2 Risk assessment why? Reduced likelihood of the occurrence of hazardous events/exposures Reduced consequences in terms of adverse effects on yourself and your own health, but also on the surroundings Increased knowledge of your own activities Increased ability to identify hazardous events/exposures Makes it possible to control activities Increased focus on safety in occupational and leisure situations Systematic focus on safety Documentation Contribute to a good company reputation The requirements of the law and other official requirements

3 Risk - definitions Combination of the likelihood of an occurrence of a hazardous incident or exposure(s) and the severity of injury or ill health that can be caused by the incident or exposure(s) Risk = The probability of the incident occurring x expected loss in case of the incident Risk is a description of possible losses that MAY occur in the future The loss may occur, but the outcome may also be desirable

4 Danger/threat Often related to energy Crane Exposed to risk (M Rausand)

5 Rupture Undesirable incident A considerable deviation from the normal situation Crane Exposed to risk

6 Crane Accident Injury to human, material damage, environmental damage

7 Crane Near accident/miss

8 Risk assessment These questions need to be answered: What undesirable incidents may occur (hazard identification)? What is the likelihood that the undesirable incident will occur? What are the possible consequences of the undesirable incident? What actions may be taken to reduce the risk? Fire Low Very critical??

9 The NTNU method Define a group of relevant participants Use the HSE guidelines and forms: Guideline (HMSR26) only in Norwegian Hazard Identification form (HMSRV2601E) with explanations (HMSRV2602E) Risk assessment form (HMSRV2603E) HSE action plan (HMSRV1224E) Before starting new activities When established procedures are altered Regularly every 2-3 years

10 Hazard identification Define what activities to assess (e.g. all activities in a room, all parts of one single process ) Use the hazardous activity identification form ( to split the activities into segments, either systematically or by brainstorming An inspection of the room/process to be assessed may be useful

11

12

13 Risk assessment Transfer the activities identified from the hazardous activity identification form to the risk assessment form Describe the different undesirable incidents that may occur in connection with each hazardous activity Assign values describing the likelihood (values 1-5) and consequence (values A-E) to each undesirable incident and enter the product of these values as the risk value Important parts of the assessment: Review of local instructions, previous risk assessments and other existing documentation Are all regulations, procedures and instructions implemented? Discussion with managers, persons who are familiar with the activity in question and other relevant persons

14

15

16

17 Supplementary work Plan of action Verify the risk values, make necessary adjustments Review and prioritize the actions planned to reduce risk. Actions reducing the likelihood of an undesirable incident must be prioritized before actions affecting the consequences following the undesirable incident. Put the prioritized actions into the plan of action ( Assess whether the residual risk is acceptable Make or revise local instructions, regulations, and documents for training according to the result of the risk assessment
