of their driving license.

Earnest Money Deposit(EMD) The bidder shall deposit and amount of earnest money of Rs /-(Rupees Fifteen Thousand Only) by way of demand draft in favour of ESIC Fund, A/c No. 1, payable at Delhi. 41. The successful bidder shall be required to deposit an amount of Rs. 30,000/- (Rupees Thirty Thousand Crore Only) by way of demand draft in favour of ESIC Fund, A/c No. 1, payable at Delhi within 15 days of issue of letter of intent as Performance security.
