1 Pet Isurace Policy Summary This documet is a summary of your policy. The full terms ad coditios of your policy ca be foud i the isurace booklet ad your isurace schedule. The isurace policy is uderwritte by Liverpool Victoria Isurace Compay Limited. Policies are admiistered by BDML Coect Limited (Registered No ). Type of isurace ad cover LV= pet isurace is available for ay dog or cat that has bee see by a vet i the 12 moths before the policy start date, is over eight weeks of age ad does t fall uder the restrictio of the Dagerous Dogs Act 1991, the Dogs (Norther Irelad) Order 1983, Dogs (Norther Irelad) Act 1991 or ay chages to those laws. We have two policies. The policy you chose is show o your schedule. The Essetial policy covers veteriary treatmet for each illess or ijury for 12 moths from the start of the illess or ijury up to 5,000. Cover for each illess or ijury will cotiue for a full 12 moth period or up to 5,000 (whichever happes first) provided we offer to cotiue the policy at the reewal or aiversary date ad you pay the premium. The Premier policy covers veteriary treatmet for all illesses, ijuries ad behavioural problems up to 10,000 i each (12 moths). The 10,000 limit is ot for each coditio but is the total we will pay for veteriary treatmet i ay 12 moth. Provided we offer to cotiue the policy ad you pay the premium the 10,000 limit will be available agai from the reewal or aiversary date. Your isurace schedule will show ay edorsemets such as special terms, coditios or restrictios that apply to your policy. The LV= pet policy is oly available to people livig i Eglad, Wales, Scotlad, Norther Irelad, Isle of Ma ad the Chael Islads. The policy covers your pet i the Uited Kigdom ad i listed coutries uder the Govermet s Pet Travel Scheme, other tha for liability, which is oly covered i EU listed coutries. 1

2 What is the period of cover? Aual policy If you pay the aual premium the policy will be i force for 12 moths from the date it starts. It must be reewed for cover to cotiue. Mothly policy If you pay mothly, the policy will be i force each moth we receive the premium util it is cacelled. The premium ad policy terms may be chaged after each 12 cosecutive moths, or if there are ay chages i legislatio that impact your premium (eg a icrease i IPT), we will let you kow at least three weeks before your mothly paymet is due. The level of cover you have for related, recurrig, ogoig ad bilateral coditios If your pet has coditios that: occur more tha oce are ogoig affect more tha oe part of the body affect the same part of the body but o the other side of the body we will treat these as oe sigle coditio ad this icludes how we apply the veteriary fees excess, regardless of how much time has passed betwee occurreces or whether the cause of the coditio may be differet. Please refer to the documet of isurace page 11 for more iformatio about this. Please ote your policy does ot cover the followig: ay veteriary fees uless your pet has bee see by a vet i the 12 moth period before the policy start date ay evet, illess, ijury, disease or chages to your pet s ormal healthy state that your pet shows sigs of before the start date of the policy ay claim because of a ijury that happes i the first 48 hours followig the start of your policy ay claim because of ay illess, disease or behavioural problems that happes durig the first 14 days followig the start date of your policy ay paymet for a claim (other tha for legal liability) if we receive coflictig iformatio ay loss, damage or liability that is also covered by ay other isurace, i these circumstaces we will oly pay our share coditios that are ormally protected agaist by a vacciatio, uless your pet has had the vacciatio. For dogs these coditios are: distemper, hepatitis, leptospirosis or parvovirus. For cats the coditios are: felie ifectious eteritis, felie iflueza or felie leukaemia pets used for breedig, guard, security, racig or commercial purposes uless you have told us ad we have agreed to provide cover ad this is show o your policy schedule aythig caused or related to your pet goig to ulisted coutries of the Pet Travel Scheme ay claim caused by or to your pet because it disturbs or ijures livestock 2

There is o cover for ay prevetative or o essetial treatmet eg vacciatios, flea treatmet, wormers, ail clippig, cosmetic surgery or treatmet, detal de-scalig/ cleaig. There is o cover for spayig or castratio or for aythig related to pregacy or givig birth. We wo t pay the cost of ay aalysis or test uless your vet cofirms this is eeded i order to kow how to treat your pet. We wo t pay extra charges for your pet to be see by a vet outside of ormal practice hours or to have a home visit uless your vet explais why this was essetial. If your pet is less tha six years old we wo t pay for blood tests to be doe before a aaesthetic or procedure uless your vet provides evidece of why this was ecessary ad what alterative treatmet would be give if the results cofirmed a icreased risk. There is o cover for food uless it is used istead of a specific medicatio. We wo t pay for ay costs you have to pay to get your pet referred to or from aother vet or place of treatmet or therapy. We wo t pay ay costs for you to get a secod opiio uless we have expressly agreed to them before you go. We wo t pay the cost of ay trasplat surgery icludig pre or post-operative care. We wo t pay costs related to your pet beig put to sleep, ay post mortem, or for your pet to be cremated or buried. 5

