2 Serious Injury and Fatality Reduction Initiative Over the years S&H efforts have been based on the Heinrich triangle. It makes sense that if you eliminate the chance of a hit you cut down the number of events. If you cut down the number of events you eliminate the chance of a serious event.
3 Serious Injury and Fatality Reduction Initiative We know that this works for the most part, but also noticed one interesting fact. While we were seeing essentially the elimination of fatalities, many medical injuries, and even many of the less severe lost time/restricted duty events, we noticed that some of the more serious LT/RD events were still occurring.
4 Serious Injury and Fatality Reduction Initiative So we decided to look more closely at these more serious events and decide how we can address them and work them into an overall preventative effort for the Company.
5 SIF Reduction Plan The focus of this Plan: Establish a Company wide definition for a Serious Incident Fatality NOTE- we utilize an actual severity level of the event or a potential severity level for the event- our intent was to identify those Near Misses and Hazard Recognition events that could have been serious if the balance went the other way Educate leadership and in turn all employees on the new paradigm for serious injuries and fatality prevention
6 SIF Reduction Plan The focus of this Plan: Through investigation we then would analyze our data for the identification of precursor situations and activities likely to result in serious injuries/ fatalities In turn, we would implement controls to mitigate these precursors through already incorporated tools such as our EHAP and HP Tool- Job Map The expectation is that each Business Unit would incorporate as needed this SIF Reduction Plan into their site-specific Safety & Health Plans
7 Institutionalizing SIF Metrics/ Educate Leadership Overall approach utilized for the Plan: Implementation date of June Series of announcements, education, and training prior to the official roll-out S&H Professionals would classify those events that occurred prior to the June 1 date so that the entire calendar year was included Continual review by S&H Professionals with their respective Business Unit. Quarterly review at the Executive Committee level At this point in time we have not set a goal. Our intent is to have the attention placed on the occurrence as opposed to a pre-set goal
8 Injury Record in Database Just a Yes or No distinction in our database, but it is in the analysis that we get the information we require 8
9 Business Unit Plans 2013 remaining actions: Work with business units to include SIF Reduction Initiatives in 2014 Safety Improvement Plans. Analyze business unit specific data to provide input to strategies based on data analysis. Ensure that business unit governance committees incorporate SIF reduction initiatives into 2014 Safety Improvement Plans Incorporate review of business unit SIF Reduction Initiative implementation into EHS compliance audits.
10 Definition of SIF Serious Injury/Fatality (SIF): Work-related injury that results in one of these outcomes: A fatality, life-altering injury or illness that causes a permanent and/or total disability to the individual where the actual or potential severity is a five or six. (This includes amputations and/or deforming events) A fire, explosion or chemical exposure where the actual or potential severity is a five or six Fall from a height of four feet or greater or an event where the fall arrest system is activated 10
11 Definition of SIF Serious Injury/Fatality (SIF): definitions continued: Failure to observe a business unit s Life Saving Rule Electrical contact or flash where the actual or potential severity is a five or six A fire, explosion or chemical exposure where the actual or potential severity is a five or six Dropped or falling object where the rigging used (or failure to rig) was a part of the event. This would include crane and/or digger derrick failure 11