We wo t cover ay liability covered by ay other isurace. Cover for the amout you paid for your pet (or the amout show o your schedule if less) if it dies as a result of a illess ad is less tha 9 years of age (for a dog) or less tha 11 years of age (for a cat). We must see evidece of the price you paid for your pet. Cover for the amout you paid for your pet (or the amout show o your schedule if less) if it dies as a result of a accidet. If your dog is 9 years of age or more or your cat is 11 years of age or more, your vet must cofirm the cause of death. If your pet dies at the scee, we must also see cofirmatio of the circumstaces from a idepedet witess. We must see evidece of the price you paid for your pet. 5. Advertisig ad reward 6. We wo t pay ay costs for ay hospital stay that was likely to be eeded o the start date of the policy. 8. Holiday cacellatio Cover for lost holiday costs if you have to cacel your holiday withi seve days of goig, or cut it short, because your pet eeds emergecy lifesavig treatmet. 9. Accidetal damage to third party property - Premier policy oly There is o cover for aythig caused by somethig you were aware of before bookig or goig o your holiday. Cover (available oly o the Premier policy) for accidetal damage to someoe else s property while visitig someoe else s home. We wo t pay if there is o-oe with the pet who is over 18 years of age. We wo t pay for ay damage caused by your pet foulig, vomitig or uriatig. We wo t pay for damage to property if it is owed or beig looked after by you, your family or household, ayoe employed by someoe i your household, or by ayoe travellig or stayig with you o a trip. 10. Paymet towards quaratie keel costs if your pet s microchip fails or your pet has to go ito quaratie because of illess. We wo t pay if the microchip was ot foud to be workig withi 30 days of your departure or for ay illess or ijury your pet was sufferig from before you left the UK. Cover for costs towards gettig the ecessary paperwork to re-eter the UK because you lose your pet passport or your pet s microchip fails. We wo t pay if the microchip was ot foud to be workig withi 30 days of your departure or you lost the pet passport before goig o the trip. 7

Cover for costs towards additioal trasport ad accommodatio if: your pet is t well eough to travel back as scheduled your pet goes missig Your rights to cacel your isurace you ve lost the pet passport or your pet s microchip fails ad you miss your scheduled departure home while gettig ecessary replacemet paperwork We wo t pay for additioal costs for you to stay overseas uless your pet goes missig i the last three days of your trip. We ll oly pay for additioal costs for a maximum of four days from the date your pet goes missig. If you lose your pet passport or your pet s microchip fails, we wo t pay more tha we would have had to pay if you had retured to the UK as scheduled ad your pet was put i quaratie keels while the required paperwork was completed from the UK. We wo t pay for the cost of gettig your pet home if it should die while you re o a trip. You have up to 14 days to cacel your policy after you receive your documets If the cover does t meet your eeds, you ca cacel your isurace by tellig us. We ll the refud ay moey you ve paid, less a charge for the cover you ve had, withi 30 days of you tellig us that you d like to cacel. More tha 14 days after you receive your documets After the iitial 14 day period, if you ve ot made a claim, we ll refud ay moey you ve paid, less a charge for the cover you ve had. If you ve made a claim, or have a mothly policy, o refud will be paid uless the claim was for the death or loss of your pet. 8

9 Aual policy Cacellatio at reewal We ll sed you a reewal ivitatio three weeks before your reewal date. This will show your reewal premium ad ay chages to your cover. You must cotact us before your reewal date to cotiue with your cover. If you reew but the decide to cacel, provided you tell us before your reewal date, we ll refud what you ve paid i full. If you cacel after your reewal date has passed you will be charged i lie with the cacellatio rules above. Mothly policy Aual review If you have chose a mothly policy we ll cotiue your pet isurace util it is cacelled. We will sed you a aual review letter before the aiversary of your pet isurace policy. This will show your premium for the ext 12 cosecutive moths ad ay chages to your cover. If you decide you do ot wat to cotiue with the cover, please cotact us ad cacel your Direct Debit istructio with the bak. We reserve the right ot to ivite reewal of your policy or ot to cotiue your policy from the aiversary date. If this is the case we ll write to you 3 weeks before the policy ed date or aiversary date. Our cacellatio otice will cofirm the reaso for cacellatio ad be posted ad / or ed to you.
